Key Responsibilities
Perform Modified Barium Swallow (MBS) and FEES bedside swallow studies
Provide trach care and Passy Muir Valve (PMV) management
Conduct speech, language, and swallowing evaluations and treatments
Document all care in the Meditech EMR system in accordance with HIPAA
Collaborate with physicians, nurses, and interdisciplinary rehab teams
Educate patients and families on therapy goals and techniques
Maintain current knowledge of clinical best practices and hospital policies

Required:
Independent proficiency in Modified Barium Swallow (MBS) studies
Independent with Trach care and PMV management
Current Nevada SLP License

We are seeking a compassionate Speech Language Pathologist (SLP) to provide high-quality speech therapy to patients in the comfort of their homes. The SLP will work under the supervision of the Director of Clinical Services/Clinical Manager to administer physician-ordered therapy services, aiming to improve communication and help patients return to an optimal level of functioning. As a critical member of the home healthcare team, the SLP will also collaborate with other professionals in patient care planning and progress evaluation.
Responsibilities:
Provide speech therapy services as prescribed by a physician, with the goal of improving or maximizing the patient's communication skills.
Assist patients in returning to their optimum and productive level of living within their capabilities.
Participate periodically in patient care planning and collaborate with the interdisciplinary care team.
Perform all skilled speech therapy procedures in accordance with the treatment plan.
Supervise and direct personnel, including home health aides, as assigned.
Conduct initial evaluations, take patient history, and develop treatment plans.
Consult with physicians to modify treatment plans as necessary.
Write detailed reports for physicians on patient progress and treatment outcomes.

Write progress notes for each patient visit, and complete a reevaluation when the expected duration of treatment is reached.
Prepare a comprehensive summary upon patient discharge.
Conduct supervisory visits to oversee care provided by home health aides, ensuring quality and compliance.
Assist physicians in evaluating the patient's level of function and recovery progress.
Contribute to the development and revision of care plans as needed.
Maintain accurate clinical documentation, including progress notes, and ensure timely submission of all required reports.
Advise and consult with families and other Agency personnel on communication issues and therapy progress.
Participate in Agency inservice programs and provide input on training needs.
Complete and submit OASIS assessments, transfers, resumptions of care, discharges, and reports of significant changes in the patient's condition in accordance with Agency timelines.
Appropriately utilize ICD-10 codes for accurate diagnosis and billing.
Engage in Quality Assessment and Performance Improvement (QAPI) activities as assigned.

Supervising Clinical Fellow and other SLPs in direct patient care and patient related activities.
Writing specific, objective documentation, which is outcome oriented and details the patient's needs, potential to benefit from treatment, and progress achieved to date. Includes specific, objective measurable goals.
Managing Medicare Medical Review and Denials process in conjunction with Director of Rehabilitation and therapy staff
Qualifications
Availability to work full-time as a Licensed Speech Language Pathologist (SLP) in Nevada
Bachelors or Master's Degree in Speech Pathology from an accredited Speech Therapy program.
Knowledge in Speech-Language Pathology evaluation and treatment as they relate to medically based speech, language, and swallowing disorders.
Ability to supervise and direct assistants and aides.
Knowledge of medical terminology.
Ability to work with all types and levels of staff and patients.
Experience in a skilled nursing facility
CPR certified

Job Summary: Under the supervision of a Clinical Fellow mentor, using a client centered model, within the confines of the clinic, as well as using a parent educational model in the Early Intervention program, a Clinical Fellowship Speech Language Pathologist will help the client develop or regain physical or mental functioning by implementing programs to improve speech delays, conditions and feeding issues. The Clinical Fellow Speech Language Pathologist performs other administrative duties as may be required in support of the program goals and objectives. Essential Functions and Responsibilities:
Develops ideas, plans, and procedures for initiation, expansion, improvement, and evaluation of therapy programs.
Completes initial evaluations and re-evaluations for patients in the program; examines the condition of patients and analyzes their needs; determines the need for an occupational therapy evaluation and selects and administers appropriate assessment tools to evaluate the patient.
Prepares reports and attends multidisciplinary case meeting to decide and review the continuing treatment.
Maintains clinical and administrative records in accordance with professional standards, state guidelines and agency system policy; adheres to federal and state regulation, and policies that affect occupational therapy practice.
Participates in continuing education for professional development to ensure practice consistent with best practice and to meet Nevada Licensure requirements.
Participates in bi monthly staff meetings. Meet for weekly supervision with CF mentor.
Attends staff trainings.
Case assignment will be dictated at the discretion of the organization's needs in the Early Intervention and outpatient clinic programs.
Manages patients with different types of personalities
Interprets evaluation data and writes comprehensive reports that reflect strengths and barriers to client participation; directs program development; and guides evidence-based intervention.
Uses professional Code of Ethics and standards of practice to guide ethical decision making in practice.

Excellent verbal, written and interpersonal communication skills.
Ability to work in an environment with individuals with severe disabilities who may be developmentally and behaviorally challenged.

Qualifications (education, training, experience, licenses):
Initial certification by National Board for Speech Language Pathology required.
Current Speech Therapy License from the State of Nevada required.
Minimum of two (2) years of experience working with children using knowledge of treatment practices per discipline required.
CPR and First Aid certifications required.
Valid driver's license required.
Knowledge of the federal, state local legislation, regulations, policies and procedures that mandate and guide speech therapy in practice required.

Responsibilities
Speech-Language Pathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician.
