POSITION SUMMARY
A SpeechLanguagePathologist is a skilled, registered and licensed professional who evaluates and treats patients referred by a physician. Treatment approaches utilize applicable modalities, supervision of rehabilitation techs, and/or exercise to restore function.
RESPONSIBILITIES
Address client's aspects of performance to support engagement in occupations that affect health, well-being, and life quality.
Select and adapt the appropriate per case speech therapy treatment model, method, and approach to direct the process of interventions.
Guide and educate family members and caregivers.
Observe, note, and report on the progress of long- and short-term treatment goals.
Collect data and document processes followed.
Regularly follow-up with clients.
SKILLS
Proven working experience as a SpeechLanguagePathologist
Excellent knowledge of principles and practices of SpeechLanguage Pathology
Ability to manage patients with different types of personalities

EDUCATION AND TRAINING

Responsibilities
Speech-LanguagePathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.
The SLP evaluates and treats patients with language impairments, motor speech disorders, dysphagia, cognitive communication difficulties, voice disorders, and those requiring alternative or augmentative communication. Assessment and Documentation

Responsibilities
Complies with facility policies and procedures for appropriately documenting, billing and coding for residents receiving speechlanguage pathology and audiology services.
Provides resident information needed for the resident assessment instrument according to facility policy such as signs and symptoms of swallowing disorders, speech and hearing patterns, cognitive patterns, Brief Interview for Mental Status (BIMS), speechlanguage pathology and audiology minutes and distinct calendar days, etc. during the defined observation period(s).
Provides resident information needed for prior functioning in functional cognition as required by facility policy (RAI User's Manual) for Medicare Part A, Medicare Advantage residents upon admission.
Required Qualifications:
Master's Degree, or equivalent, in SpeechLanguage Pathology
A valid, unrestricted license in his/her therapy discipline in the state.
Knowledgeable of skilled nursing home regulations, procedures, laws, regulations and guidelines pertaining to long-term care.
