Conduct supervisory visits to oversee care provided by home health aides, ensuring quality and compliance. Assist physicians in evaluating the patient's level of function and recovery progress. Contribute to the development and revision of care plans as needed. Maintain accurate clinical documentation, including progress notes, and ensure timely submission of all required reports. Advise and consult with families and other Agency personnel on communication issues and therapy progress. Participate in Agency inservice programs and provide input on training needs. Complete and submit OASIS assessments, transfers, resumptions of care, discharges, and reports of significant changes in the patient's condition in accordance with Agency timelines. Appropriately utilize ICD-10 codes for accurate diagnosis and billing. Engage in Quality Assessment and Performance Improvement (QAPI) activities as assigned.
