We currently have an opportunity for a Case Manager to join our team in Norwich and New London, CT.
The position is 40 hours/week, Monday - Friday, day shift.
Essential Duties and Responsibilities include:
Interview clients, complete intake information, review records and confer with other professionals to evaluate the client and determine their needs.
Use motivational interviewing, to coach and empower clients toward positive outcomes.
Refer clients to other support services as needed such as medical evaluation and treatment, social services and employment services.
Use professional judgement, in conjunction with any assigned providers, in planning, developing and implementing an effective intervention plan for clients to provide for a continuum of care and to facilitate the most positive clinical outcome.
Assist with treatment plan objectives through exploration of problems, examination of attitudes and feelings, consideration of alternative solutions, and decision making.
Coordinate case management needs to bring services, agencies, resources, and clients together to assist clients with discharge plans and referrals.
Provide crisis intervention services to respond to client's needs during acute emotional and or physical distress.
Provide education and information to individual clients, and groups concerning alcohol and drug abuse, and the available services and resources, i.e. additional treatment options, 12-step and other community-based programs.
Meet as a team with counseling staff or other relevant stakeholders to acknowledge and report on client progress, discuss and modify established treatment plan.
Education and/or Experience:
Associate's degree preferred or knowledge and experience in Substance abuse and community support services. Requires valid driver's license and ability to drive agency vehicle.

Essential Duties and Responsibilities include:
Develops, implements and coordinates service provision, quality management, and utilization review in collaboration with supervisor.

The RTFT Case Manager is responsible for facilitating reunification by providing intensive support and education to families to help achieve a successful and permanent family reunification. Through Therapeutic Family Time with the utilization of the Visit Coaching Model, focusing on, repairing relationships and improving parental functioning is initiated with Family Conferences held at critical junctures during a three stage process. The Case Manager will work closely and collaboratively with families, substitute care-givers and DCF.
The selected individual will be responsible for but not limited to the following Essential Functions:
Provide Therapeutic Family Time/Family Time Services to assigned families who have been identified as needing services, including but not limited to parenting and child development education, advocacy and support, and assistance with obtaining community resources.
Review/explore safety concerns and risk factors that may impact child safety with the family and DCF
Assess family functioning, skills, parental capabilities, and parent's motivation to change
Implement the Visit Coaching Model
Identify family resources and informal/formal supports with the family and how they may be used in safety planning
Observe family interactions and identify family strengths and needs
Identify problems and barriers that may be impacting reunification
Complete initial North Carolina Family Assessment Scale for General Services and Reunification (NCFAS-G+ R) within 14 days of referral
Utilizes recognized curricula to improve parenting skills and increase parents understanding of normal child growth and development
Develops strength-based service plans in partnership with the parents to address areas identified as challenges or goals
Assists and model for the parents in how to seek and obtain outside resources
Maintains collaborative relationship with substitute caregivers and other community and medical providers as needed
Maintains accurate records of all client and collateral contact including service hours, number of home and office visits, contact with collateral agencies per program requirements
Testifies in court and attends required DCF Family Conference or other meetings
Maintains adequate case-load of families working within the guidelines, policies, procedures, and mission of The Child & Family Guidance Center.

Essential Duties and Responsibilities include:
Assist Clinic staff in coordinating activities of provider and nursing staff relative to medications and the provision of medical and treatment support services.
Maintain responsibility for safe and effective patient care using evidence-based interventions and keeping current with new and proven treatment strategies.
Conduct comprehensive psychiatric evaluations and ongoing medication management
Support clients receiving Medication Assisted Treatment including buprenorphine and, once eligible and trained, methadone
Provide brief therapeutic interventions using short term models
Participate in weekly clinical case conferences and interdisciplinary meetings
Engage actively in clinical supervision and professional development
Assist in the development and implementation of the integrated trauma track including work with complex PTSD

Ensure documentation meets all regulatory and quality standards
Collaborate with psychiatrist as needed to develop the most appropriate and effective psychotropic medication regimen according to the patient's psychiatric history and active symptoms.

SCADD is currently seeking a compassionate Part Time Registered Nurse: Sundays, 8am-12pm, Monday & Tuesday 8am-4pm
Compensation: $42.55/hour (Generous shift differential and weekend premiums!)

We currently have an exceptional career opportunity at our Medically Enhanced Recovery House for a Part Time 20 hours per week Registered Nurse. This innovative model combines the safety of individuals who have medical needs that require on-site nursing support with the supportive environment of recovery housing, representing a bold new step in our strategic vision to expand integrated care and recovery support, expanding our continuum. Essential Duties and Responsibilities include:
Perform initial nursing assessments of clients and document ongoing assessments of clients that are response to treatment.
Perform admissions of clients.
Develop, implement, record, and evaluate nursing care plans according to age and specific needs of clients.
Participate in or supervise the nursing care of clients, providing direct physical care measures as needed.
Act as a primary nurse working with individual clients and coordinating their treatment.
Execute the medical regimen of care as prescribed by the physician.
Educate the client and family toward an understanding of health and illness and the plans for post-discharge care.
Respond to medical and psychiatric emergency needs of clients .
Job Requirements:
Current License in the State of Connecticut - Department of Public Health Registered Nurse (RN)
CPR certification
Prior Psychiatric/Behavioral Health experience preferred
