Fraser is currently seeking Case Managers to support individuals on the CADI/BI and DD waivers. Case Managers serve as advocates for Fraser clients, helping them access resources and services within the community. The ideal candidate will demonstrate compassion, strong organizational skills, and a commitment to client success, with experience working with individuals with developmental or intellectual disabilities. Excellent documentation and case noting skills are also essential.
Responsibilities include:
Providing care coordination, monitoring client health and safety needs, and ensuring services effectively meet client goals.
Intervening as needed to support clients and resolve service gaps.
Serving as a consistent advocate and connection to resources.
Caseloads:
CADI/BI Waiver Case Managers: Provide client-centered services to individuals receiving CADI/BI waiver support. Caseloads average about 35 clients (primarily adults, with some children). Case Managers meet with clients 2-3 times per year.
DD Waiver & Non-Waiver Case Managers: Provide client-centered services to individuals receiving DD services. Caseloads average about 43 clients (a mix of children and adults). Case Managers meet with clients 2-3 times per year.
Requirements:
DD Case Managers must have one of the following:
A bachelor's degree in Social Work, Special Education, Psychology, Nursing, Human Services, or another field related to the education or treatment of persons with developmental disabilities or related conditions. If degree is in Social Work, Social Work licensure is required.
One year of experience in the education or treatment of persons with developmental disabilities or related conditions (QDDP qualified).
Completion of at least one course specifically focused on developmental disabilities (Fraser can provide this course free of charge).
Valid Minnesota Driver's License with acceptable record.
Ability to pass a DHS background study.

CADI/BI Case Managers must have one of the following:

Case Management
Meet with clients to review relevant information to assess needs for assistance with housing, utilities and a range of social services.
Complete assessments to help clients identify strengths and areas for growth to establish self-sufficiency; help to develop goals, strategies and action plans to produce desired outcomes; provide personal financial management and budgeting assistance; record visits and outcomes achieved.
Determine eligibility for CAA programs and services; approve participation based on program guidelines.
Collect and analyze information, verify documentation in accordance with program guidelines; enter data into appropriate tracking systems; ensure confidentiality of client personal and financial information.
Assist with identifying other community resources to address needs (e.g., hospitals, housing agencies and other non-profit organizations); coordinate referrals as appropriate; serve as liaison between clients and other community services providers.
Help clients access and navigate PRC and LIFT services as needed.
Administer and maintain accurate records of client referrals, ensuring the confidentiality of all personal and financial client information.
Follow up with regularly with clients, property owners and utility providers.

Ensure that the filing system is in compliance with regulations set forth by funding sources and agency management.
May travel to outreach sites and/or participate in weekend or evening events to administer program services to clients residing in the area.

Evaluate subcontracted vendor's programs for quality assurance by observing procedures at locations throughout the organization's territory.
Review and approve treatment plans and progress reports submitted by the subcontracted vendor. Assure authorization is active for each participant receiving subcontracted therapy and participant is making progress toward established goals.
Submit annual quality assurance reports for services for each participant focused on reducing the number of participant concerns that result in a grievance with the subcontractor.
Troubleshoot and resolve subcontracted vendor/parent concerns, e.g. expired authorization, monitoring vendor productivity, or ensuring participant receives appropriate level of care.
Evaluate effective subcontracted vendor service by reviewing participant assignments to a subcontracted vendor and the adequacy of communication to relevant parties (e.g., family, administrative assistant, etc.).
Participate in monthly CSCM meetings. Conduct annual vendor audits and corresponding written follow-up as assigned by the Director.
Prepare and report monthly caseload status including participant concerns and progress, and vendor concerns. Perform other duties as assigned.
Qualifications
Bilingual in Spanish required.
Master's degree from an accredited college or university with a concentration in early childhood education/development, early childhood special education, special education, psychology or related field.
Must be a Board Certified Behavior Analyst (BCBA); Marriage Family Therapist (MFT) or licensed psychologist preferred.
Three years of related professional experience working with children with Autism Spectrum Disorders (ASD) in a multi-disciplinary team setting preferred.
Advanced knowledge of scientifically-validated methodologies and approaches found to benefit children with ASD; familiar with current related research findings.
Expertise in approaches to intervention based on the science of Applied Behavior Analysis. Competent in employing and directing behavior analytic methodologies including Pivotal Response Training (PRT), Natural Environment Teaching (NET), Picture Exchange Communication System (PECS), Behavior Skills Training (BST), and Experimental Functional Analysis (EFA).
Expertise in all empirically evaluated assessment and intervention strategies related to program and service delivery for individuals with ASD.
Strong clinical, administrative, and leadership skills. Case management experience required.
Ability to speak a foreign language e.g., Spanish, Chinese, etc. preferred.
Able to interpret and implement policies, procedures, and regulations.
Able to consistently demonstrate good judgment and decision-making skills.
Ability to maintain customer service orientation and professionalism in all interactions.

WHAT YOU WILL DO:
The case manager position is designed to assist person served in identifying and acquiring both natural and paid supports which allow them to live as independently as possible in the community. The case manager should assist the individual in a person centered approach, in a way which supports the person in making his/her own choices and decisions about these supports. The case manager should function as an educator and a resource as well as service coordinator. Life decisions should be made by the individual and/or guardian.
The case manager will focus on coordinating services for vulnerable individuals and assisting them to navigate the complex service delivery system. The position will provide a systematic process of on-going assessment, planning, referral, service coordination, monitoring, consultation and advocacy assistance, through which multiple service needs of individuals are addressed. This position will be responsible for directly coordinating a caseload, and facilitate/coordinate other services or supports, which will positively influence individuals' lives.
The case manager will have contact with a wide range of organizations, families, and other community members. The case manager will act in a professional manner at all times, and understand that they represent the interests of the individual they support.
Work Environment and Hours:

Job Title: Clinical Case Coordinator
CASES is currently seeking a highly organized and detail-oriented Clinical Case Coordinator for our Pretrial Services Case Management Team. Rapidly after release into the program, the Clinical Case Coordinator conducts Supervision Introduction/Orientation meetings with new participants to review supervision conditions that will be required under the supervision plan. The Clinical Case Coordinator assists participants to comply with supervision appointments and court appearances during the pendency of their legal case. The Clinical Case Coordinator will conduct ongoing service need assessments with participants and develop collaborative service plans addressing identified needs and goals.
The goal is to increase engagement in community treatment services and support participants to achieve their self-identified goals for treatment, recovery, rehabilitation, and crime-free community integration. Program participants include youth and young adults, people with substance use disorders and mental illnesses, and individuals who are homeless. SRP participants have not been found guilty of a crime and are not mandated to engage in treatment or other services. This requires staff to tactfully deliver approaches to help participants tap into their intrinsic motivation to leverage their strengths and skills for meaningful life improvements while fulfilling all court requirements.

Work collaboratively with the court and other pretrial staff team to facilitate and ensure caseload participants' successful completion of mandated supervision appointments and court appearances during the pendency of their legal case.
Review legal and court screening documentation and work with participants to analyze their offending behavior patterns, identifying behavioral changes, need for implementation of supports that may reduce likelihood of any pretrial arrests.
On an ongoing basis, conduct service need assessments with participants to examine psychosocial needs and problems, including the severity of mental and substance use disorders, problems affecting participation, engagement, and continuity in supervision and treatment and individual motivation for treatment.
Link participants to treatment based on their eligibility, preferences and needs and provide escort to community services.
Maintain collaborative relationships with community providers (supported housing providers, Health Homes, care management programs, addiction treatment and mental health providers, peer-run providers, education, vocational. training and employment services providers, legal services providers, medical care providers, and family providers).
Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor.

What You Will Do:
Under the supervision of the Program Coordinator, provides community-based services to youth and adults with mental health and substance use disorders.
Provides the scope of mental health and substance use related case management services including, but not limited to: ongoing assessment of needs; skills to facilitate personal independence and daily living skills; coordination of the ITP including services identified within, linkage with natural support and formal community support service/systems; symptom monitoring; advocacy and outreach; interventions for symptom management; activities to report recovery including integration with self-help programs, etc.
Provides crisis support and stabilization activities including de-escalating persons in crisis, assisting therapists with facilitating hospitalizations for clients when indicated, actively assisting with safety planning, and coordinating with treatment team and afterhours services as need.

Position Summary

The Courts Department is seeking a highly skilled and compassionate Court Intake Social Worker to join our team. The social worker will play a crucial role in the initial assessment and referral process for individuals entering the court system, particularly those with mental health, substance abuse, or other behavioral health needs. The ideal candidate will have a strong background in clinical assessment, excellent communication skills, and a deep commitment to improving the lives of those in the community.
CASES is a New York City agency at the forefront of the intersection between behavioral health and the criminal justice system. Our mission is to increase public safety through innovative services that reduce crime and incarceration, improve behavioral health, promote recovery and rehabilitation, and create opportunities for success in the community. The CASES Court Services department services court mandates or endorsed programming at all points in the criminal legal process. CASES operates, alternative to detention (ATD) and alternative to incarceration (ATI) community-based programs that are facilitated by the NYS Office of Court Administration (OCA) judiciary. CASES interfaces with multiple stakeholders to support these ATD and ATI program options for thousands of defendants in multiple boroughs. Specifically, the arraignment team screens approximately 10,000 defendants a year for ATD and ATI programs, Supervised Release and Newstart. Additionally, the ATI Intake Assessment Team screens 500 people per year to divert people into 3 (three) CASES community-based programs: Reframing Opportunity Alternatives and Resilience (ROAR), Nathaniel Community Success (NCS), and Nathaniel Assertive Community Treatment (NACT). All Court Services teams are dedicated to the CASES mission and facilitating release and positive court outcomes.
Salary: The salary for the Court Intake Social Worker position is $61,800 - $70,000 per year.
Shift Hours: The Court Intake Social Worker role is full time, Monday-Friday 9am-5pm.

What you will be doing:

Complete all necessary intake paperwork, including entering participant information into the ECM/AWARDS databases.
Participate in clinical supervision, case review meetings, staff meetings, in-service training programs, relevant quality improvement, coaching, and planning activities.
Provide culturally competent services ensuring the consideration of the role of ethnic/racial, cultural, social, economic, and behavioral factors in the accessibility, availability, and delivery of evaluation services.
Mentor less experienced colleagues in the intricacies of Court procedures, and CASES programs, especially eligibility and clinical criteria.
Help ensure we maintain a safe working environment by reporting any hazards or unsafe conditions, e.g., liquid spill in hallway, to the appropriate facilities staff member or supervisor.
