What you will do: Responsibilities listed include but not limited to:
Perform evaluations and develop treatment plans of patients with speech, language, cognitive and swallowing disorders
Regularly re-assess effectiveness of treatment plans, attend staffing meetings and family conferences
Perform discharge evaluations, make appropriate recommendations for home and community safety, and provide referral and education resources
Communicate patient's needs and progress to the treatment team, physician, person receiving services and family members
Communicates with other disciplines to ensure collaboration, coordination of care and enhance patient outcomes
What you will need:
Minimum of a Master's Degree from an accredited SpeechLanguage Pathology program
Valid SpeechLanguage Pathology license in the state(s) of practice
Current CPR Certification in accordance with state regulations
A minimum of one year's work experience as a SpeechLanguagePathologist
Demonstrates knowledge of rehabilitation techniques related to complex neurological injury preferred
Communicates effectively and professionally in verbal and written interactions
Ability to lift 50 pounds
Moving, lifting, or transferring of patients which may involve lifting of up to 100 pounds following safety procedures
Duties require fine motor skills,visual acuity, and walking/ standing for extended periods
Additional physical requirements include: pushing/pulling, bending/stooping, reaching, kneeling, and positioning frequently at times
A health screen or examination may be required prior to assignment and periodically thereafter, depending on specific location policy, local and state regulations to verify employee is physically capable of performing assigned duties with or without reasonable accommodations

Collaborates with members of the rehabilitation team to develop the most appropriate treatment plan consistent with the needs and level of functioning of each patient.
Develops innovative home programs individualized to meet the needs of the patient/family. Instructs the patient and family in these programs.
Initiates consideration of patient discharge on the day of admission to facilitate transition to the home or long term care environment.
Utilizes a comprehensive approach to patient evaluation and implementation of the therapy program; assesses physical, social and emotional needs of the patient.
Reports significant alterations in patient's condition to patient's physician.
Demonstrates appropriate professional judgment in presenting alternative or innovative solutions to problems encountered in the course of speech therapy, revises treatments as necessary.

ESSENTIAL JOB FUNCTIONS:
Provides treatment programs to maximize speech, language, cognitive, and swallowing abilities within each patient's structural or functional limitations; including, but not limited to: evaluates and treats patients, communicates with families, physicians and other interdisciplinary team members, documents services in the medical records. Provides patient treatments, which meet patient needs, and uses current treatment knowledge in accordance with facility clinical procedures. This includes but is not limited to:
Performing accurate, comprehensive evaluations.

Acquiring and documenting consents/approvals for treatment when necessary.
Consistently demonstrates facility's interdisciplinary approach to patient care. This includes but is not limited to:
Integrating findings of other members of the interdisciplinary team in treatment assessment and recommendations.
Integrating input from patient and family in treatment assessment and recommendations.
Preparing for and providing meaningful contributions to the rehab team conferences, patient care conferences, utilization review meetings, family conferences, and caregiver training sessions.
Collaborating with and informing facility staff in areas of patient progress.
