Summary of Position The Connected Care Coach is responsible for proactively contacting identified members to drive behavior changes and positively impact clinical outcomes. The schedule for this role will be 1:45pm-10:15pm CST. The work days will be: Sunday, Monday, Wednesday, Thursday, and Friday. The off days will be Tuesday and Saturday. Essential Duties and Responsibilities Conduct member outreach to drive behavior change and positively impact clinical outcomes (95%) Actively engage with members via a variety of communication tools including telephone, email, text, virtual application, and coaching conversations to provide feedback and support, answer questions, share resources, and hold members accountable to their goals. Utilize a coaching approach congruent with motivational Interviewing in order to empower members to be able to effectively self-manage their own chronic condition(s) and health. Refer members for support and education for conditions as needed based on clinical care guidelines. Maintain accurate and thorough case documentation to the standards set by Teladoc Health.
