Vital Signs: Take and record residents' vital signs, such as blood pressure, temperature, pulse, and respiratory rate, and report any changes or concerns to the supervising nurse. Mobility Assistance: Help residents with walking, using canes, walkers, or wheelchairs, and transfer them safely between beds, wheelchairs, and other locations as needed. Comfort and Safety: Monitor residents to ensure they are comfortable and safe. Report any changes in behavior, discomfort, or safety concerns promptly. Documentation: Accurately record residents' health information and daily activities, following facility and HIPAA guidelines for privacy and confidentiality. Communication: Collaborate effectively with the care team, communicate residents' needs, and respond to requests promptly. Infection Control: Follow all safety, health, and infection control policies and protocols to maintain a clean and safe environment. Additional Tasks: Perform other duties as directed by supervisory nursing staff. Qualifications: Certification: Active CNA certification in Pennsylvania. Education: High school diploma or GED. Experience: Prior experience in a skilled nursing facility or long-term care setting preferred. Skills: Strong communication and interpersonal skills, with a compassionate and resident-centered approach. Ability to work effectively as part of a team and under the supervision of RNs/LPNs. Physical Requirements: Ability to stand, bend, lift, and move for extended periods, with the capability to lift up to 50 pounds. Comfortable providing personal care and physical support to residents.
