Masonicare Home Health & Hospice, an affiliate of Masonicare, is seeking a dedicated and accomplished Registered Nurse Case Manager local to East Windsor, CT
The RN Visiting Nurse Case Manager ensures quality and safe delivery of nursing services to patients in the home setting.
Visits are done primarily during the DAY SHIFT, but exact scheduling is flexible. Weekend Rotation will be required. Typically, this is one weekend per month or less.
Masonicare's Registered Nurses work to develop, implement and evaluate the plan of care and make necessary revisions to address all problems in the plan of care. They provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.
Our Visiting Nurses supervise the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conduct supervisory visits per regulatory guidelines.
Masonicare is fully electronic! Our nurses demonstrate timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports.

We are an 7 doctor, busy, full-service, small animal practice serving the community for over 22 years. The entire staff at Northampton Veterinary Clinic is dedicated to providing the highest level of veterinary medicine to our patients alongside friendly & compassionate service to our clients. We offer a variety of services including digital full body and dental radiography, diagnostic ultrasound, full service in-house laboratory, laser therapy, soft tissue surgery, dental prophylaxis, wellness & preventative care, and much more!

About You:
You are an experienced veterinary technician looking to join a team that prioritizes communication, collaboration, and is ready to hit the ground running!
Take the initiative - you are a hard worker who contributes to the operation of the entire hospital, you will help get all the day's duties done!
You are versatile - you know how to effectively adapt to the individual needs of each patient and are dedicated to making a difference in the lives of patients and their families.
You possess clinical skills and experience that include (but are not limited to):
o Patient triage and assessment
o Phlebotomy and catheter placement
o Knowledge of radiology safety and proper technique
o Anesthesia monitoring proficiency
o Perioperative surgical nursing skills
o Routine dental prophylaxis experience
o Knowledge of basic laboratory procedures
Anticipated schedule:
Our clinic is open Monday thru Friday 8:00am - 5:00pm and Saturday 8:00am - 12:00pm
4-day work week! Four 10-hour shifts.
Saturdays required, every other rotation, half day off during one of your regularly scheduled days when you scheduled for a Saturday shift.
No on call
No Sundays
Compensation:
* We believe that each team member should be compensated considering their knowledge, skills, and experiences. We expect the range for an experienced technician to be $20-25 per hour.

Job Responsibilities
Educator responsibilities in SEZP include:
Enacting anti-racist instructional practices that ensure students own the learning
Providing grade level rigor in the course content using highly-rated, research-based curriculum
Providing embedded interventions for students needing additional support, time, or scaffolds
Working closely with a co-taught team of two to three additional educators in the classroom
Engaging with families as partners in the learning around successes, challenges, and problem-solving on behalf of our scholars
Other duties as determined by the school Principal
Position Qualifications
Experience teaching in middle school or high school
preferred
Appropriate content/program area and grade level educator license (School Counselor) from the Massachusetts Department of Elementary and Secondary Education
required
(Educators who are not yet licensed by MA DESE in the appropriate content/program area and grade level may be eligible for a licensure waiver and are still encouraged to apply.)
Experience as an active anti-racist, LGBTQ+ advocate, or organizer for equity

Up to $90,424 base salary
