The Clinical Documentation Improvement Second Level Reviewer is a Professional Registered Nurse with a broad clinical knowledge base and understanding of DRG documentation requirements who works under the supervision of the CDI Manager. Responsibilities include secondary clinical chart reviews, resolution of DRG discrepancies, and education to clinical staff regarding opportunities for diagnosis clarification, principal diagnosis accuracy, and improvement of capture of additional comorbid conditions diagnoses. The Clinical Documentation Improvement Second Level Reviewer will conduct concurrent and retrospective medical record review for defined patient populations to identify opportunities to improve accuracy of practitioner documentation; collaborate with Case Management and Coding department to assure documentation is clinically appropriate, and accurately reflect the severity of illness for the patient that is reflective of current CMS standards.
Education
Associates, Diploma or Bachelor's degree in Nursing
Licenses and Certification
Current Illinois RN license
Certification in Clinical Documentation Improvement or Coding preferred.
Experience and Skills
Technical Experience: 2 years
Must be able to demonstrate broad clinical knowledge base necessary to review a wide variety of medical records.
Must be able to demonstrate positive communications skills.
Role Specific Responsibilities
Completes secondary reviews of targeted patient populations to identify missed opportunities and accurate selection of principal diagnosis.
Acts as a liaison between the Coding Department and the Clinical Documentation Specialist to reconcile discrepancies in DRG assignment.
Communicates findings of secondary reviews to respective Clinical Documentation Improvement Specialists and Coding staff for follow up.
Documents and tracks secondary review data. Shares this information with staff at monthly DCI meetings.
Completes medical record review using an organized approach to survey admit notes, past medical history, home meds, physician/provider documentation, treatments, orders, ancillary department notes, laboratory data and other pertinent components of the clinical record.
Analyzes and interprets clinical data to identify gaps, tends, inconsistencies and/or opportunities for improvement in the clinical documentation.
Regularly interacts with/educates physicians to enhance understanding of the CDI program and to insure the medical record can be coded accurately in order to reflect patient severity of illness and risk of mortality.
Collaborates with other clinical disciplines and members of the coding department to insure high quality clinical documentation and efficient, timely coding of the medical record.
Collaborates with Coding Department when discrepancies in the medical record are identified and reviews medical record to provide clinical feedback for accurate coding.
Develops collaborative relationships and promotes team work with co- workers and other departments.
Organizes and performs work responsibilities effectively and efficiently.
Maintains strict patient confidentiality, adhering to HIPPA guidelines.

Job Description
The NM Internal Agency RN reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
A clinically competent, registered, professional nurse who prescribes, delegates and coordinates nursing care to the acutely ill patient through the nursing process of assessment, planning, intervention, implementation and evaluation. As a member of the NM Internal Agency RN Team, the nurse in this role will have assignments that span across all NM hospital locations. Generally, the minimum assignment at the same hospital location will be six (6) continuous weeks, with the maximum assignment not to exceed six (6) continuous months at the same hospital location. This position offers the RN the opportunity to enhance their clinical skills, as well as sharpen communication and teamwork. The NM Internal Agency RN works with many diverse people and grows relationships with fellow staff members all over Northwestern Medicine's system hospitals.
Responsibilities:

Uses retention strategies to recognize and reward staff on shift.
Serves as Qualified Observer to ensure completion of annual competencies of staff on his/her shift.

Supports staff to incorporate EBP/research findings into practice at the bedside.
Oversees environment of care and regulatory readiness with all regulatory bodies.

Qualifications
Required:
3 Years RN experience

American Heart Association Basic Life Support (BLS)

Consistently works within the framework of the Illinois Practice Act for Nursing and ANA Standards of Professional Nursing Practice.

Job Description

Description
The Clinical Operations Coordinator, RN reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Clinical Operations Coordinator, RN is responsible for supervising the daily clinical operations of practice sites, supervising clinical support staff, coordinating clinical staff and provider schedules, and performing service recovery as necessary. The operations Coordinator is the clinical lead and subject matter expert at the local level to ensure staff knowledge, competence and processes align with evidence based high quality patient care. Under the clinical direction of the Director, Clinical Practice and the administrative direction of the Clinical/Practice Administrator, the Operations Coordinator participates in the planning, leading, and organizing of activities relating to quality patient care in the practice. Direct clinical patient care would be provided less than twenty percent of the time in this role. The Operations Coordinator may oversee multiple sites or departments within RMG.
Responsibilities:
* Supervise and direct the office's clinical support staff, manage workload distribution and prioritization of tasks.
* Participate in interviewing, hiring, training, progressive discipline, dismissals, and performance appraisals.
* Oversee and direct day to day office clinical operations within budget guidelines.
* Assist with managing providers schedule templates and optimize patient care hours to enhance productivity.
* Schedule staff and allocate resources across hours of operation to maximize patient access and satisfaction, provider productivity and efficiency, and staff productivity.
* Responsible for front-line communication with staff to receive and provide feedback regarding clinical workflow, quality and productivity.
* Ensure that service excellence is delivered and perform service recovery whenever necessary.
* Manage medical surgical supplies within budget.
* Ensure accuracy of all data collection and data entry by site staff through training and compliance monitoring and facilitate and oversee flow of information to and from central administrative offices.
* Conduct office staff meetings on a regular basis in coordination with practice leadership.
* Attend and actively participate in leadership meetings.
* Ensure that patient dignity and confidentiality are always maintained.

Student Nurse Intern (SNI)
is an unlicensed person who works, under the supervision of a Registered Professional Nurse (RN), to support the assessment, planning, implementation, and evaluation of patient care. Responsibilities:
The Student Nurse Intern's (SNI) tentative responsibilities will include but are not limited to the following:
* Provide direct patient care under RN supervision, consistent with the Relationship Care Model
* Participate in bedside report, patient rounding, and care planning discussions
* Support patient hygiene, mobility, nutrition, and comfort
* Collect specimens and perform basic monitoring tasks
* Document care activities in the electronic medical record, as permitted
* Practice infection control and isolation precautions
* Engage in educational opportunities, performance improvement projects, and professional development activities
Successful Student Nurse Interns will receive an employment offer at the conclusion of their 8-week internship.

Qualifications
Required
* Basic Life Support (BLS) CPR through American Heart Association
* Documentation of enrollment in an accredited Nursing Program. Students must graduate no later than June of the calendar year following completion of the SNI program. In addition, they must have completed at least one clinical rotation.
Preferred
* Currently enrolled in an accredited professional nursing program and entering the last year of college. Some locations may require enrollment in a Bachelor of Science in Nursing program.
* A letter of recommendation from a faculty member
