• Implement aspects of the plan of care
• Participate in health teaching and counseling to promote, attain, and maintain the optimum health level of patients
• Serve as an advocate for the patient by communicating and collaborating with other health service personnel
• Participate in the evaluation of patient responses to interventions
• Communicate and collaborate with other health professionals
• Provide input into the development of policies and procedures to support patient safety
• Evaluates patient progress and reports changes to RN, supervisor and/or physician.

• Assists the physician with procedures.
• Assists with cleaning, decontaminating, sterilization and storage of equipment and instruments.
• Assists in transportation and positioning of patients
• Assists in assembling and dismantling of tables and instruments before and after procedure
• Prepares patient and room for the planned surgical procedures.
• Opens sterile supplies and sets up for procedures
• Completes skin preparation and draping
• Assist Surgeon with gowning and gloving
• Assist surgeon by passing instruments and suture
• Assists in maintaining correct count of instruments, sutures, and sponges
• Applies dressings and bandages as needed
• Assists in identifying, collecting, and caring for specimens.
• Decontaminates instruments (point of use cleaning), supplies and equipment according to established policies and procedures; handles and stores instruments, equipment and supplies appropriately
• Aids in the efficient turnover of the operating room for next cases
• Monitors and restocks supplies

Perform concurrent reviews for appropriateness of utilization to optimize clinical and financial outcomes.
Communicate with physicians, patients, members of the Healthcare team, Coordinated Business Office staff, Denial Management staff, and third-party payors to justify the admission or continued stay.
Notify appropriate staff members of any admission, service, length of stay, lack of medical necessity criteria, as well as denials/appeals and issuing of letters to patients.
Provide Physician, Patient, Family, Staff and Student education.
Act as a resource person for the case management department regarding payer rules, regulations, policies and procedures, and utilization issues.
Perform admission necessity screening using criteria as established by the various federal, state and private sector programs.

WHAT YOU CAN EXPECT
Minimum Start Pay $24/hour
Implements nursing plan of care at the direction of the registered nurse.
Provides home-based patient care based on physician orders and the established nursing plan of care.
Reports any change in patient status to the Home Health RN and documents as appropriate.
Collaborates with Home Health RN and multidisciplinary treatment team to individualize each patient's treatment needs in order to achieve defined goals for improved patient outcomes.
Conducts patient psychosocial assessments to explore the physical, psychological and social aspects of the patient and their conditions, obtains history, and elicits an accurate description of psychosocial needs in order to inform care team
Contributes to developing discharge plans, identifying the post-discharge needs of the patient and target appropriate resources for follow-up care, in collaboration with the patient, family and care team.
Documents patient care information and ensures timely and accurate completion of patient care records in accordance with policies and procedures.
Participates in the education of patients and their caregivers about extended care facilities, home health agencies, and other resources as appropriate to transition patients along the continuum of care.

Job Description

Responsibilities:

Serves as a communication link between the patient, family, caregivers and interdisciplinary team

* A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the acutely ill patient through the nursing process of assessment, planning, intervention, implementation and evaluation. Three West, a fifteen-bed unit is the location of our Inpatient Pediatric and Spinal Cord Injury rehabilitation programs. Our rehabilitation team is comprised of physicians, advanced practice nurses, therapists, nurses and nursing assistants working together to provide exceptional care of our pediatric and adult patients. Within the constructs of the unit, our pediatric patients have a designated location where we can meet their age specific needs. It is in this environment that we care for children from infant to eighteen with varying diagnosis including orthopedic with multi-trauma, cerebral palsy, traumatic brain injury, and oncology. The Spinal Cord program specializes in the treatment of individuals diagnosed with traumatic and non-traumatic spinal cord injuries, tetraplegia, paraplegia and musculoskeletal injuries. Our team has many educational opportunities to advance skills in spinal cord injury and rehabilitation while working in a multidisciplinary environment to safely care for patients with spinal cord injuries with the use of safe patient handling equipment. The diversity of our patients' ages and diagnoses provides an outstanding opportunity to enhance clinical skills and develop professionally.
Patient Care/Clinical Duties:
* Collects and reports pertinent physiological, psychological, and functional data based on patient plan of care or patient need.
* Assists and instructs patients with personal hygiene and activities of daily living, including showering, promoting maximal independence and skill practice.
* Performs preventive skin care program as directed by RN and report any changes in skin integrity immediately to the RN.
* Assists patients with bladder and bowel training programs including post void residuals and spot bladder scans. Perform straight catheterization for select patient populations under direction of RN and provide care for indwelling and external catheters and ostomies.
* Accurately obtains and documents patients' vital signs/pulse oximetry and communicates any reportable values, symptoms or signs of distress to the RN.
* Assists patients with transfers and mobility activities utilizing appropriate techniques, precautions and assistive devices. Practice therapy transfers and ambulation with patients within safe patient handling guidelines as delegated by the RN.
* Assists patient with application of assistive devices and prosthetics/orthoses according to individual patient plan of care.
* Assists patients with completing menus. Passes patient meal trays and HS snacks, verifying any modified diet is as ordered. Accurately calculates and documents calorie count and carbohydrates, intake and output as ordered.
* Assist patient with 1:1 feeds as ordered. For patients with dysphagia, cue patient to use ordered swallowing strategies, under the direction of the RN.

Required:
* High School or equivalent (GED) High School degree, or G.E.D equivalent, is required.
* Current Nursing Assistant certification (C.N.A.) is required or current enrollment in an RN Nursing Program, with the successful completion of two clinical rotations.
* Maintains current CPR certification.
* Basic math skills are required.
* Ability to communicate with a wide variety of people and provide highest level of customer service.

* Flexibility and ability to manage work in a changing environment.

Student Nurse Intern (SNI)

The Prentice Women's Hospital SDS/PACU is a multifaceted unit that provides world-class care to patients undergoing gynecological, plastics, and breast surgery at Northwestern Memorial Hospital. Our nurses provide care to some of the most complex gynecological, plastics, and breast patients in the State of Illinois. The Rehab Nurse Tech-MJ (RNT) reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The IP Rehabilitation RNT has expertise related to the specific needs for patients in the rehabilitation environment which include chronic conditions and disabilities. The Patient Care Technician-MJ is a member of the interdisciplinary rehabilitation care team.
Under the direction/delegation and supervision of a professional registered nurse, the Patient Care Tech-MJ assists the patient with activities of daily living, mobility skills, skin, bowel and bladder programs, and maintains a clean, safe environment for the patient. Additional technical skills, under the direction of the RN, de-escalation techniques and 12 lead EKG. The Patient Care Tech-MJ role serves as an active participant in the unit quality projects which can include data collection as well as unit equipment inventory and maintenance. The Patient Care Tech-MJ is responsible to orient and precept PCT roles.
Responsibilities:
Patient Care/Clinical Practice:
Answers and triages patient call lights and meets patient care needs with an appropriate sense of urgency.
Assists patients with personal hygiene and activities of daily living promoting maximal independence.
Assists patient with application of assistive devices and prosthetics/orthoses according to individual patient program.
Performs preventive skin care program as directed by RN and report any changes in skin integrity immediately to the nurse.
Accurately calculates and documents calorie count, intake and output and patients' weights.
Assists patients with transfers and mobility activities utilizing appropriate techniques, precautions and assistive devices.
Assists patients with bladder and bowel elimination programs including collection of laboratory specimens. Straight catheterization for select patient populations under direction of RN.
Accurately obtains and documents patients' vital signs and communicates any concerns to the registered nurse.
Measures and documents blood glucose measurements.
12-lead EKG as ordered
Assists patients with completing menus daily within established timeframes.
Passes patient meal trays and HS snacks under the direction of the nurse.
Provide supervision and activities for patients with high risk to fall.
Patient rounding: Circulate on the nursing units to monitor patients.
Provide de-escalation techniques for agitated and aggressive patient behaviors
Engage patient in activities as appropriate.
Patient mobility champion on the unit.
Maintains and organizes unit equipment inventories.
Preceptor to new employees.
Inclusive of all other duties as assigned by nurse manager, nurse supervisor, staff nurse and charge nurse.
Quality Improvement:
Participates in system and local quality improvement goals.
Collects data and audits for Joint Commission readiness and improvement projects.
Assists in the maintenance of unit readiness to meet regulatory standards.
Communication:
Maintains a therapeutic relationship to provide optimal customer-focused care.
Communicates effectively with patients at level appropriate to age and cognitive status, showing cultural sensitivity.
Interacts effectively with peers and other hospital staff to foster positive working relationships.
Effectively triages patient and the general concerns to RN.
Infection Control/Patient Safety:
Contributes to the safe, effective and professional delivery of patient care including use of safety/restrictive devices.
Follows Infection Control policies and procedures.
Maintains knowledge of Marianjoy's policies concerning precautions, codes, conditions, warning and general safety procedures.
Adaptability and Flexibility:
Rotates to all units based on patient care needs.
Adjusts to changes in policy and procedure, unit and patient population.
Qualifications
Required:
High school or equivalent (GED)
Current Nursing Assistant certification (C.N.A.) or current enrollment in an RN Nursing Program, with the successful completion of two clinical rotations
Maintains current CPR certification
Basic math skills
Ability to communicate with a wide variety of people and provide highest level of customer service
Understanding and support of Northwestern Medicine's Mission and Values
Flexibility and ability to manage work in a changing environment
12 lead EKG competency completed within 6 months of employment
Preferred:
2-5 years of work experience is preferred in a rehabilitation hospital, acute care hospital or skilled nursing facility.

A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the acutely ill patient through the nursing process of assessment, planning, intervention, implementation and evaluation. Effectively interacts with patients, significant others, and other health team members, while maintaining ANA standards of professional nursing practice. Responsibilities: Performs all aspects of patient care in a manner which optimizes patient safety and reduces the likelihood of medical/health care errors. Follows the five medication rights and reduces the potential for medication errors. Demonstrates knowledge of medications and their correct administration based on age of the patient and their clinical condition. Involves patient/family in the formation of the plan of care. Develops, implements and updates an individualized plan of care based on identified patient care needs. Includes nursing, teaching, continuum of care and discharge planning. Able to state rationale for clinical decisions based on critical thinking. Assesses, analyzes, validates and integrates data to assess patients' physical psychosocial, rehab, safety, learning, spiritual, cultural, age-specific, special risk and self-care needs. Identifies patient care needs, problems and variances. Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management. Identifies learning needs, abilities, preferences and readiness; also, identifies support systems, coping mechanisms and barriers to learning. Makes referral for follow up as appropriate. Explains procedures, therapies and treatments to patient and family, evaluates and documents patient and family response to teaching. Uses appropriate resources to provide education. Conducts body and belongings search in proper manner. Demonstrates awareness to specific contraband allowed on unit. Collaborates with the physician and team to review and evaluate the plan of care. Communicates effectively and works cooperatively with other team members. Has respect for and understanding of other clinical disciplines. Coordinates the care of the patient by including members of the health care team and other departments to plan, deliver, and evaluate patient care. Delegates appropriately to other care providers to facilitate meeting patient needs in a timely manner. Conducts telephone screenings (inquiry calls) in a fashion resulting in appropriate disposition based on the phone contact. Understands patient placement criteria and can identify proper level of care to convey to Managed Care along with the pre-cert information requested. Communicates directly to department manager routine and non-outline clinical and staff issues. Able to make independent appropriate decisions regarding patient care and staffing issues. Contributes to the Utilization Review process by adding a consultative nursing assessment regarding total patient care delivery. Participates and communicates with third party payers with regards to the nursing component of patient care during the Utilization Review process.
