Personal Trainer / Group Fitness Coach
Location: F45 Training, Lorton, VA
Employment Type: Part-Time
Pay: $23- $35 per hour

Responsibilities
Provide an exceptional F45 experience by welcoming members, motivating them, and ensuring proper form during exercises.
Lead classes, including introductions, warmups, and demonstrations of exercises, offering modifications for all fitness levels.
Collaborate with the Studio Manager and team to achieve daily, weekly, and monthly goals.
Conduct fitness evaluations using the Fit 3D Body Scanner and assist members in tracking their progress.
Maintain a clean, organized, and fun studio environment.
Deliver studio tours and assist with in-studio sales processes when needed.
Requirements
Certifications: Current CPR/AED Certification and a Personal Training certification accredited by an NCCA organization (required).
Experience: Personal or group training experience preferred.
Ability to prioritize safety and provide corrective coaching in a group setting.
Passion for health, fitness, and helping others reach their goals.

Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. Works with the health care team to promote positive adjustment, rehabilitation and improved quality of life for our patients. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. Supports the FMCNA commitment to the Quality Indicators and Outcomes and Quality Assessment and Improvement (QAI) Activities, including those related to patient satisfaction and quality of life and actively participates in process improvement activities that enhance the likelihood that patients will achieve the FMCNA Quality Goals.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment / Care Planning / Counseling
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.

Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.

Patient Education
Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.

Staff Related
Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.

Provides training to staff pertaining to psychosocial topics as needed.
Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.

QUALITY:
Responsible for supporting and driving organization quality standards through meeting regulatory and accrediting body requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools.
Assists with implementation of organization quality goals and facility specific action plans in order to achieve organization quality standards.

PATIENT:
Obtains necessary medical history, discusses procedure and answers questions, and performs nursing assessment on the day of the procedure.
Provides education to patients and patient's family regarding post procedure care of vascular access.
Monitors patient condition before, during and after the procedure, especially patients having moderate sedation and contrast media injections for possible complications or reactions.
Acts as a resource for the patient to address patient concerns and questions.
Identifies and communicates patient related issues to facility management, as needed.
Completes documentation of patient information from admission to discharge in an accurate manner.
Confirm written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions; delivery to appropriate labs; reporting and forwarding of lab results to appropriate physician.
Maintains competency with all emergency operational procedures, and initiates CPR and emergency measures, as needed.
Administers medications and IV moderate sedation to patients as directed per physician's orders, and in compliance with organization, company, federal, state and local regulations.
Acts as scrub technologist to assist the physician during procedure as needed, maintaining a sterile technique.
Assists with patient workflow by transporting patients as needed.
Performs point - of - care laboratory testing as directed per physician's orders and in compliance with organization, company, federal, state and local regulations.

PHYSICIANS:

Inspects operation of all facility equipment and reports malfunctions, and maintenance needs to ensure the safe and effective operation of all vascular access equipment.
Inspects operation of all facility operating systems, including temperature checks in appropriate areas and physical structure of the facility and reports any issues to the Facility Manager.
Ensures a clean, safe and sanitary environment in the vascular access facility at all times.
Ensures all blood spills are immediately addressed according to the organization/FMCNA Bloodborne Pathogen Control Policies.
ADMINISTRATIVE:
Assists with efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the organization formularies.
May assist in the development and revision of any applicable policies and procedures related to the facility to ensure continuous process improvement.
Assists with Continuous Quality Improvement (CQI) activities including monthly reports and attending meetings as required.
Assists with maintaining medication supply, including inventory control, checking for expired medications, and ensuring appropriate recording of controlled substances as required by law.

PHYSICAL DEMANDS AND WORKING CONDITIONS:

General Definition of Work
Mental Health Technicians perform safe and effective human support work maintaining the safety of clients, monitoring clients with special needs, coordinating meal service and delivery , facilitating educational groups, teaching coping skills, performing and assisting clients with laundry duties, encouraging clients to adhere to treatment goals, and related work as apparent or assigned. Work is performed under the supervision of the CSU Team Leader.

Primary Duties and Responsibilities
Delivers expert patient care in helicopters and ambulances to multiple patient types including scene work, adult subspecialties, pediatrics, and high-risk obstetrics. Assesses patient care needs, establishes priorities and delegates tasks to peers and ancillary support staff as appropriate.
Assesses the needs of patients with highly complex problems. Identifies current and potential complex problems, critically analyzes trends and changes in patient status, and plans appropriate interventions directly or indirectly through other nurses and paramedics
Performs basic and advanced airway techniques and treatment modalities, including oxygen therapy, airway maintenance, assisted ventilation, insertion of supraglottic airways, nasal and orotracheal intubation, rapid sequence pharmacological induction to facilitate intubation, surgical cricothyroidotomy, and needle thoracostomy.
Administers and maintains pharmacologic agents and intravenous fluids at the scene and during inter-hospital transfer.
In collaboration with the Critical Care Transport Specialist - RN, monitors and evaluates invasive and non-invasive techniques/procedures and their therapeutic effects, including the monitoring and operation of the intra-aortic balloon pump (IABP), Ventricular Assist Devices (VAD), and intracranial monitoring devices during transport. Troubleshoots these devices, recognizes problems, and makes necessary corrections. Recognition and intervention of abnormal parameters includes potentially lethal cardiac dysrhythmias requiring electric cardioversion and defibrillation.
Performs a variety of clinical duties including, but not limited to: insertion of nasal and oral gastric tubes and management of autologous blood transfusion systems. Initiates and maintains venous access and intraosseous lines.
Obtains patient information, including detailed reports, when appropriate, and disseminates all necessary information to appropriate medical staff, per protocol. Responsible for monitoring and recording all patient data and completion of all appropriate clinical documentation in a timely manner.
Maintains program readiness and ensure cleanliness and appropriate par levels of supplies used by MedSTAR Transport. Ensures helicopter or ambulance readiness with daily review of equipment and supplies and records findings on daily vehicle checklists.
Ensures system readiness post-transport. Ensures that par stock levels of equipment at remote bases are maintained and informs MedSTAR leadership of supply needs, as necessary.
Establishes and maintains professional radio communications with MedSTAR Communications Center and all other agencies, adhering to all FCC protocols.

Medication Management:
Provides strategic leadership for resident care in the community by managing, directing, and monitoring the medication care managers, as applicable by State/Provincial law, to promote the health and wellness of the resident population.
Provides oversight of the community medication management program to promote resident safety in the medication use process including onboarding, training, and performance reviews.
Provides clinical training and education, as needed, to nurses, medication care managers, care managers, and others who provide resident care. Performs skill evaluation to assure clinical capability of care team members.
Quality Assurance and Regulatory Compliance:
Tracks, trends, and reports clinical quality data to identify risk.
Participates actively as a member of the community Quality Assurance and Performance Improvement committee.
Leads clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes.
Recruits, hires, and trains clinical team members and is responsible for performance management, evaluations, and engagement.
Completes direct report team member staffing and scheduling according to operational and budgetary guidelines.
Partners with the community leadership team to promote resident safety and compliance with Risk Management and OSHA/British Columbia Workers Compensation Act and Occupational Health and Safety Regulation requirements.
Serves as the ICC and CLIA Director as applicable for the community.
Responsible for infection control programming, including delegation of infection control preventionist, as per state/provincial requirements.
Assures compliance with all Federal, State/Provincial, board of nursing, and other applicable regulations.
Financial Management:
Manages the department budget to include labor/labour and other expenses and understands it's impact on the community's bottom line.
Processes and submits monthly expenses and budget data in a timely manner, per Sunrise policies and internal business controls.
Understands the internal costs associated with all Sunrise resident care programs.
Training, Leadership and Team Member Development:
Partners with the Regional Director of Resident Care and/or Executive Director in the delivery and participation in Sunrise University training and self-study programs.
On-boards new RCD leaders and other department coordinators as needed.
Develops a working knowledge of state/provincial regulations and ensures compliance through supervising and coaching team members.
Completes clinical team member staffing and scheduling according to operational and budgetary guidelines when assigned to a community.
Holds clinical team accountable, corrects actions when necessary, and documents.
Attends regular meetings; Stand Up, Cross Over, Department Head, Town Hall, QAPI, and others as directed by the Executive Director.
Keeps abreast of professional developments in the field by reading and attending conferences and training sessions.
Maintains compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
Qualifications:

Graduate of an accredited college or school of nursing with a current state/provincial license as a professional Registered Nurse (RN).

Certified in CPR and First Aid.

Identifies on-going needs and services of residents in promotion of the highest quality resident services to be delivered.
Ensures proper follow through, participates with evaluation and documentation for residents with a change in health care services and status.
