Position Summary
• Responsible for the overall operation of case management and social services activities within the department.

• Technical Experience: 2 years of experience in providing individual, family and/or group therapy required. Experience in addiction disorders/cross addictions; excellent knowledge of the concepts of stigma, addiction, recovery, and relapse helpful

• Excellent knowledge of available community resources
Role Specific Responsibilities
• Performs concurrent patient assessment related to appropriateness of the level of care, diagnosis, procedures performed, and intervenes to expedite the patient's plan of care.
• Provides consultations to medical staff and other members of the multi- disciplinary team regarding the impact of socio-emotional factors on the patient's illness and intervenes to expedite the plan of care.
• Provides and documents discharge planning services in a professional, collaborative and timely manner to optimize meeting patients' post- hospital discharge care needs.
• Collaborates with the interdisciplinary team to assess, monitor, implement and evaluate patient care needs during the hospital stay in order to smoothly transition the patient to the next level of care.
• Utilizes evidence-based practice standards to guide the evaluation of care, length of stay, medical necessity of services, and appropriate use of organizational and patient centered resources. Medically complex setting determinations may require consultation with the R.N. Case Manager.
• Intervenes on behalf of patients with specific needs not limited to abuse, neglect, mental health issues, end of life issues, ethical concerns, legal matters, financial concerns, and challenges in family dynamics.
• Facilitates and acts as a resource to other members of the healthcare team for functions to include but not limited to guardianship proceedings, adoption, advance directive planning, and emergency detentions.
• Develops and maintains collaborative relationships with organizations in the community that facilitate provision of appropriate care during the hospital stay and facilitate efficient and effective planning for continued care for the patient:
• Collaborates with necessary staff and post-discharge care providers to assure a safe and effective discharge plan; Facilitates care conferences with patient and/or family support structure to foster decision making that promotes patient advocacy.
• Supports Service Excellence initiatives contributing to the organization becoming a top performer in quality care, patient safety and patient satisfaction strategies: Understands and applies techniques to support the provision of population based appropriate care utilizing Joint Commission standards, CMS Core Measure requirements, and other best practices
• Acts as an expert resource to the interdisciplinary healthcare team, physicians and other leaders:
•Participates in interdisciplinary team meetings in a collaborative manner resulting in achievement of best patient outcomes; Participates actively in departmental and hospital wide teams, committees, or other improvement initiatives.

Description

* The therapist provides outpatient psychiatric treatment services to persons suffering from psychiatric/chemical dependency difficulties and/or their families/significant others. The therapist must be knowledgeable in areas of assessment, counseling, therapy, relapse prevention, referral and recovery, and will consider the specific needs and behaviors of specific age groups. The therapist is responsible for establishing a therapeutic alliance with the patient and is considered the patient's advocate.
Responsibilities:
ASSESSMENT/PROBLEM IDENTIFICATION
* Demonstrates an understanding of the trans theoretical model, including stages of change and motivational interviewing. Consistently maintains the spirit of motivational interviewing when interviewing the patients.
* Demonstrates an understanding of Recovery Based Services and attends to the four major dimensions that support recovery: Health, Home, Purpose and Community.
* Assesses and records patient status, identifying the psychological and emotional needs of the patient and family/support system.
* Organizes the assessment data so that they are accurate, complete and accessible.
* Collaborates with the patient to develop the Comprehensive Problem List.
* Collaborates with the registered nurse to assure that the patient's nursing/medical needs are addressed.
* Assesses discharge planning needs including identification of needed referrals and initiates the discharge planning process.
* Completes intake assessments and assess the treatment needs of callers seeking psychiatric/chemical dependency treatment.
GOAL SETTING/PLANNING
* Primarily responsible for the development of the Phased Treatment Plan, recommends updates of treatment plans, and contributes to the discharge planning for program patients. Incorporates principles of age-specific care into the Phased Treatment Plan.
* Collaborates with the patient to identify treatment goals and methods during each phase of treatment.
* Begins plans for discharge at time of admission; consults with physician, registered nurse, patient and family regarding the discharge plan.
* Identifies patient/family teaching needs on admission and throughout treatment.
* Arranges conferences with family or other members of health team when indicated to plan care.
IMPLEMENTATION/INTERVENTION
* Provides group therapy sessions and educational seminars for psychiatric and/or chemically dependent patients and their families.
* Provides individual and family counseling to primary patients and other patients in the outpatient programs as needed.
* Provides outpatient psychotherapy in various modalities including individual, family/marital and groups.
* Utilizes evidence-based interventions for each phase of treatment.
* Utilizes self-report or behavioral measures to evaluate patient's progress toward goals.
* Documents all assessments, interventions and patient response. Maintains clinical records in accordance with department policy.
* Provides referral to the appropriate level of care for intakes.
* Participates in annual screening events or community education presentations.
EVALUATION
* Evaluates care each session, changes the Phased Treatment Plan as needed and communicates changes to other members of the multi-disciplinary team.
* Consults with the attending psychiatrist and other physicians as needed, regarding patient's progress.
* Discusses the patient at multi-disciplinary conferences for feedback from peers.
* Reassesses treatment interventions for effectiveness in achieving desired outcomes.
* Transitions patient to a new phase of treatment based on response to significant changes in patient's primary concerns and within policy time frames.
* Therapist routinely seeks patient evaluations of therapy sessions and/or the therapeutic relationship.
COMMUNICATION
* Communicates effectively with patient/family.
* Appropriately communicates with public, co-workers and psychiatrists to promote a harmonious work environment.
* Communicates regularly with the attending psychiatrist when treatment issues or problems arise that require physician input/involvement.
* Provides appropriate input to the Director for outpatient psychiatry and hospital committees.
HUMAN WORTH
* Maintains confidentiality of patient care information.

Qualifications
Required:
* Master's Degree in Clinical Social Work, Clinical Psychology or other related discipline with appropriate current licensure in the State of Illinois.
* Three to five years of experience in treating patients with psychiatric and substance use problems.
* Extensive experience with families and/or individuals in psychiatric/substance use setting dealing with crisis or problematic situations.
* CPR Certified.

This is a casual/as needed position.

As a member of a multidisciplinary team, the Behavioral Health Counselor assists in providing patient assessments and treatment as directed. Under supervision, this role provides supportive counseling, case management, and community support services. Responsibilities:
* Screen and evaluate requests for service
* Collect and report social history for assigned cases

* Work with an interdisciplinary team in reviewing cases and developing individual plans

Utilizes de-escalation techniques to support and treat psychiatric patients. Utilizes evidence-based, trauma, cultural, and recovery-informed practices to guide practices.
Develops effective discharge planning with input from multidisciplinary team and managed care/utilization review. Knowledgeable about referral sources and available community resources.
Demonstrates ability to cross train to other areas of BHS and seeks professional development opportunities to expand knowledge base. Accepts assignments of work to other units within BHS.

Qualifications
Required:
1-2 years of experience working with substance abuse and mental health populations.
Masters Degree in Social Work, Psychology, counseling, or related field

Experience and training in assessment, individual, family, and group counseling.
Must be computer comfortable and/or proficient as much of the medical records are automated.

Required:

Experience and training in assessment, individual, family, and group counseling.
Must be computer comfortable and/or proficient as much of the medical records are automated.
Must be detail oriented, able to multi-task, and dedicated to supporting patients.
Preferred:
3-5 years in intensive treatment setting and/or mental health setting.
Comprehensive knowledge of crisis intervention techniques for a broad spectrum of the mentally ill and chemically dependent population is preferred.

OPCD Therapist

The therapist provides outpatient psychiatric treatment services to persons suffering from psychiatric/chemical dependency difficulties and/or their families/significant others. The therapist is responsible for establishing a therapeutic alliance with the patient and is considered the patient's advocate.

ASSESSMENT/PROBLEM IDENTIFICATION
Demonstrates an understanding of the trans theoretical model, including stages of change and motivational interviewing. Consistently maintains the spirit of motivational interviewing when interviewing the patients.
Demonstrates an understanding of Recovery Based Services and attends to the four major dimensions that support recovery: Health, Home, Purpose and Community.
Assesses and records patient status, identifying the psychological and emotional needs of the patient and family/support system.
Organizes the assessment data so that they are accurate, complete and accessible.
Collaborates with the patient to develop the Comprehensive Problem List.
Collaborates with the registered nurse to assure that the patient's nursing/medical needs are addressed.
Assesses discharge planning needs including identification of needed referrals and initiates the discharge planning process.

Primarily responsible for the development of the Phased Treatment Plan, recommends updates of treatment plans, and contributes to the discharge planning for program patients. Incorporates principles of age-specific care into the Phased Treatment Plan.
Collaborates with the patient to identify treatment goals and methods during each phase of treatment.
Begins plans for discharge at time of admission; consults with physician, registered nurse, patient and family regarding the discharge plan.
Identifies patient/family teaching needs on admission and throughout treatment.
Arranges conferences with family or other members of health team when indicated to plan care.
IMPLEMENTATION/INTERVENTION
Provides group therapy sessions and educational seminars for psychiatric and/or chemically dependent patients and their families.
Provides individual and family counseling to primary patients and other patients in the outpatient programs as needed.
Provides outpatient psychotherapy in various modalities including individual, family/marital and groups.
Utilizes evidence-based interventions for each phase of treatment.
Utilizes self-report or behavioral measures to evaluate patient's progress toward goals.
Documents all assessments, interventions and patient response. Maintains clinical records in accordance with department policy.
Provides referral to the appropriate level of care for intakes.
Participates in annual screening events or community education presentations.
EVALUATION
Evaluates care each session, changes the Phased Treatment Plan as needed and communicates changes to other members of the multi-disciplinary team.
Consults with the attending psychiatrist and other physicians as needed, regarding patient's progress.
Discusses the patient at multi-disciplinary conferences for feedback from peers.
Reassesses treatment interventions for effectiveness in achieving desired outcomes.
Transitions patient to a new phase of treatment based on response to significant changes in patient's primary concerns and within policy time frames.

* Clinical Sciences to include, but not limited to the major areas of: Medicine or Surgery. Physical therapy theory and application, including therapeutic exercise, evaluation procedures, physical agents, mechanical modalities, electrotherapy, massage, orthotics and prosthetics, and professional issues.
* Clinical Education a minimum of 800 clock hours.
* Graduates after January 1, 2002 must have a minimum of a master's degree in physical therapy.
* Licensed Physical Therapist in State of Illinois.
* BLS within 30 days of hire.
