* Performs a variety of clerical duties
* Identifies and trouble shoots technology failures and handles computer downtime
* Independently aids Caregivers, including daily rounding and delivering necessary office supplies
* Operates and utilizes office equipment and software applications, including Word, Excel, PowerPoint, Copy / fax Machine, and answering and directing phone calls
* Communicates with patients, families, members of the health care team and others in a professional, diplomatic and empathetic manner
* Coordinates as needed with Homecare, LTACH, Mental Health Services, sub-acute rehab, hospice, Human Arc, DME suppliers, maintains DME asset location for Patient Support Services
* Assists with compiling the necessary supportive documents required for patients transitioning to a different site of care
* Responds in accordance to the appeal process as it relates to Medicare & Medicaid
* Faxes Children Specialized Health Care Services and Early On applications as needed, SSI packets, and maintains handouts for families
* Maintains and updates contact information for the available community resources
Job Requirements
General Requirements BLS - preferred Work Experience Minimum of 6 months previous secretarial experience Education High school diploma, GED or successful completion of 12 grades combined with continued enrollment in early college program Associates degree in Business or related field - preferred Specialized Knowledge and Skills Clerical skills necessary to perform data entry, prepare reports, and tabulate routine data Interpersonal skills for effective communication Knowledge of basic medical terminology, health insurance, policies, or procedures Ability to view a computer CRT for prolonged periods Able to respond to and prioritize multiple requests and variable degrees of unpredictability requiring adaptability in work activities Demonstrated working knowledge of computer applications/ software's, including Epic and keyboard functions Demonstrates ability to use a keyboard as may be required to perform the essential duties of the job. Proficient with Microsoft Outlook, Word, Excel at the beginner level..

Patient Care Coordinator - C3 (Pediatrics/Women's/Children's Services)
C3 is a 17 bed unit that provides pediatric women's and children's services, plastic and reconstructive surgery and general medicine overflow. C3 accepts Maternity overflow, medical surgical patients undergoing gynecologic procedures, plastic and reconstructive, and mastectomy surgery. Pediatric Intermediate Care provided.
Maintains and evaluates patient and staff data collection systems and processes to support the practice of nursing and delivery of patient care within their defined areas/s of responsibility.
Evaluates assessment practices to assure timely, reliable, valid and comprehensive data collection.
Diagnosis/Problem Identification-Fosters and evaluates an environment that empowers and supports staff, including the professional nurse in analysis of assessment data and in decisions to determine relevant diagnoses.
Identification of Outcomes-Maintains and evaluates processes that promote RBC and desired patient-centered outcomes that are culturally sensitive and inclusive of patient/family needs according to accepted standards of clinical nursing practice.

Obtain all needed patient related information including diagnostic records, lab results etc.. Verify all appointments/procedures in EPIC/Mosaiq. Greet and check in patients for testing using AIDET. Support Physicians/Administration/Caregivers in their daily needs.
Essential Duties:

* Interviews patients and physicians office staff for the purpose of entering confidential patient insurance and demographic information into the EPIC/Mosaiq scheduling system.
* Serve as Liaison for Centralized Scheduling. Assures that improvement measurements are implemented.
* Schedules patient procedures using EPIC/Mosaiq obtaining all necessary information and coordinating procedures with physicians, radiation oncology staff, equipment and personnel availability.
* Obtains insurance authorizations as needed.
* Coordinate the compiling of patient records to facilitate patient rounds and quality assurance matters.
* Monitor accuracy of scheduled orders coming through Centralized Scheduling.
* Process patient orders via EPIC/Mosaiq.
* Support scheduling of radiation oncology appointments.
* Answer all incoming, outgoing and inter-hospital phone calls using AIDET. Relays information to proper party, takes messages, contacts necessary person, and answers inquiries.
* Handles confidential information on a daily basis and maintains HIPPA compliance.
* Types reports, communications, records and forms as well as copying, filing, and delivery of these materials. Performs all other secretarial duties as assigned
* Coordinates ordering needed supplies for department.
* Initiates pages to physicians and staff.
* Maintains patient medical records.
* Instruct new Caregivers in all procedures and responsibilities.
Job Requirements
General Requirements • None Work Experience • Minimum of two years advanced secretarial training and/or experience. • Minimum of 6 months experience with EPIC scheduling applications within the past year • Preferred- Knowledge and experience with Mosaiq scheduling system. Education • High school diploma • Preferred- Professional certification. Specialized Knowledge and Skills • Position requires self-discipline, excellent communication skills, a high degree of personal motivation and excellent teamwork skills • History of outstanding customer service skills • Ability to multi task • Strong attention to details • Work history that exhibits outstanding dependability and ability to work independently • Proficient in medical terminology. • Proficient word processing and dictation/transcription skills. • Use multi-line phone • Knowledge and working experience with Excel • Demonstrate knowledge of EPIC

* Greet and assist all patients, visitors or other persons upon entrance into the department.
* Answer all incoming telephone calls, direct calls to the appropriate persons or department.
* Schedule patient appointments and obtain information for registration and billing.
* Compile, maintain, and distribute daily and weekly rehabilitation schedules.
* Prepare pertinent information for new patient charts in an organized manner. Prepare charts for discharge following department guidelines. File pertinent information into charts as the need arises.
* Mail or fax appropriate information including new prescription forms, plans of care, progress notes, and discharge notes.
* Track Medicare/Medicaid and Blue Cross plans of care and progress notes to ensure physician's signature.
* Sort and distribute departmental mail.
* Demonstrate effective use of all office equipment.
* Perform routine clerical tasks and assists other clerical associates with office duties as needed.
* Enter ICD-9 diagnostic codes, and daily patient charges into the STAR system as assigned.
* Monitor, inventory, and order departmental supplies. Maintain inventory of department therapy supplies and purchasing records. Generate purchasing requisitions.
* Responsible for collecting, receipting and balancing monies received from patients.
* Be familiar with information requirements for admission including prescriptions, insurance information, diagnosis, etc.
* Be familiar with procedures for insurance pre-authorization and ensure that pre-authorizations are obtained on a timely basis prior to treatment.
* Discharge patients from STAR system.
* Cooperate with outpatient rehab staff and other health system departments in order to maximize the efficiency of non-clinical services such as the registering, scheduling, and billing of our patients.
Job Requirements
Work Experience See Education section Education High school diploma or G.E.D and minimum of 2 years clerical experience working in a medical office setting OR Bachleor's degree in related health sciences/administration Graduate of an accredited trade school or community college focused on medical office administration preferred. Specialized Knowledge and Skills Working knowledge of Microsoft Office Applications. Effective written and verbal communication skills, and strong interpersonal skills. Able to prioritize and keep pace in a busy setting. Able to perform multiple tasks and adapt to changing situations. Knowledge of medical terminology. Familiarity with various insurance programs and their benefit level preferred.
