Full-time: 31 East Main Street, Rochester, NY SUMMARY OF POSITION FUNCTION: The Forensic Housing Peer Specialist uses lived experience with incarceration, successful reintegration and/or the Behavioral Health System to provide mentorship, support, and advocacy to individuals who were formerly incarcerated at NYS Correctional Facilities. The Peer Specialist helps clients navigate resources, overcome barriers and build a stable healthy life after being released into the community. MAJOR DUTIES AND RESPONSIBILITES: Knowledge and demonstration of agency core values in day-to-day activities Assist individuals with their Daily Living Skills Assist with home visits to client residence Provide engagement and support for individuals experiencing social, emotional, medical, developmental, financial, and substance use challenges in their homes and/or communities. Provide education, peer counseling, and assist with modeling effective coping skills. Work with care coordinators and other community providers Self-advocacy, self-efficacy, and empowerment Promote advocacy on behalf of individuals and support self-advocacy Assist in the development of and utilization of self-help tools if applicable. Sharing personal experiences to build rapport inspires hope Facilitate or co-facilitate peer support groups, workshops or educational sessions. Collaborate with Legal entities such as parole/probation officers, re-entry coordinator, court systems, Department of Social Services, etc. to assist with successful transitions. Assist individual in obtaining vocational and employment opportunities Provide Critical Time Support upon immediate release from incarceration Build and maintain community connections and natural supports SKILLS/COMPETENCIES: Proficient in the use of computer software such as MS Word, Excel, Power Point, Desktop Publishing, Outlook, etc. EDUCATION REQUIREMENTS: Current NYCPS Certified Peer Specialists Credential or CRPA Certified Recovery Peer Advocate or working towards obtaining it High School Diploma or GED EXPERIENCE: Preferred One-year work related system Utilization of Electronic Data Systems Lived experience in the Criminal Justice System and/or Behavioral Health System.     The ECMO Specialist is enrolled and actively participating in the department's ECMO Training Program. This role is responsible for developing and maintaining the skills necessary to proficiently and safely establish, manage, and control extracorporeal membrane oxygenation (ECMO) technology and assist with associated procedures in acutely ill patients of all ages in critical care settings. The specialist will learn to troubleshoot devices and associated equipment under the supervision of experienced ECMO personnel, provide ongoing care through surveillance of clinical and physiologic parameters, adjust ECLS devices as needed, administer and document blood products and medications in accordance with hospital standards, provide airway and ventilator management, and perform the full scope of practice of a Respiratory Therapist II. Schedule: 36 hours per week, rotating day/night shifts, every third weekend. **This position is eligible for full time benefits $20,000 sign-on bonus (not eligible for internal candidates and not eligible for former BCH employees who worked here in the past 2 years) Key Responsibilities: Assemble, prepare, and maintain extracorporeal circuits and associated equipment with assistance. Assist in priming extracorporeal circuits and preparing systems for clinical application. Assist with cannulation procedures. Assist in establishing extracorporeal support; monitor patient response, provide routine assessments, circuit evaluations, patient monitoring, and anticoagulation management. Assist with ECMO circuit interventions, weaning procedures, and transports. Administer blood products per hospital standards. Interact and communicate with caregivers, nursing, surgical and medical teams, patients, and family members. Maintain relevant clinical documentation in the patient's electronic health record. Participate in professional development, simulation, and continuing education. Attend ECMO Team meetings and M&M conferences on a regular basis. Act as liaison and consult with community agencies and families to maintain coordination in the treatment process. Document client progress to maintain a permanent record of client activity according to established methods and procedures. Ability to provide substance use therapy utilizing evidenced-based therapy interventions which include psychoeducation and information related to treatment, harm reduction techniques and medication assisted treatment options. Provide treatment, rehabilitation, and support services; with some interventions directed or performed by staff with specialty training and skills (e.g., vocational specialists). Ability to provide family psycho educational services and assist with integrating family goals and services for all clients with other team members. Take the lead role and participate in providing Substance use treatment services, which includes IDDT group/individual therapy, relapse prevention planning, harm reduction, referrals/linkage to detox/inpatient/community referrals etc. Frequent or occasional driving of personal vehicle for purpose of transporting clients in the community and/or site visits (client or work related) SKILLS/COMPETENCIES: Demonstrate a clear understanding of the characteristics and problems of adults with severe and persistent mental illness and substance abuse disorders and be knowledgeable about the ACT model. Demonstrate thorough knowledge of effective clinical practices and principles. Knowledge in working with a population of people transitioning from jail/forensic setting. Demonstrate knowledge and experience in assessment, diagnosis, co-occurring disorders, and substance abuse or dependency. Excellent written and oral skills communicating in English Must have ACT training within six (6) months of hire Must be proficient in the use of computers and all Microsoft Office Products i.e. Word, Excel, Power Point, Outlook, etc. EDUCATION REQUIREMENTS: Master's or doctoral Degrees in Nursing, Social Work, Clinical Mental Health Counseling, Rehabilitation Counseling, or Psychology are eligible as are Registered Occupational therapists. Must be licensed to practice in the state in which the program is operating. Training from ACT Institute within 6 months of hire LMSW, LCSW, LMFT, LMHC, PHD, PSYD, RN, LPN EXPERIENCE: Minimum of three (3) years of post-secondary school education working in the field of behavioral health treatment; especially with an emphasis on the Co-occurring population and working with individuals in a forensic setting, including internships and other supervised practical experiences in a clinical or rehabilitation setting with persons with severe and persistent mental illness. 1 year of providing substance use disorder treatment in an outpatient, inpatient or community-based setting. Experience integrating dual-recovery treatment in collaboration with clinical team members. Must possess the ability to provide and coordinates assessment, treatment, planning, and services for substance use based on the needs of the individual. Must possess a valid Driver's License with a satisfactory driving record, and possess a personal vehicle for job requirement COMPENSATION: $52,000/yr - $66,000/yr
