Responsibilities: Provide speech therapy services as prescribed by a physician, with the goal of improving or maximizing the patient's communication skills. Assist patients in returning to their optimum and productive level of living within their capabilities. Participate periodically in patient care planning and collaborate with the interdisciplinary care team. Perform all skilled speech therapy procedures in accordance with the treatment plan. Supervise and direct personnel, including home health aides, as assigned. Conduct initial evaluations, take patient history, and develop treatment plans. Consult with physicians to modify treatment plans as necessary. Write detailed reports for physicians on patient progress and treatment outcomes. Instruct patients and their families on home programs and exercises to continue progress outside of therapy sessions. Provide in-service training to Agency staff on speech therapy techniques.
