    CONTRACTOR SALARY: $21/HR Scope: Under the direction and supervision of the Home Care Manager, the Personal Care Worker (PCW) is responsible for providing and assisting clients with activities of daily living and supportive care necessary to the preservation of the home environment. The PCW will assist in monitoring the health status, providing a safe and positive living environment to enhance the client's quality of life. Job Responsibilities: Provide unskilled personal care services and homemaking services. Accompany clients to appointments, on errands and shopping as appropriate. Provide assistance or stand by assistance with bathing, skin care, hair care, toileting, dressing, shaving, nail care, mouth care, etc. Assists with IADLs (grocery shopping, cleaning, changing bed linens, laundry, meal preparation and other routine household care) Observe clients taking prescribed medications. Clean stove, kitchen, bathroom, and only the client's living area and bedroom Vacuum, sweep, dust, take out trash, run errands as needed, mop floors. Advises Program Manager of requests which cannot be met or of conditions which are unsafe. Record and report observed noticeable changes in client's physical condition, behavior, or appearance immediately to Program Manager and/or nurse. Documents services provided to clients consistently and accurately. Documentation is completed in a timely manner. Participates in agency required and assigned training programs and any required staff and training meetings. Document daily activities and report any significant changes in a client's needs or living conditions. Additional responsibilities may be added as appropriate. Essential Job Requirements: Minimum High School Diploma or GED Ability to lift, push, or pull 25 pounds. Ability to bend, twist, stoop, kneel and reach. Ability to read, analyze and interpret regulations and other documents. Must have reliable transportation. Possess a valid state issued driver's license and good driving record. Attend appropriate Resident Care conferences; maintain Resident Care Plans ensuring accuracy and relevancy.
