DESCRIPTION
Assess residents to determine care needs
Optimize residents' functionality and skills
Complete all necessary documentation via Net Health
Attend required meetings and in-services, including but not limited to family meetings, care plans, staffing, and utilization reviews (Not required for PRN staff)
In conjunction with the DOR, develop programs appropriate to the needs of facility residents across the continuum of care
Remain compliant with infection control and environmental protocols while following OBRA Resident Rights Guidelines
Responsible for appropriate scheduling of residents
Facilitate and complete family and facility staff in-services and education
Assist the facility in meeting standards necessary to maintain state and federal certification
Complete tasks deemed appropriate and assigned by the Director of Rehabilitation
Serve Veterans and Gold Star Families with pride and commitment
REQUIRED SKILLS

Developing Treatment Plans:

Set short-term and long-term goals for each patient to improve speech, language, and swallowing abilities.

Speech and Language Therapy:

Swallowing Therapy (Dysphagia Treatment):

Patient and Family Education:
