Role Overview:
As an SLP, you will develop and implement individualized education/treatment programs for students whose ability to communicate has been impacted by developmental differences, disabilities, or injuries. This role offers the opportunity to work with young learners in a supportive school environment while making a lasting impact on their development.
Key Responsibilities:
Deliver therapy and interventions to students in accordance with their Individualized Education Programs (IEPs) to address communication disorders.
Work closely with teachers, administrators, and other specialists to develop and implement strategies that support students' speech, language, and communication development.
Offer expertise in speech and language assessments, goal setting, and progress monitoring to ensure students receive appropriate accommodations and support.
Document all services, assessments, and interventions provided, ensuring compliance with district and state requirements.
Qualifications:
Master s degree or higher in SpeechLanguage Pathology.
Illinois PEL with Speech-Language endorsement (or Type 73, Type 10, or Type 03/09)

Promotes Elara Caring's philosophy, mission statement and administrative policies to ensure quality of care.
Evaluates the Speech-Languagepathologist patient to determine the rehabilitation needs and potential for achievement and develops a plan of care in accordance with the findings.
Admits the patient and assesses speech-language therapy and all other needs according to OASIS and PPS regulatory guidelines. Initiates plan for patient safety, using the patient, family, and community resources.
Communicates significant findings, problems, and changes in condition or environment to the appropriate supervisor, the physician, and other team members involved with patient care.
Participates in the implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning. Verifies the Plan of Care prior to each visit and provides care according to physician's orders, assessment data, and established standards and guidelines.
Writes physician orders to cover additional visits and changes to the Plan of Care, per agency policy.
Incorporates patient care goals established in the Plan of Care, into therapy care, as evidenced by documentation in therapy notes. Initiates and revises the Plan of Care in response to identified patient care issues.
Performs direct Speech-Language pathology services in accordance with accepted standards of practice and certified by the patient's physician.
Selects diagnostic and therapeutic materials appropriate to patient needs. Instructs and provides patients and/or their caregiver in the home therapy program providing modification and clarification when necessary.
Assures clinical notes indicate continuing communication and coordination of services with physician, other staff, and disciplines.
Ensures that all therapy notes, supply log/visit logs are accurate and turned into the office according to office policy.
Physically demanding, high stress environment

What is Required?
Master's degree from an accredited educational program in Language Pathology
Current, unrestricted SLP license in the state of work

Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws.

The low end of the compensation range is $81,000 to the high end up to $111,000 annually, which can include additional earnings for work beyond quota, bonuses, on-call, or other performance-based compensation variables.

Speech-LanguagePathologists (SLP) evaluate and provide skilled speech-language therapy to individuals in a home setting, under the care of an attending physician. At Enhabit Home Health & Hospice, the home setting may include single or multi-family homes, assisted living, independent living, or memory care.

* Other duties. This encompasses any other duties as may from time to time be delegated by the supervisor and the supervisor may make any adjustment in the scope of responsibilities as outlined above which will be in the best interest of the school district.
* All administrative actions must be in concert with the mission statement, beliefs, objectives and parameters found in the strategic plan of the Rockford Board of Education.
EDUCATION/TRAINING:
A master's degree in Communication Disorders from an American Speech, Language and Hearing Association accredited program or the equivalent.
KNOWLEDGE, SKILLS AND ABILITIES:
A speech-languagepathologist is expected to have a broad base of knowledge and skills in a number of areas, but not limited to: normal and disordered speech and language development, the diagnostic process, therapeutic interventions and techniques, school achievement, various disabilities of childhood and adolescence and child development.

* The Building Principal must be notified of a compensatory plan if services cannot be provided prior to the services not being provided.
* Document all minutes electronically on a weekly basis and submit them to Building Principal.
* Report any discrepancy or concern to the Principal.
Physical and Special Demands:
* Demonstrates preventative measures relating to Infection Control concepts.
* Required to frequently sit, stand, bend, stoop, crouch, kneel, balance, push, pull and twist as well as walk and drive for extended periods. Ability to walk up and down stairs.
* Ability to operate machinery and equipment on premises.
* Works continuously in an environment where the noise level is frequently high and where:
* Chemicals are used for cleaning
* Mechanical and electrical hazards may be present occasionally
* Dust, mist and steam are generated in housekeeping tasks frequently
* Able to lift, carry, and lower 50 pounds without assistance. Ability to respond in a crisis to angry and sometimes disorganized, psychotic and/or verbally and physically abusive students while maintaining focus on student and staff safety. * Must possess the ability to provide physical restrain students with proper CPI methods after training.
* Must have the visual, hearing, and learning capabilities sufficient to perform the essential functions defined above.
Environment and Scheduling:
* Work is performed during school and program days and hours.
* Work is performed in and in association with Parkland Preparatory Academy.
* Duties involve contact with students who may be threatening, aggressive, suicidal, mentally ill and/or under the influence of drugs or alcohol.
