Develops and implements individual plans of care which promote development and/or restoration of skills required for successful participation in community activities and activities of daily living.
Evaluates treatment outcomes at various stages and modifies the plan of care to reflect any changes in the patient's performance or condition.
Designs home exercise programs to facilitate carry over and promote independence. Provides instruction and supervision in the implementation of home programs.
Complies with standards for medical; and/or educational; records management (e.g. insurance authorization; consents; physician's referral; documentation of treatment notes) and maintains patient/client confidentiality in accordance with HIPAA guidelines.
Complies with professional and licensure standards on code of ethics.
Maintains a safe and therapeutic environment for patient and family in accordance with hospital standards including assurance and support of patient's rights and responsibilities.
Collaborates with other healthcare; contract; and school providers to coordinate services and maximize outcomes.

I. Major Responsibilities:
1. Conducts diagnostic evaluations of various communication disorders in both children and adults. This may include, but is not limited to, language disorders, articulation deficits, auditory and auditory-perceptual deficits, voice disorders, stuttering and neurological disorders such as congential, developmental, or acquired. Assessment may include consideration of the physical status of the oral and vocal mechanisms, breathing patterns, vocabulary, language structure, articulation, voice quality, fluency, auditory processing, language integration, written language and such other parameters as may be deemed appropriate. Analyzes client's data and interprets test results to physicians.
2. Provides remedial treatment in both individual and group settings for children and adults with the above communication and swallowing disorders, utilizing clinical techniques appropriate to each type of disorder. Uses complex decision making to analyze each patient's problems and develop treatment interventions to maximize function in daily living activities.
3. Recommends, designs, and/or implements augmentative methods of communication such as communication boards and mechanical devices when oral and/or aural communication is not possible. This is done in conjunction with the patient, family and/or others.
4. Provides information about the communication disorders to patients, families and others.
5. Designs and establishes home programs and other activities which may be used for carryover or reinforcement of the therapy program.
6. Maintains records and reports necessary to the conduct of treatment. This may include, but is not limited to diagnostic evaluation reports, daily treatment notes, monthly physician renewals as appropriate, progress reports as appropriate or requested and losing summaries of treatment.
7. Confers with other individuals and/or agencies involved with the patient's program. This may include physicians and other related professions, school personnel, social service agencies and the like.
II. Position Qualifications:

1. Master's Degree in Speech Pathology.
2. Current MA Speech Pathology license.
3. Member of ASHA (American Speech Hearing Association).

1. Attainment of the certificate of clinical competence. This typically takes one year.
2. Effective verbal and written communication skill; demonstrated clinical knowledge.
3. Excellent customer service skills and ability to work cooperatively with all members of the healthcare team.

What you'll be doing at Autism Care Partners:
Ethically implement services based on knowledge and skills in speechlanguage pathology and related subject areas.
Demonstrate ability to conduct appropriate comprehensive evaluation and treatment for individuals who may be experiencing a variety of communication disorders.
Use appropriate and dynamic service delivery methods consistent with the wide variety of individual needs, including use of evidence-based treatments such as the Visual Immersion System.
Create individually tailored Plans of Care (POC) that reflect an understanding of each client's age and skill level.
Collaborate with families and caregivers to report progress and challenges as well as provide opportunities for them to be involved, promoting the carryover of services.
Collaborate and communicate effectively with all staff and team members, both within and external to ACP, to consistently serve the needs of the clients.
Complete necessary documentation and billing that reflect patient care accurately. This includes the conversion/verification of daily SOAP notes (within 24-hours of sessions), update Plans of Care (POC) at least every 6 months, and completion of annual evaluations or as needed by specific insurance requirements.
Document duties performed (billable and non-billable) within the approved number of hours worked on EMR system before pay period ends.
Maintain agreed upon number of treatment sessions per week as communicated by your supervisor.
Make every effort to attend scheduled team meetings including monthly SLP team meeting and scheduled check-in meetings with Regional Lead SpeechLanguagePathologist.
Perform other duties, responsibilities, and projects at any program or location as assigned by Regional Lead SpeechLanguagePathologist or other member of leadership at Autism Care Partners.
Promote a positive public image of Autism Care Partners and demonstrate an understanding of philosophy and values of the agency.

Position Description:
The SpeechLanguagePathologist (SLP) has a primary responsibility to accelerate client success and drive coordination of care with ABA services. What you'll be doing:
Provide full range speechlanguage pathology services as ordered by physician.
Gather background data, reviews referrals, selects and modifies evaluation tools, and evaluates patients determining the level of patient's physical and psychosocial functioning.
Reevaluate the patient's progress during treatment at appropriate intervals, recommending, acquiring, and modifying necessary adaptive equipment to augment the patient's independence.

About the Job
The role of the speechlanguagepathologist within Elliot Health System is focused on the delivery of rehabilitative services. The speechlanguagepathologist evaluates, plans and implements specific treatment programs for patients.
What You'll Do
Prepares for patient evaluation by conducting medical record review identifying areas of focus, contraindications and precautions. Ensures orders for therapy are active and appropriate.
Evaluates the skilled therapy needs of the referred patient and develops a written prioritized plan of care, including short- and long-term rehabilitation goals developed in collaboration with the patient and/or other members of the health care team.
Updates treatment plan re-evaluates patient and communicates with referrer and other clinicians involved in the patient's care.
Performs treatments within specific discipline effectively and efficiently, incorporating current best practices and evidence supported treatment interventions, to promote optimal functional outcome and attainment of patient centered goals.
Establish and implements discharge plan and instructs patient, family member(s), and other essential personnel in effective performance of home exercise programming or other strategies to maximize function. These may include but aren't limited to, instruction in the use of home medical equipment, adaptive devices or customized equipment (splints, orthoses, ACC devices, etc).
Provides clinical supervision of therapy assistant(s), rehabilitation assistants, and/or other essential personnel in the implementation and revision of the rehabilitation plan of care.
What You'll Need
Education: Graduate of a SpeechLanguagePathologist Program with corresponding Entry Level Degree required.
Experience: Previous experience in clinical setting preferred, but not required.
Certification/Licensure: Licensed SpeechLanguagePathologist by the State of New Hampshire governing boards required within one year of hire; if provisional license, must complete provisional requirements within one year of hire. Ability to complete BLS certification within first 90-days of employment required; BLS certification preferred at time of hire.
