Our treatment model is impairment based and focused on hands-on, evidence-based care that is delivered on a one-to-one basis. Therapists are trained in the latest advanced skills including, dry needling, manual therapy and manipulative skills, load progression, and evidence-based return to play. Our programming and equipment strive to provide excellent care for all patient populations and conditions. This position will involve a split between our Kings Mills location and floating across our other east central locations to assist with high volume, clinician leave of absence and PTO coverage. The patient population will vary based on the location, but will consist of primarily orthopaedic-based coverage. Compensation for this position will be above typical starting clinician ranges due to the expectation of flexibility.
**Job Summary:**
The Physical Therapist completes initial assessments, ongoing assessments and provides skilled therapeutic interventions to patients through the use of their educational knowledge, skill, and ability. This may involve outpatients, inpatients, pediatrics and off-site locations. Services the patients as a part of the entire continuum of professional care.
**Essential Functions:**
+ Evaluates patients, gathering pertinent information from various sources, and develops an appropriate treatment plan with specific goals. Responsible for ongoing assessment, efficiency of treatment plan and revisions of plan as needed.
+ Provides direct therapy to patients daily following patient's plan of care and current standards of practice for Physical Therapy . Provides instruction in therapeutic procedures to patients and family.
+ Maintains accurate and concise records on treatment provided and patient's progress. Documents all care provided, coordination of services, orders, goals, etc. in the patient's electronic medical record in an accurate and timely manner. Provides timely co-signature for supervised physical therapy assistants, if applicable
+ Maintains set level of productivity without jeopardizing quality. Delivers efficient and effective care to patients meeting and exceeding department standards.
+ Maintains departmental policies and procedures regarding safety, infection control, time off requests and timecard recording. This will include all necessary education and yearly compliance requirements.
+ Supervises the physical therapist assistant's treatment interventions and professional growth.

The SocialWorker assists patients and families to resolve all aspects which may prohibit or limit the patient from receiving maximum benefit from medical care. Evaluates social-emotional functioning of patient and families, providing counseling when necessary and facilitates the discharge plan. Works cooperatively with other hospital departments, physicians and outside agencies. Provides assistance to neonate, pediatric, adult and geriatric patients.
KNOWLEDGE
1. Work requires a Bachelor's degree in Social Work with licensure from the State of Ohio as a Licensed SocialWorker.
2. A relatively high level of analytical ability is required to assess patient needs, develop and manage discharge plan and to effectively utilize community and governmental resources.
3. A significant level of interpersonal skills is required to counsel patients and families concerning sensitive emotional and social issues.
4. Six to twelve months previous experience is required to acquire diagnostic interviewing and counseling skills.
WORKING CONDITIONS
1. Work is generally performed in an office or normal patient care environment where there is some exposures to communicable diseases.
2. On call responsibilities as assigned.
RESPONSIBILITIES
1. Conducts interviews with patients and families to obtain social work assessment and evaluation relevant to medical problems and discharge plan. Collaborates with entire health care team to ensure maximum benefits of medical care.
2. Completes social history and documents, when necessary on patients.
3. Evaluates data and refers patients to appropriate community resources, and if necessary, facilitates placement. Coordinates the exchange of medical and social information necessary to ensure continuity of care.
4. Provides counseling to patients, as needed, to assist them in decision making and acceptance of illness and related problem, e.g. Advance Directives, adjusting to disfiguring illness, permanent disabilities.
5. Maintains ongoing and develops new contacts with outside agencies in order to be used as a referral source.
6. Coordinates financial referrals and counsels patients and their families informing them of various assistance programs and community resources.
7. Participates as a team member in multi-disciplinary conferences.
8. Maintains JCAHO and Department standards. Prepares and submits monthly statistics as directed.
9. Provides service to all age groups. Demonstrates knowledge and skills necessary to provide care appropriate to the age of the patient served on assigned unit.
Demonstrates knowledge of the principles of growth and development over the life span by completing continuing education program.
Interprets information need to identify each patient's requirements relative to his/her age-specific need:
a. Neonate
b. Pediatric
c. Adolescent
d. Adult
e. Geriatric

Job Summary:
The Clinical Supervisor works in conjunction and collaborates with the Practice Manager to provide clinical and administrative managerial oversight and direction for clinical day-to-day operations of the practice. * Manages day-to-day supervision of the clinical delivery of services
* Plan, implement and administer programs and services, including personnel administration, training, and coordination of medical, nursing, and practice staff
* Appropriately trains new staff on clinical and administrative duties and trains current staff on procedures or equipment, as needed
* Provides input regarding associate performance evaluations, delivers performance feedback and coaching to clinical staff to assist with workflow improvement/efficiency
* Must possess the ability to troubleshoot and resolve problems promptly
* Provides oversight for scheduling and timekeeping
* Other duties as assigned
Education:
* High School Degree or GED
Licensure/Certification:
* Must possess one of the following:
* Active state Licensed Practical Nurse (LPN) licensure with medication and IV privileges
* Active Medical Assisting Certification
* Basic Life Support (BLS) - American Heart Association (preferred upon hire, required within 90 days of hire)
Experience:
* Two years of practical nursing or medical assisting experience within a healthcare setting, preferably within a physician practice (required)
* Electronic Medical Record (EMR) experience (required)
Skills & Abilities:
* Demonstrates leadership competencies and skills for the position, including excellent communication, customer service, continuous quality improvement, relationship development, and decision making
* Possesses extensive knowledge of clinical practice or function and a thorough understanding of the organization and work environment
