Job Requirements We are hiring a Registered Nurse to join our Home Health team working Part Time! * Part-Time; 8:00am to 4:30am At Spartanburg Regional Home Health, we are proud of our skilled, caring, trustworthy and compassionate Home Health experts. Our clinicians visit patient homes to deliver customized, physician-prescribed care after a hospital stay, illness or injury to rehabilitate and achieve optimal level of independence. Home Health is an alternative to repeated hospitalizations or an extended hospital stay. Nurses and medical professionals work closely with primary care physicians and ensure patients are receiving optimum care tailored to individual needs.
Our Registered Nurses enjoy autonomy while caring for patients in their home environment. The team works with a wide variety of cases such as: Wound Care, Cardiac, Diabetes, Endocrine, Musculosketal, Pressure Ulcers, Infusion, etc. The average length of patient care is 47 days.
The Registered Nurse provides skilled nursing services within the home care setting in accordance with the orders of a physician and under a plan of care. Through use of the nursing process, utilizes knowledge and skills at a competent level to provide direct age-specific and culturally appropriate patient care management in the homecare setting. The RN also serves as a clinical resource, participates in patient and staff education, demonstrates the use of independent nursing judgment, and serves as a preceptor. Develops and coordinates the plan of care and provides case management. Performs other duties as assigned.
Minimum Requirements
Education
* Graduate of an approved nursing program
Experience
* One (1) year of recent clinical nursing experience
License/Registration/Certifications
* Current license to practice as a Registered Nurse in South Carolina
* Valid SC driver's license with good driving record
* CPR certification
Core Job Responsibilities
* Responsible for continuous review of all aspects of patients on his/her caseload to include but not limited to: appropriate utilization of services; review of documentation in the medical record; maintenance of interdisciplinary communication and discharge planning.
* Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es) and medication reconciliation.
* Collaborates with patients, families, and other members of the interdisciplinary care team to establish appropriate treatment plan and develop a plan of care.
* Initiates the plan of care, makes necessary revisions as patient status, and needs change. Regularly re-evaluates patient nursing needs.
* Initiates and facilitates interdisciplinary collaboration to enhance outcomes.
* Initiates appropriate nursing interventions. Provides treatments as prescribed by the physician.
* Provides health care instructions to the patient as appropriate per assessment and plan. Educates the patient and family/caregiver on self-care techniques medications, diet and other instructions as ordered.
* Recognizes and utilizes opportunities for health counseling with patients and families/ caregivers.
* Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; communicates regularly with the interdisciplinary team; communicates with community health related persons to coordinate the plan of care; and communicates on a regular basis with manager.
* Provides supervision of home health aides per regulatory requirements.
* Complies with established policies and procedures.
* Knowledge of and compliance with state, federal, and local regulations for the delivery of home care services.
* Performs other duties as assigned.

Become the Case Manager you always wanted to be
Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans
Participate in planning for and the execution of patient discharge experience.
Monitor patient experience: quality/timeliness/service appropriateness/payors/expectations.
Facilitate team conferences weekly and coordinate all treatment plan modifications.
Complete case management addendums and all required documentation.
Maintain knowledge of regulations/standards, company policies/procedures, and department operations.
Review/analyze case management reports, including Key Care Indicators, and plan appropriate actions.
Understand commercial contract levels, exclusions, payor requirements, and recertification needs.
Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs.
Meet with patient/family per Patient Arrival and Initial Visit Standard within 24 hrs. of admission.
Perform assessment of goals and complete case management addendum within 48 hours of admission.
Educate patient/family on rehabilitation and Case Manager role; establish communication plan.
Schedule and facilitate family conferences as needed.
Assist patient with timely procuring/planning of resources to avoid discharge delays or issues.
Monitor compliance with regulations for orthotics and prosthetics ordering and payment.
Make appropriate/timely referrals, including documentation to post discharge providers/physicians.
Ensure accuracy of discharge and payor-related information in the patient record.
Participate in utilization review process: data collection, trend review, and resolution actions.
Participate in case management on-call schedule as needed.
Qualifications
- License or Certification:
Must be qualified to independently complete an assessment within the scope of practice of his/her discipline (for example, RN, SW, OT, PT, ST, and Rehabilitation Counseling).
If licensure is required for one's discipline within the state, individual must hold an active license.
Must meet eligibility requirements for CCM or ACM™ certification upon entry into this position OR within two years of entry into the position.
CCM or ACM™ certification required OR must be obtained within two years of being placed in the Case Manager II position.
- Minimum Qualifications:
For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirement for licensure or certification is a diploma rather than an associate degree.
For all other eligible licensed or certified health care professionals, must possess a minimum of a bachelor's degree and graduate degree is preferred.
2 years of rehabilitation experience preferred.

This is a full time opportunity for experienced NICU Registered Nurses with minimally two years of NICU experience. Unit shift hours are 6:00 PM- 6:30 AM. Weekends, holidays and mandatory on call shifts are required. NRP certification within 90 days of employment required. Provides professional nursing care to patients and families in an acute clinical care environment, guided by the SMHCS nursing professional practice model and in accordance with the ANA's Standards of Professional Practice.
Salary Details:
Hourly rate is determined by years of worked experience as an RN, education level, and internal equity within organization. Minimum rate starting at $34.10, max of salary range is $52.80. In addition to hourly rate, if candidate holds an approved certification position qualifies for certification pay of $1.50.
Required Qualifications
- Require basic working knowledge of commonly used computer applications.
- Require graduate of an accredited school of nursing or a diploma program. For new hires after 2012, Bachelor of Science Degree in nursing (BSN) is required within specified timeframe as outlined in the New Hire BSN Agreement policy (126.045).

W2 full-time physicians earn on average $300,000 per year plus benefits.
Appreciate the autonomy and flexibility in our work schedules, routinely a 45 hour work week for full time positions.
Paid Training
Requirements:
Active unrestricted medical license (MD/DO) in the state you wish to practice
Minimum commitment of 3 weekdays per week.
Practice Overview:
Full time work: Monday - Friday (7am-4pm) or part time available.
No call / No weekend clinical responsibilities.
Physicians deliver modern wound management at the bedside in the post-acute care setting: Skilled Nursing Facilities and Long-Term Acute Care Hospitals.
Our physicians conduct weekly wound rounds at each facility.
Part-time role is a minimum of 3 days per week. Full time role is 4 - 5 days per week.
This is a mobile practice: The physicians visit facilities in a mile radius of practice location.
Our physicians visit 2-3 facilities per day treating approximately 20-30 patients per day.
Clinical documentation is at point of care, in real-time during clinical rounds, using our proprietary wound specific EMR.
Paid Training is provided during onboarding + 6 months wound care fellowship
W2 employment contract (No work visa sponsorship is available)
