HOW YOU'LL WORK:
You'll provide home-based skilled speech therapy care for patients as directed by the attending physician. You will educate patients and their family members and ensures the safety of the patient.
MAJOR AREAS OF RESPONSIBILITY:
Plan of Care:
Develop an individualized ST plan of care and makes adjustments as needs change. Guide the patient in his/her use of therapeutic, creative and self-care activities for the purpose of improving functioning. Observe, record, and report patient reactions, changes, and progress to attending physician and any appropriate agency staff.
Patient Outcomes:
Use activities and home exercise programs to improve swallowing, breathing, cognition, speech and communication, and/or any other speech therapy related function.
Minimize Patient Risk:
Help decrease re-hospitalizations by teaching the patient on preventative measures and making good decision.
Family Educator/Advocate:
Counsel patient and family on the disease/injury and how to manage in all phases of speech therapy. Counsel and instruct patient and family on the disease/injury/disability and how to manage in all phases of speech therapy.
Policies:
Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
Rules and Regulations:
Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
Operations: Attend in-service training and mandatory agency meetings.
HARD & SOFT SKILLS:
Compassionate communicator with a positive attitude.
Patience is a virtue when working with patients, families, physicians, and coworkers.
Attention to detail is critical, as is being observant and following directions.
REQUIREMENTS:
Speech Therapist with a current license in the state of employment.
