The Breakwater Program is a long-term treatment program for adolescent males referred by the Department of Children and Families. As a therapeutic community, we use trauma-informed care, cognitive behavioral therapy, family support, and recovery-based approaches to substance abuse treatment. Our goal is to help youth build pro-social skills, support their recovery, and work toward a stable, productive future. Recovery Specialists play a key role in guiding and supporting youth through this process.

Summary:
A Behavioral Resource Technician ("BRT") is responsible for providing safety observation of the
patient or patients they are assigned to. The BRT's role is to ensure that the patient does nothing to
harm themselves or others. The BRT must maintain a safe environment and interact with the patient,
family, and staff in a caring professional manner. As a member of the health care team, the BRT will
provide care for the patient and may include bathing, feeding, toileting, ambulating, turning, and

5) Maintains A Safe Environment For The Patient: Remains with the patient at all times if assigned to one-to-one observation unless relieved by a member of the staff. Must remain awake and alert and have the ability to provide safety observation as defined in the Care of the Suicidal Patient policy, even when the patient is sleeping. Demonstrates knowledge and skill in maintaining a safe environment and can identify ligature and safety risks in any environment of care. Accompanies the patient off the unit as directed by the nurse and remains with the patient at all times in these areas. Transports patients in wheelchairs and on stretchers to procedures. Reports any safety hazards to the RN.
6) Understands the care for specific diagnoses such as suicidal, eating disordered, drug or alcohol withdrawal, PICA, Alzheimer's, anoxic brain injury, etc.
7) Communicates to patients that they are a high priority even during busy times. Treats patient information as confidential. Displays sensitivity to patients through non-verbal communication. Utilizes body language such as eye contact, smiling, etc., to demonstrate an interest in the patient.
8) Participates in Competency Validations, Educational Activities, and Staff Meetings. Completes all mandatory in-services and web strategies 100% of the time. Attends and participates in staff meetings. Attends annual mandatory validations. Attends all additional programs as required. Continually seeks to improve skill level and knowledge base.
9) Demonstrates competency for one-to-one observation of patients based on the care of the suicidual patient policy. Staff providing patient safety observation must remain awake and alert at all times.

Has a working knowledge of emergency response plans and assists the supervisor and Emergency Management Director in setting up the emergency operations center (EOC). They will also notify the Police when appropriate.

1) Bed Management: Tracks, prioritizes, and assigns beds. Communicates with BMD and BM for admissions, discharges and transfers (ADTs). Coordinates unit/department ADTs. Checks BMD enters new data, and advises Charge Nurse of changes. Checks CIS for discharge orders and follows up with MD, CM RN to ensure discharge documents are complete. Enters discharges in OA Gold and pending discharges in BMD. Rounds 3 times daily with RNs and CM regarding discharges, discharge times and special instructions, and appropriateness for Discharge Lounge. Notifies CM if an ambulance is unable to come @atthe assigned time. With CM, monitors BMD for observation patients to determine if patient needs discharge or status change. 2) Proficiently and accurately utilizes the following computer applications in daily work: C Word, OA Gold, SMS, CIS, Premise, BMD, PCM Auditor, People Soft and Staples, and Kronos access if applicable.
3) Unit-specific projects: Project work as assigned and/or needed such as Payroll, committee participation, developing unit time schedules, etc.
4) Communication: provides report/handoff to next shift including pending discharges, admissions in process, empty beds, beds being cleaned, and closed beds with the reason. Contacts other departments for priority tests for patients identified for next day discharge. Answers telephones and relays messages promptly with consistent accuracy to team members. Assist Hospitalist during clinical rounds, i.e. initiate phone calls, send faxes and call consults as directed by Zoned Hospitalist. Make available the unit based assignment (RN/Case Manager/Social Worker as well as covering RNs [i.e. lunch and breaks]) daily for Hospitalist. Maintain and identify specific workspace for Hospitalists which include computer and telephone access. Works with Access Services and patient/family to secure post discharge follow-up appointments. Identifies daily the Hospitalist of the day through electronic systems and prints this form for team use throughout the day. Posts break and lunch coverage in a central location for ease in obtaining patient related information and fostering communication.
6) Functions as a greeter for the arrival of new patients/families to the unit/department. Assists other departments with information and or answers to questions as requested.
7) Hospital Records and logs:
Maintains patient chart in proper sequence and labels pages with patients name. Prepares record prior to transfer and discharge. Explains and obtains signatures for Medicare forms. Organizes chart for Medical Records, faxes or copies records as needed. Sends discharge record to HIM (old and new) immediately after discharge. Updates discharge/admission log. Insures complete information is sent with transfers. Prepares patient charge ticket for data entry and keeps accurate record on unit. When appropriate, enter data into PCM auditor at specified times. Replenish and restock unit audit tools (i.e. refrigerator logs, negative pressure room logs, etc.). Other unit logs i.e. assignment books, sick call records, Kronos variance forms, etc.
8) Unit Resource: Post/file MD schedules for clinical team inquiry. In coordination with charge nurse, prepare staffing assignment for the next shift. Place calls to replace unplanned staffing vacancies. communicate staffing and scheduling to Clinical Supervisor. As needed, but not limited to obtain mail from the mailroom, blood products from the Laboratory, medications from Pharmacy and supplies from Distribution and Dietary as needed.
9) Inventories, maintains and charges supplies. Initiates requisitions and requests accurately and in a timely fashion. Stores supplies within 24 hours of receipt in designated area. Records or submits charges and/or credits for patient supplies as needed. Coordinate paper supplies with Print Shop and/or Distribution to maintain unit/department par levels.

Summary:
The Expressed Human Milk Technician (EHMT) will prepare pumped breast milk according to physicians' orders for the population of infants served in the NICU and CCN for a 24-hour period. Under the direction of the Revenue Cycle Registration Manager/Supervisor, the ED Registration Representative is responsible for interviewing patients and collecting patient demographic and insurance information in a fast-paced, high-stress Emergency Department environment. Collects necessary patient responsibility payments at the time of registration. Summary:
Under the supervision of the Radiologist and according to established policies and procedures, operates imaging equipment as required, to obtain diagnostic images according to Radiologist specifications; will assist the Radiologist in procedures as required.
Responsible for coordinating patient flow, operating all systems for routine, specialty, and research studies, supporting protocol development, and performing all duties related to scanning and image processing for their assigned specialty. Ability to operate ultrasound machine to produce cross-sectional images of patient's body for diagnostic purposes where applicable. Through the use of independent judgment and initiative is able to perform imaging sequences necessary to reach a medical diagnosis and decision. Must be able to provide professional and quality care while following hospital policies, procedures, and standards. May be responsible for specific areas of instruction in the training of new staff or others in specialty assigned areas for procedures, protocols, equipment and/or safety issues.
May float to other locations/hospitals in the Baystate Health system to support patient care and staffing needs.
Job Responsibilities:

Perform technical procedures which may include one or more of the following; abdominal, OB/GYN, trans-vaginal, trans-rectal, scrotal, thyroid, breast, and vascular.
3) Maintains knowledge of procedures/protocols and applies knowledge of cross-sectional anatomy and is able to identify pathology, within the scope of practice to obtain necessary diagnostic information. Responsible for independent and proficient operation of all assigned imaging equipment in the area of specialty to provide the best image outcomes in a proficient and time-sensitive manner.
4) Properly identifies patient information using two patient identifiers and complies with National Patient Safety Goals standards. Performs procedures as outlined including but not limited to routine, specialized, emergency, and urgent imaging procedures. Responsible for providing accurate, high-quality exams.
5) Maintains operational communication with Supv/Mgr; shares information and follows through with assigned tasks.
6) Works as a member of the healthcare team including physicians, nurses, and other healthcare professionals in diagnostic testing.
7) Operates and maintains exam room and equipment: evaluates equipment problems and takes appropriate action to expedite necessary service or repair. Demonstrates knowledge/understanding in effectively operating all equipment. Performs quality checks on equipment in accordance with departmental procedures. 10) Performs accurate and high-quality exams to assure proper technique, positioning, and visualization of the appropriate anatomy requested. Instructs patients before and during the procedure to maximize the probability of obtaining a complete and accurate result.
11) Maintains standards of skills: demonstrates/maintains current knowledge/skill in providing appropriate care for all patients potentially served in an imaging service, including pediatric, adolescent, adult, and geriatric.
12) Presents self to all patients and other customers in a pleasant, professional and courteous manner at all times.
13) Attends and participates in in-service meetings and other designated training events that will enhance clinical skills as documented by attendance at meetings/seminars.
14) Reports any suggestions for positive changes or recommendations within departmental policies/procedures to the supervisor.

Short Description
The auditor reviews professional fee billing, coding and documentation. Reviews to be performed are identified based on the then-current OIG Workplan and compliance risk analyses. Customers include employed providers, senior leadership, clinical and non-clinical staff of Cooper University Health Care.
Under the supervision of the Chief Compliance Officer, auditors are responsible for supporting the corporate compliance program, responsibilities include:
Performance of timely and effective compliance and operational reviews to assess coding, documentation and billing accuracy, identify compliance related risks, internal control weaknesses, revenue capture opportunities and assist in determining the root cause of any identified non-compliance with government rules and regulations, state laws and Cooper policies and procedures
Preparatory work for reviews/audits including developing a scope of work.
Reviewing available documentation.
Analyze/review audit data and prepare reports for review and presentation to management, providers and departments, making recommendations for improvement
Determine charge corrections and refunds resulting from compliance reviews and ensure they have been completed.
Post-review/audit education/training when applicable.
Performing follow-up reviews when necessary.
Ensuring appropriate work papers, either paper or electronic, are maintained in accordance with regulations/policy
Assist in the development of policies and procedures that establish standards for compliance, as well as preparation of other guidance documents and tools to assist Coper providers and staff in appropriate billing, coding and documentation.
Serve as liaison for questions, concerns, incidents and complaints regarding compliance matters, responding directly to the inquiry and/or consulting or interacting with other team members or departments. Inform Chief Compliance Officer of major findings; based on types of questions/concerns received, recommend remedial correction and prevention actions; identify education/awareness opportunities and guidance topics
Work with all levels within the organization to ensure that internal controls throughout the system provide for accurate, complete and compliance program and processes
Experience Required
3+ years' experience in an academic medical center preferred, with emphasis on provider compliance activities, including but not limited to: auditing, monitoring, investigation and training
Demonstrated knowledge and understanding of provider professional fee billing, coding and documentation practices in inpatient and outpatient settings.
Demonstrated expertise in medical terminology.
Demonstrated expertise in healthcare coding (CPT, ICD-9, ICD-10, APC, HCPCS).
Demonstrated knowledge and understanding of HIPAA rules and regulations affecting the management of confidential protected health information (PHI).
Demonstrated knowledge and understanding of federal and state statutes, laws, rules and regulations affecting billing, coding and documentation practices in support of healthcare services provided to beneficiaries of federally-funded healthcare programs and other third party payers.
Demonstrated knowledge and understanding of the essential elements of an effective compliance program
Working knowledge and understanding of:
- provider professional fee revenue cycle and reimbursement.
- electronic billing and medical record systems
- sampling technologies and statistical analyses
.Experience using personal computers required.
Experience using the following applications is desirable: Word, Excel, e-mail, and healthcare related billing systems.
Experience using MDAudit audit software and/or EPIC EMR desirable

Position Summary
The Sterile Processing Technician performs a wide range of duties related to the decontamination, inspection, assembly, and sterilization of surgical instruments and equipment in accordance with manufacturer instructions, hospital policies, and regulatory standards. This role requires strong knowledge of infection control practices, attention to detail, and the ability to work efficiently in a fast-paced healthcare environment.
Key Responsibilities
Perform decontamination of biohazardous materials using automated processors and manual methods while adhering to infection control and safety standards
Inspect instruments and equipment for cleanliness, function, and integrity prior to assembly
Assemble instrument trays and specialty sets accurately using reference materials and surgical instrumentation knowledge
Operate steam, ethylene oxide (EO), and low-temperature gas plasma sterilizers according to manufacturer instructions and departmental policies
Monitor and document all sterilization parameters, including biological and chemical indicators
Conduct quality checks to ensure sterile goods meet package integrity and expiration requirements
Maintain accurate documentation and perform clerical duties related to sterile processing workflows
Maintain a clean, organized work environment to support efficiency and safety
Prioritize work assignments to meet the needs of surgical and clinical customers
Collaborate effectively with peers, nurses, physicians, and other healthcare professionals
Inspect sterile goods for package integrity and expiration dates

Summary:
Under the supervision of the Radiologist and according to established policies and procedures, operates CT and X-ray equipment as required, to obtain diagnostic images according to Radiologist specifications; will assist the Radiologist in protocols/procedures. The CT/X-ray Technologist performs exams in compliance with regulatory agencies and hospital policies and procedures. The incumbent selects appropriate imaging parameters according to the knowledge of standard settings to produce exams of diagnostic quality with 90-95% accuracy. Assists radiologist in specialized exams such as biopsies, ablation, and abscess drainage. Consistently follows proper patient screening/care protocols and provides accurate documentation associated with each exam while supporting expedited patient throughput and diagnosis. Ability to operate multi-slice Computed Tomography (CT) scanners to produce cross-sectional radiographs of patient's body for diagnostic purposes by performing the following duties. Safe operation and care of CT equipment and exposure of patients to radiation for diagnostic information. Must be able to provide professional and quality care while following hospital policies, procedures and standards. Performs all duties of a CT technologist on a daily basis and provides direct patient care to patients ranging in age from pediatric through geriatric. Perform a variety of technical procedures that will require independent judgment with ingenuity and initiative to supply prescribed ionizing radiation for radiographic diagnosis.
This position will rotate through both CT and X-ray shifts as needed.
Job Responsibilities:
1) Sound knowledge and proficiency in the use of CT Scan equipment.
2) Perform CT exams as per protocol on a variety of different body parts in a timely manner.
3) Complete appropriate history forms, technologist exam sheet and check labs when necessary.
4) Properly identifies patient information using two patient identifiers and complies to the National Patient Safety Goals standards, ARRT Code of Ethics Standards and embodies the Cultural Compass. Performs procedure as outlined in the job description and practices the ALARA principle at all times.
5) Performs accurate and high quality exams by using the ALARA principle to assure proper technique, positioning and visualization of the appropriate anatomy requested.
6) Responsible for accurately inputting data, demographics and information into computer information systems, e.g., RIS, Synapse, Radnet and verifies timely transfer of images to the PACS system..
7) Reviews authorization /requisition and other companion paperwork to verify type of examination to be performed, to be alert for contra-indicators and suspected pathologies with 90-95% accuracy. Ensures proper imaging/treatment procedures by checking with nursing personnel, physicians and/or radiologists whenever questionable exams are ordered.
8) Explains nature of procedure completely and in laymen terms, in order to allay apprehension and elicit cooperation. Validates that patient understands procedure by questioning or other means.
9) Ability to start I.V.s independently and administer contrast media with power injector.
10) Performs quality checks on equipment in accordance with departmental procedures and and takes appropriate action to expedite necessary service or repair.
Required Work Experience:
1) Satisfactory completion of a formal radiologic technology program in an AMA approved school preferred.
2) Must have the ability to work independently and be self-motivated.
Preferred Work Experience:
1) Any combination of education and experience that would likely provide the required knowledge, skills and abilities as well as possession of any required licenses or certifications is qualifying.
Skills and Competencies:
1) 1 year in CT in an acute care setting.
2) Technical skills in CT with understanding of cross sectional anatomy, 3 D post processing, Computer skills, power injector, IV skills and CPR.
