We're looking for a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families facing end-of-life care. As a valued member of our interdisciplinary team, you'll help ensure that no one walks this path alone.
What You'll Do as a Hospice Chaplain:
Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.
Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, socialworkers, aides, and medical staff.
Assess spiritual needs of patients and families and develop individualized spiritual care plans.
Conduct visits, prayers, blessings, and rituals as appropriate and requested by the patient or family.
Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.
Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
About You
The candidate must meet one of the following educational requirements:
Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.

Responsibilities
Enhabit Home Health & Hospice is searching for a Chaplain with hospice experience to join our team. The Chaplain is a member of the interdisciplinary team and reports to the Hospice Branch Director.
Providing direct spiritual support and/or counsel to patients/families in keeping with patients/families' beliefs.
Working with staff, clergy and community groups to enhance their sensitivity to the spiritual needs of patients/families and reporting on services as indicated.
Providing consultation and education for patients/families and interdisciplinary team members.
Providing bereavement follow-up services as defined by Hospice.
Maintaining proper records of visits to patients/families.
Performing occasional liturgical assignments such as the annual Service of Remembrance and monthly memorial service when indicated.
Qualifications
Education and Experience (ESSENTIAL):
Must be an individual who, by ordination or by ecclesiastical endorsement from the individual's denomination, has been approved to function in a pastoral capacity.
Must have demonstrated experience in working with patients and families dealing with life-threatening illness and death.
Education and Experience (DESIRED):
Training in clinical pastoral education, meeting the requirements for the college of chaplains, may be considered in lieu of ordination or endorsement.
Qualifications:
Must be able to effectively communicate, both orally and in writing.
Must have exceptional customer service skills and be able to communicate well with a diverse group of stakeholders.
Must be organized and able to perform multiple, critical tasks simultaneously and frequently.
Must have an understanding of issues related to delivery of home care services.
Must have demonstrated ability to be open, sensitive, flexible, and ecumenical.
Must be able to function efficiently and in a positive manner within a high stress environment.

Prepares a spiritual plan to address specific issues identified by the spiritual assessments. Shares plan with appropriate staff members.
Is an active member of the interdisciplinary team and participates in team conferences every two weeks.
Communicates impressions of ongoing visits and assessments with other staff involved in the case management as well as with outside clergy involved. Offers assistance through participation in Hospice team conference.
Through work with the client/family assists them to constructively deal with the specific issues that are being addressed refers them to other resources as needed.
Completes required documentation on a timely basis.
Chaplain provides bereavement services including phone calls, visits and memorial services to family members upon the death of a patient.
Must be knowledgeable of and comply with Medicare Hospice conditions of Participation-Spiritual Caregiver.
Must be able to travel with in a 6 county radius

Key Responsibilities
Optimal Care Hospice Chaplains are responsible for the provision of spiritual care services to patients and families/caregivers of Optimal Care Hospice, either directly or through coordination of care with other spiritual counselors.
In this role you will be responsible for:
Assuring spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner
Making the initial evaluation within 5 days of hospice admission and re-evaluating the patient's social work needs during each subsequent visit
Making follow-up visits according to patient, family, and facility needs
Facilitating the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs
Providing consultation, education, and support to the interdisciplinary group on spiritual care.
Performing funeral or memorial services for patients as requested
Planning periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team
Required Qualifications
Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).
Current and valid driver's license
Reliable transportation and valid automobile insurance coverage
Experience working with death and dying, individuals, families, and caregivers
Experience and/or education in grief or loss counseling
Ability to work as a member of Hospice Interdisciplinary Team
Ability to accept different lifestyles, cultures, beliefs, values and provide comfort in an interfaith setting
Ability to network with community clergy and congregations
Knowledge of and commitment to hospice philosophy of care
Desired Qualifications
Hospice experience is preferred
Familiarity with Home Care Home Base (HCHB)

Main Service Area: Marion, Hamilton, Madison, Hancock, Shelby, Johnson, Morgan, Henricks, Boone, and Clinton
Hours

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
2. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels patient and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary.
3. Assesses for, and educates interdisciplinary group, on any special needs related to the culture of the patient and family. Includes communication, role of family, space, and any special traditions or taboos.
4. Maintains clinical records on all patients referred to social work.
5. Educates patients and families on, and assists in, preparation of advanced directives.
6. Provides information and referral services for organization patients and families/caregivers regarding practical and environmental needs.
7. Provides information to patients and families/caregivers and community agencies.
8. Serves as liaison between patients and families/caregivers and community agencies.
9. Maintains collaborative relationships with organization personnel to support patient care.
10. Maintains and develops contracts with public and private agencies as resources for patient and personnel.
11. Participates in the development of the individualized plan of care, involving the patient and family, and attends regularly scheduled interdisciplinary group meetings, assisting the team in recognizing the effects of the psychosocial stresses on the symptoms of the terminal illness.
12. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues.
13. Actively participates in quality assessment performance improvement teams and activities.
14. Assists family and patient in planning for funeral arrangements, financial, legal, and health care decision responsibilities.
15. Other duties as delegated by the Clinical Supervisor/Director of Clinical Services.

Qualifications
POSITION QUALIFICATIONS
1. A graduate of a master's program in social work (MSW) accredited by the Council on Social Work Education
OR
2. A graduate of a bachelor's program in social work accredited by the Council on Social Work Education. Meets personnel qualifications stated in ยง418.114(b)(3).
3. The employee has the required licensure, education, training, and qualifications required by rule, regulation, law and/or statute to qualify for the role in their state. Previous home care/hospice is required. Experience in a hospice care preferred.
4. Demonstrates good verbal and written communication, and organization skills.
5. Possesses and maintains current CPR Certification (per state regulations)
6. Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
