Provides speech-language pathology services in inpatient and outpatient settings during assigned shift during department operating hours, including assignment in rotation of weekend and holiday coverage.
On physician's referral, evaluates patients, develops specific treatment plans and goals, administers procedures and modalities specific to the learning needs and age of the patient, and plans for discharge utilizing appropriate Speech-Language Pathology techniques.
Acts as a case manager for assigned patients.
Prepares and maintains accurate and current patient records in accordance with departmental policies and procedures.
Re-evaluates and revises patient's treatment plan on a regular basis, with approval of physician as necessary.
Provides patient, family/caregivers with age appropriate instruction regarding therapy procedures and home program, and provides additional instruction as indicated.
Instructs the patient and family/caregiver in care and use of assistive and adaptive equipment.
Communicates with rehabilitation team members, family, physicians and third party payers regarding patient progress and program changes as appropriate.
Attends and/or participates inter/intradepartmental in-services and meetings as required and appropriate.
Participates in short and long-term program development, including participation on committees as requested.
Participates in student intern and/or volunteer program as appropriate.
Provides supervision to CFY as appropriate.
Maintains clean, safe and orderly treatment areas and equipment. Reports to supervisor any problems or suggestions regarding safety in the workplace.
Assists with orientation and supervision of other health care professionals as appropriate.
Establishes and maintains effective communication with physicians, co-workers, patients and other hospital personnel.

Identify patient, family, and caregiver needs, preferences, and goals.
Treatment Planning & Implementation
Develop individualized, measurable therapy plans aligned with patient goals, family input, and physician orders.
Provide direct skilled therapy (speech, language, swallowing, cognitive) to patients.
Adjust therapy plans based on patient response and progress.
Instruct and train caregivers on carryover strategies and home exercises.
Coordinate with an interdisciplinary team (nursing, OT, PT, CNAs) to align therapy with overall care plan using google workspace applications and EMR system, known as HomeHealthMD .
Documentation & Compliance
Complete evaluations, treatment notes, progress notes, re-evaluations, and discharge documentation per agency policy and regulatory timelines (48 hours).
Document all therapy contacts, missed sessions, cancellations, and outcomes accurately.
Record therapy elements that support billing, justification, and compliance.
Communicate changes in patient status or plan to physicians and care team.
Communication & Collaboration
Participate in care team meetings, case discussions, and interdisciplinary conferences.
Report therapeutic findings, recommendations, and concerns to physicians, nursing, and leadership.
Provide clear, respectful, and timely communication with patients, families, caregivers, and staff.
Additional Duties
May cover on-call therapy as needed according to agency policy if both parties agree.
Assist in scheduling, travel coordination, and therapy resource management.
Maintain continuing education and fulfill licensure / competency requirements.
