• Technical Experience: 2 years of experience in providing individual, family and/or group therapy required. Experience in addiction disorders/cross addictions; excellent knowledge of the concepts of stigma, addiction, recovery, and relapse helpful

Role Specific Responsibilities
• Performs concurrent patient assessment related to appropriateness of the level of care, diagnosis, procedures performed, and intervenes to expedite the patient's plan of care.
• Provides consultations to medical staff and other members of the multi- disciplinary team regarding the impact of socio-emotional factors on the patient's illness and intervenes to expedite the plan of care.
• Provides and documents discharge planning services in a professional, collaborative and timely manner to optimize meeting patients' post- hospital discharge care needs.
• Collaborates with the interdisciplinary team to assess, monitor, implement and evaluate patient care needs during the hospital stay in order to smoothly transition the patient to the next level of care.
• Utilizes evidence-based practice standards to guide the evaluation of care, length of stay, medical necessity of services, and appropriate use of organizational and patient centered resources. Medically complex setting determinations may require consultation with the R.N. Case Manager.
• Intervenes on behalf of patients with specific needs not limited to abuse, neglect, mental health issues, end of life issues, ethical concerns, legal matters, financial concerns, and challenges in family dynamics.
• Facilitates and acts as a resource to other members of the healthcare team for functions to include but not limited to guardianship proceedings, adoption, advance directive planning, and emergency detentions.
• Develops and maintains collaborative relationships with organizations in the community that facilitate provision of appropriate care during the hospital stay and facilitate efficient and effective planning for continued care for the patient:
• Collaborates with necessary staff and post-discharge care providers to assure a safe and effective discharge plan; Facilitates care conferences with patient and/or family support structure to foster decision making that promotes patient advocacy.
• Supports Service Excellence initiatives contributing to the organization becoming a top performer in quality care, patient safety and patient satisfaction strategies: Understands and applies techniques to support the provision of population based appropriate care utilizing Joint Commission standards, CMS Core Measure requirements, and other best practices
• Acts as an expert resource to the interdisciplinary healthcare team, physicians and other leaders:
•Participates in interdisciplinary team meetings in a collaborative manner resulting in achievement of best patient outcomes; Participates actively in departmental and hospital wide teams, committees, or other improvement initiatives.

Description
The Social Worker Psych LCSW PNE reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
A licensed, master's prepared social worker who is responsible for developing, coordinating, implementing, and evaluating comprehensive psychiatric social work services in collaboration with the interdisciplinary care team. Provides clinical social work services to patients of the programs of the Stone Institute of Psychiatry in accordance with department and NMH policies and procedures. Integrates the assessment and treatment of patients into the social environment of their family, significant others, and community to maximize the benefits of treatment.
The Social Worker Psych LCSW PNE meets the professional exemption under FLSA; but works an irregular schedule.
Responsibilities:
* Completes biopsychosocial assessment as appropriate to setting in order to guide treatment planning.
* Conducts individual, group and family services specific to the program scope of care, and as indicated by the assessed needs of the patients.
* Participates with members of the treatment team, the NMH network of programs, and community systems to plan and implement treatment, including discharge/linkage.
* Participates in managing funding issues related to patient care.
* Provides education to patients and families and significant others about relevant aspects of mental illness and/or chemical dependency to support maximum utilization of treatment.
* Participates in projects beneficial to the provision of clinical services within SIP and NMH.
* Supervises graduate social work student(s).
* Documents timely, accurate and appropriate clinical information in patient's medical record per documentation standards.
* Collaborates with other departments/programs and external resources as needed.
Additional Functions:
Administration of Work:
* Performs a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances for diverse populations of patients and recommends a plan of treatment based on mutually established goals.
* Provides psychosocial treatment to patients, families and groups, utilizing up-to-date knowledge of clinical, case management, and human service delivery system interventions.
* Utilizes expertise in assessment, diagnosis and treatment of mental illnesses to optimize psychosocial functioning of assigned patients.
* Participates in clinical treatment planning as a member of the interdisciplinary treatment team.
* Educates patients and families about mental illnesses and treatments and provides other information necessary for the patient to participate in treatment.
* Demonstrates knowledge of community resources and an ability to connect patients and families with these resources.
* Acts as an advocate on behalf of patients who require assistance to gain access to needed information, resources, or services.
* Performs the discharge planning function to obtain a timely discharge through early identification, assessment and intervention for post-hospital care needs (inpatient units).

Responsibilities:
Completes Care Coordination Assessment within 24 hours of Order to assist with throughput.
Collaborates with the Interdisciplinary Team to assist with patient satisfaction and discharge plan.
Ensures discharge plan is initiated at admission.
Ensures patient is discharged with appropriate referrals so can successfully move within the healthcare continuum and prevent re-admission.
Is concerned about the perception of patients, families, and the community regarding Northwestern Medicine's Healthcare System.
Services are focused on addressing behavioral, social, financial, and environmental issues in order to achieve optimum clinical quality, patient satisfaction, and cost effectiveness of care.
Assesses patients to gather data for bio-psychosocial assessment upon admission and conducts re-assessments as needed.
Assessments accurately reflect relevant age-specific and cultural-specific factors.
Assists the patient/caregivers/family in understanding and working through bio-psychosocial and financial concerns as they relate to the patients continuum of care, rehabilitation process, behavioral health treatment, substance abuse, or medical treatment.
Facilitates patient/family decision-making process for developing an appropriate treatment plan and coordinates with community organizations in order to ensure effective utilization of services.
Coordinates efforts of patient, family, and healthcare professionals to develop a discharge plan for post-hospital care based on documented bio-psychosocial needs
Provides continuity of care through referrals and linkage to appropriate community resources based on identified needs.
Ensures timely discharge planning and throughput initiatives.
Coordinates post-hospitalization and/or rehabilitation services that may include, but are not limited to home health, home medical equipment, transportation, inpatient behavioral health placement, and nursing home placement.
Determines and complies with payer requirements in order to ensure maximum coverage for services in the continuum of care.
Assesses patients for potential abuse/neglect and need for guardianship, takes appropriate action, and follows-up with appropriate community agencies.
Acts as a patient advocate in all communications with internal and external systems.
Provides education to patient/family in regards to effective use of community resources.
Facilitates interdisciplinary care conferences and provides education on psychosocial issues.
Works collaboratively withpatients interdisciplinary treatment team to identify opportunities for improvement and implement best practice solutions.
Provides brief, supportive psychotherapy to patient and family at bedside when appropriate, to assist with management of symptoms and engage patient in treatment while hospitalized for medical concerns.
Demonstrates knowledge of the principles of growth and development over the life span and possesses the ability to assess data reflective of the patients status. Includes being able to interpret the appropriate information to identify patients age-specific needs
Completes appropriate documentation and facilitates discharge plan and treatment recommendations for patients. Communicates assessment and clinical findings to the interdisciplinary treatment team
Provides psycho-education to nursing and medical staff on mental illnesses and substance abuse disorders.
Acts as a clinical resource for Behavioral Health and Chemical Dependency services to the other service lines within Northwestern Medicine.
Works collaboratively with medical and nursing staff to share necessary information regarding specific patient care issues and to coordinate intervention strategies.
Participates in System wide initiatives on Social Service issues.

A licensed, masters-prepared social worker who is an experienced and independent generalist clinician responsible for complex psychosocial interventions and facilitating the plan of care through discharge planning and resource utilization for all patient populations throughout the Northwestern Memorial Hospital campus.
Proactively collaborates with Patient Care staff, Medical Staff, and other ancillary departments integrating the Department's functions and services with other aspects of the patient care process. The role of the Emergency Department Social Worker involves providing direct face-to-face patient care using modalities such as brief intervention and crisis intervention in a fast-paced setting with the overall goal of reducing hospital admissions and improving access to resources and care.
For each problem, issue, or concern, develops observable and measurable goals and expected outcomes in the areas of problem resolution, utilization and resource management, and patient satisfaction.
RESPONSIBILITIES:
Psychosocial Assessment & Intervention
* Meets directly with patient/family to perform a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment of the patient. Recommends a plan of intervention based on mutually established goals.
* Provides psychosocial interventions which include:
* Reactions to illness and disability, especially the chronically and terminally ill.
* Facilitation of informed decision making (including advanced directives) and development of treatment/intervention plans.
* Adjustment to the hospital setting and compliance with treatment plan.
* Adjustment/coping with post-hospital care needs and linkage to community resources.

* Issues related to insurance coverage and payment.

* Assists in identifying Unidentified Patients, including working with emergency medical services (EMS) and Chicago Police Department (CPD), searching belongings of patient as necessary and talking to family.
* Assists in identifying and notifying emergency contacts of trauma patients.
* Be present when the medical team notifies family of patient death and provides resources and information regarding body disposition as needed.
Manages Discharge Planning through Placement Coordination and Resource Utilization
* Actively participates in the stages of discharge planning and ensures that the plan of care is coordinated, facilitated and effectively communicated to the physician(s), healthcare team, patient and family.
* Coordinate with medical team to assess if patient's discharge needs can be met from the Emergency Department and avoid a hospital admission.
* Provide initial screening for all new patients to assure medical necessity, source of funding, and likelihood of needing social work and/or discharge planning services.
* Serves as the point person for the plan of care as it applies to discharge planning needs through facilitation of direct and continuous communication and collaborative decision-making with medical team
* Coordinates action plans when barriers are present to facilitate resolution.
* Coordinates discharge planning to ensure a timely discharge (placement or return to community) through early identification, assessment and intervention for post-hospital care needs. Patient assessment, plan coordination and changes to the plan occur, as necessary, to ensure that the patient is discharge when medically ready to:
* Other acute hospitals
* Rehabilitative facilities
* Extended care facilities

* Chemical dependency care

* Other living arrangements
* Meets directly with patient and family to assess needs, preferences and develop appropriate plan that involves home health care services in collaboration with the physician. Ensures/maintains plan consensus from patient/family, physician and payer. Timely discharge is facilitated through early identification, ongoing assessment and intervention for post-hospital care needs.
* Collaborates and communicates with multidisciplinary team in all phases of discharge planning. Ensures/maintains plan consensus from patient/family, physician, and payer as indicated.
* Proactively identifies and resolves delays and obstacles to discharge. Utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues and system problems.
* Seeks consultation from and makes referrals to appropriate disciplines/departments as required to expedite discharges.
* Demonstrates knowledge of community resources and an ability to connect patients and families with these resources. Acts as an advocate on behalf of the patient who requires assistance to gain access to needed information, resources, or services.
* Facilitates review of high-risk cases by Office of General Counsel, Risk Management and informs appropriate members of the healthcare team as to interventions needed. Coordinates interventions in collaboration with healthcare team.
* Provides patient and family education that promotes wellness and increases knowledge of the health care system.
* Completes timely documentation of activities in the medical record and hospital wide information systems.
* Demonstrates knowledge of the utilization management process which includes level of care assignment, communication with payors and benefit authorization for applicable situations.
* Coordinates with GEDI RN to assist with resources for high-risk geriatric patients.
* Assess for appropriateness and refer patients to Transitional Care Clinic as needed.
* Schedules appointments for patients when Physician Referral Services (PRS) is unavailable.
Actively Participates in Clinical Performance Improvement Activities related to Case Management Services
* Assists in the collection and reporting of financial indicators including LOS, avoidable days, resource utilization, discharge barriers, cost per case, readmission rates, denial and appeals.
* Uses data to drive decisions and plan/implement performance improvement strategies related to case management for assigned patients/units, including financial, clinical, quality and patient satisfaction data.

* Provides 24/7 coverage in 12 hours shifts for the emergency department.
* Provides graduate level Social Work field supervision for students requiring a field placement.
* Assumes responsibility for professional development and social work CEU requirements by participating in workshops, conferences, and/or in-services.
* Complies with Northwestern Memorial Hospital policies on patient confidentiality including HIPAA requirements and Personal Rules of Conduct.
Qualifications
Required:

Responsibilities:
* Psychosocial Assessment and Intervention:
* Meets directly with patient and family to perform a comprehensive assessment including social, emotional, cultural, mental status, environmental and financial circumstances in conjunction with interdisciplinary assessment of the patient.
* Recommends a plan of intervention based on patient needs, preference and mutually established goals.
* Provides psychosocial interventions which include reactions to illness and disability, especially the chronically and terminally ill. Facilitation of informed decision making [including advanced directives] and development of treatment and intervention plans. Adjustment to the hospital setting and compliance with treatment plan. Adjustment and coping with post hospital care needs and linkage to community resources. Gynecological and obstetrical related issues including teen pregnancy, parenting issues, adoption planning, infant developmental problems, drug exposed neonate, fetal death, unplanned pregnancy, pregnancy termination, and other care as needed. Issues related to insurance coverage and payment. Psychiatric symptoms and chemical dependency. Conflict resolution. Family and personal relationship that impact the plan of care and discharge plans.
* Performs assessments of the physical environment and adequacy of support systems for outpatients to prevent a crisis and/or hospitalization.
* Provides crisis intervention and/or Protective Services for the elderly without support systems, with impaired mental status and/or victims of suspected abuse/neglect, as well as victims of suspected sexual/physical assault (includes rape and molestation), victims of suspected child abuse or neglect, or victims of domestic violence. Guardianship and/or protective services for patients with significant mental status impairment or unsafe living environment and/or the homeless.
* Manages Discharge Planning through Placement Coordination, Resource Utilization, and Coordination of Skilled Home Health Care.
* Actively participates in the stages of discharge planning and ensures that the plan of care is coordinated, facilitated and effectively communicated to the physicians, healthcare team, patient and family.
* Provide initial screening for all new patients to assure medical necessity, source of funding, and likelihood of needing Social Work and/or discharge planning services.
* Serves as the point person for the plan of care as it applies to discharge planning needs through facilitation of direct and continuous communication and collaborative decision making, including participation in multidisciplinary rounds and case conferences and other collaborative forums.
* Coordinates action plans when barriers are present to facilitate resolution.
* Coordinates discharge planning to ensure a timely discharge through early identification, assessment and intervention for post hospital care needs.
* Patient assessment, plan coordination and changes to the plan occur, as necessary, to ensure that the patient is discharged when medically ready to other acute hospitals, rehabilitative facilities, extended care facilities, sub-acute care, psychiatric and chemical dependency care, return to home or other living arrangements.
* Meets directly with patient and family to assess needs, preferences and develop appropriate plan that involves home health care services in collaboration with the physician.
* Ensures and maintains plan consensus from patient and family, physician and payer.

* Upon completion of three years post masters degree, is eligible to provide graduate level Social Work field supervision requiring a field placement.
* Assumes responsibility for professional development and meeting Social Work CEU requirements by participating in workshops, conferences, and / or inservices.

Job Description

A licensed, masters-prepared social worker who is an experienced and independent generalist clinician responsible for complex psychosocial interventions and facilitating the plan of care through discharge planning and resource utilization for all patient populations throughout the Northwestern Memorial Hospital campus.
Proactively collaborates with Patient Care staff, Medical Staff, and other ancillary departments integrating the Department's functions and services with other aspects of the patient care process. Victims of suspected sexual/physical assault (includes rape and molestation).
Coordinating safe discharge plan, including verifying transportation to safe location.
Ensuring coverage of appropriate medication.

Assists in identifying Unidentified Patients, including working with emergency medical services (EMS) and Chicago Police Department (CPD), searching belongings of patient as necessary and talking to family.
Assists in identifying and notifying emergency contacts of trauma patients.
Be present when the medical team notifies family of patient death and provides resources and information regarding body disposition as needed.
Manages Discharge Planning through Placement Coordination and Resource Utilization
Actively participates in the stages of discharge planning and ensures that the plan of care is coordinated, facilitated and effectively communicated to the physician(s), healthcare team, patient and family.
Coordinate with medical team to assess if patient's discharge needs can be met from the Emergency Department and avoid a hospital admission.
Provide initial screening for all new patients to assure medical necessity, source of funding, and likelihood of needing social work and/or discharge planning services.
Serves as the point person for the plan of care as it applies to discharge planning needs through facilitation of direct and continuous communication and collaborative decision-making with medical team
Coordinates action plans when barriers are present to facilitate resolution.

Patient assessment, plan coordination and changes to the plan occur, as necessary, to ensure that the patient is discharged when medically ready to other acute hospitals, rehabilitative facilities, extended care facilities, sub-acute care, psychiatric and chemical dependency care, return to home or other living arrangements.
Meets directly with patient and family to assess needs, preferences and develop appropriate plan that involves home health care services in collaboration with the physician.
Ensures and maintains plan consensus from patient and family, physician and payer.
Timely discharge is facilitated through early identification, ongoing assessment and intervention for post hospital care needs.
Collaborates and communicates with multidisciplinary team in all phases of discharge planning, ensures and maintains plan consensus from patient and family, physician, and payer as indicated.
Proactively identifies and resolves delays and obstacles to discharge.
Utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues and system problems.
Seeks consultation from and makes referrals to appropriate disciplines and departments as required to expedite discharge plan.
Demonstrates knowledge of community resources and an ability to connect patients and families with these resources
Acts as an advocate on behalf of the patient who requires assistance to gain access to needed information, resources, or services.
Facilitates review of high risk cases by Office of General Counsel, Risk Management and informs appropriate members of the healthcare team as to interventions needed.
Coordinates interventions in collaboration with healthcare team and ensures that interventions are successful.
Provides patient and family education that promotes wellness and increases knowledge of the health care system.
Demonstrates knowledge of the utilization management process which includes level of care assignment, communication with payors and benefit authorization for applicable situations.
Actively Participates in Clinical Performance Improvement Activities
Assists in the collection and reporting of financial indicators including LOS, avoidable days, resource utilization, and discharge barriers.
Uses data to drive decisions and plan/implement performance improvement strategies related for assigned patients/units, including financial, clinical, quality and patient satisfaction data.
New graduates are required to participate in weekly clinical supervision with a LCSW Social Worker until a minimum of 3000 supervised hours is fulfilled.
Upon completion of three years post masters degree, is eligible to provide graduate level Social Work field supervision requiring a field placement.
Assumes responsibility for professional development and meeting Social Work CEU requirements by participating in workshops, conferences, and / or inservices.
Complies with Northwestern Memorial Hospital policies on patient confidentiality including HIPAA requirements and Personal Rules of Conduct.

Required:
Masters Degree in Social Work from a school of Social Work accredited by CSWE.
