Plans and provides rehabilitative services for speech/language and swallowing disorders; performs interim evaluations, if necessary, for recertification. Records type of treatment and client reaction; evaluates effectiveness and outcomes of care; revises plan as appropriate and informs the physician and other Agency staff of significant changes in client needs. Selects and administers diagnostic and therapeutic techniques and materials. Identifies barriers to achievement of desired outcomes. Teaches and counsels other Agency staff and caregivers to improve, correct, and accept client disabilities. Participates in clinical meetings as directed; consults and coordinates services with other Agency staff involved with client care. Utilizes electronic documentation of client care and complies with Agency policy regarding timeliness in submission of documentation.
