Interventions may be individual, group based, community collaboration. Performs crisis stabilization functions. Assists consumer and/or guardian in accessing resources with the community. Completes necessary paperwork for referrals, coordination, admission, discharge, and out of home placements. Serves as central support for the clinical team. Performs care coordination functions. These functions include clinical care coordination between resources within and outside the agency, and authorizations for residential or out of home placements.
Duties & Responsibilities:
Reviews documents received and requests required documents to appropriate level of care for members.
Assesses consumers in accordance with established policies and procedures and clinical protocols.
Develops service plans with consumers based on assessment and diagnostic process.
Coordinates consumer care or reports on interventions by meeting with CFT and ART to review efficacy of interventions and to revise plans of service with consumers.
Develops and maintains appropriate level of relationships with consumers and formal/informal support systems.
Provides support and information, education or guidance to consumers and support system members concerning issues of treatment planning, advocacy, and medication management.
Monitors mental status of consumers and response to treatment plans through face-to-face contact, and updates treatment plans as needed.
Works collaboratively with treatment team and all external members of consumer's services.
Provides face-to-face crisis intervention.
Completes safety plans with consumers.
Conducts groups as needed.
Transitions consumer to appropriate level of care upon conclusion of treatment.
Provides information to consumers and family members or guardians concerning community resources and linkages to other services.
Assists consumers, families or guardians in securing appointments with support services and agencies, as needed.
Maintains accurate, thorough and current documentation of contact with consumers, treatment progress and services received/provided.
Close coordination with the PCP health plan, stakeholders, and funding source will be maintained for all consumers.
Attends weekly clinical staffing's and all staff meetings.
Coordinates hospital discharges by scheduling follow up appointments and ensures proper communication is kept between inpatient and outpatient teams.
Enrolls members through the intake process where assessment, service plan, safety/support plan is completed.
Completes outreach for members who miss an appointment and follows up with re-engagement process.
Completes chart closures.
Maintains quality and compliance in consumer's charts.
To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and or department.
Education & Qualifications:
Minimum requirement consists of a 21-year age
A Bachelor's degree with one year of full-time behavioral health experience. A combination of experience and education totaling 5 years is acceptable.
Must have fingerprint clearance issued by the Arizona Department of Public Safety or be eligible to receive within eight (8) weeks of hire

Meets with residents in transitional housing (2x/month) month or more, to assess needs and ensure compliance with program agreements.
Oversees hygiene and safety standards of all units; inspects apartments, smoke detectors and fire extinguishers; reviews emergency escape plans and unit up-keep with residents.
Identifies unmet needs of residents, provides service engagement, linkage to services, coordinates referrals and provides follow-up.
Facilitates procedures for entering and exiting program and preparation of units.
Maintains housing/Health & Safety standards of all properties and master leased units.
Consults with internal staff and external case managers/service providers in establishing goals and objectives; participates in internal and external staffing.
Develops Individualized Housing Plans with residents.
Intervenes in crisis situations and coordinates with other supports as needed.
Consults with landlords and management companies, as needed or directed, regarding responsibilities to units and ensures that repairs are made.
Understands Arizona Landlord and Tenant Act and its application to residents and units.
Participates in community planning, advocacy and networking with other providers, as directed by supervisor.
Maintains strong collaboration with all Project HHHOME providers and property site management.
Facilitates annual recertification, maintains HQS Housing Habitability Standards for all units. Follows income and rent procedures as well as resident lease compliance
Shares emergency on-call duty as required.
May be expected to drive a SBH vehicle to transport consumers, per the discretion of the clinic and/or program.
Education and Qualifications:
Must be at least 21 years of age
Have a high school diploma/GED and two years of behavioral health/integrated healthcare relevant work experience; OR
An Associate's degree in an unrelated field and at least one year of full-time behavioral health / integrated healthcare relevant work experience; OR
A related Associate's degree and six months of behavioral health / integrated healthcare relevant work experience; OR
Have and maintain a valid Arizona driver's license and are able to pass a motor vehicle screen for insurance purposed in order to transport clients

Works collaboratively with clients, treatment team, program staff and the InShape Coordinator to design and support individual service plans for SBH and InSHAPE.
Provides fitness training, monitoring, and continuing reassessment of exercise, nutrition and healthcare commitments.

Attend and participate in InSHAPE training ( to include all-expenses paid travel) and/or phone calls along with SBH required in-service training, supervision and/or clinical oversight.

Responsible for supervision of proper calculation of Tenant Rent Amount based on current HUD regulations.

Responsible managing maintenance services, and program support.
Approves and monitors the administration of leases and contracts to ensure compliance with HUD regulations.

Establish and maintain effective working relationships with all staff, management, internal and external customers.
Manage vacancy of units and aggressively turn vacancies into habitable units.

Supervise the general appearance of the properties and Real Estate.
Arbitrate any disputes involving tenants while complying with all HUD tenant treatment requirement
Maintain solid and effective communication with all property issues and developments at the various properties.
Maintains EIV Required Reports
Send notice to tenants for non-compliance and follow up if legal action is required.
Responsible for on-going communication with internal and external contacts to ensure compliance and enforcement of rules, procedures and guidelines.
Responsible for other related duties relevant to position description as assigned by Supervisor.
Requirements:
Minimum 21 years of age
Required High School Diploma or GED
Effective performance of the position's duties requires a two years directly related experience with HUD housing. Experience in a behavioral health or hospital setting is helpful.
Preferred:
Must be computer literate in Excel.
Data entry, ten-key by touch, good written, verbal and organizational skills.
Ability to effectively communicate with and maintain an effective working relationship with clinical staff, management, and administration.
Demonstrates decision-making and problem-solving abilities in conflict resolution.

Support:
Therapists report to a Licensed Clinical Program Director and Coordinator, and also have expert Leads for additional mentorship. Therapists receive free 1:1 clinical supervision, and are a part of a supportive interdisciplinary team which meets bi-weekly. Therapists are invited to an array of on-going, in-house education opportunities regarding cultural competency and modality building skills.
