What you'll do for DDI:
Complete screening, initial and annual assessments ensuring paperwork is completed and filed appropriately in each individual's record
Conducting functional assessments for the purpose of behavior support planning, design and monitoring (involving observation, interview with staff)
Author behavior support plans based on positive behavior support
Implement and monitor ongoing data collection to assess progress of behavior support plan and identify need for referral to Human Rights Committee where applicable
Observe staff working with individuals to ensure BSP is being followed
Provide feedback and modeling for staff on implementation of behavior support strategies

Train and supervise new staff to work with individual(s) served and ensure active treatment for the shift you supervise.
Assist Program Manager with solving staff issues, disputes, and completing required paperwork.

What you'll do at DDI:
Meal preparation, cooking, shopping, and meal planning, in coordination with menus prepared by Nutritionist.
Follow all menus & fill in substitutions according to exchange list, follow diets; gluten free, halaal, kosher style and ensure individual meals if necessary dependent on student.
Prepare meals to use in cooking; chopping/peeling/cutting meat
Cook utilizing various methods; utensils/grills/ovens
Be able to measure ingredients as recipe calls for
Maintain accurate food inventories
Ensure all food not utilized stored properly
Report any maintenance needs or hazardous conditions to Manager or Maintenance Person.
Notify supervisor immediately if house or appliances need repair.
Be familiar with and follow OPWDD regulations regarding food, kitchen area.
Meet with nutritionist in setting up appropriate dietary menus for individuals in residence
Help schedule and plan parties for special occasions.
Answer telephone in a courteous and professional manner & take appropriate messages.

What you bring to DDI:
Ability to follow both verbal and written directions.
Proven experience as a cook ( at least one year experience)
Ability to work well with staff members and the individuals we support.

Must be in good physical condition and be able to bend, twist, and reach in the performance of your duties.

What You Bring To DDI:
High School Diploma
One (1) year experience working with developmentally disabled adults, required.
Valid NYS driver's license and approved to drive by DDI's Transportation Department
Working knowledge of Applied Behavior Analysis and Positive Behavioral Support
What You'll Do:
Responsible for daily supervision of staff activities and creating a comprehensive daily schedule for each shift. Knowledge of Emergency Procedures and accurate implementation of procedures in handling crisis situations as they arise.
AMAP Certification - Obtain within the first 90 days in position and keep current.
Train and supervise new staff to work with individual(s) served and ensure active treatment for the shift you supervise.
Assist Program Manager with solving staff issues, disputes, and completing required paperwork.
Instruction and documentation of formal goals and protocols in accordance with the CFA/ILife Plan/Staff Action Plan.
Implementation of Behavior Plans and all other components of the annual plan.
Evaluate (monthly) progress as related to short term objectives and formulation of new objectives based on level attained. Write Monthly Summaries.
Accompany individuals on medical appointments. Ensure staffing and completion of medical appointments.
Ensure completion of documentation, including incident reports, SCIP-R reports, body checks, logs, etc.
Participate in the development and revisions of formal goals, protocols, and Behavior Plans
Participate in Interdisciplinary Team Meetings as requested., Attend staff meetings and annual recertification trainings.
IRA's- Ensure billing data entered into electronic health record by the end of each shift.
Ensure completion of CI responsibilities
Completion of assigned responsibilities.
