The coordinator also implements clinical data activities, and performance improvement and population health activities across Ambulatory Care, Emergency Department and Inpatient settings. They collaborate with stakeholders to gather data, run analytic reports, and interact directly with patients to improve their health. This includes meeting patients to discuss specific program goals, reviewing patient charts to identify gaps in care, and completing patient text message and telephone campaigns. Produce and distribute chase list reports to clinical and patient-facing teams to guide patient care and improve care quality and performance. Support quality improvement projects in Population Health and Ambulatory Care services, and take responsibility for agreed actions. Oral Surgery Opportunity Available The SBH Health Systems Dental Department, Oral Surgery Section is seeking candidates for a part-time, 3-day a week, position as an oral surgery attending. The oral surgery section operates at a clinic site in Bronx, NY and provides care to a large pool of underserved patients with diverse needs and backgrounds. Our department provides general practice, dental anesthesia, pediatric, orthodontic and oral surgery services. The scope of oral surgery care involves both interceptive and comprehensive care, child and adult treatment, using a large variety of techniques. As an educational site, we accept residents for each service we provide including 3 fellow oral surgery residents every year, quarterly, from Brooklyn Hospital. Medical Laboratory Technologist to work in Microbiology under supervision of the Microbiology Supervisor. Will be responsible for performing a variety of complex and routine test within the Laboratory complying with all safety and regulatory requirement. Required to work weekends and scheduled holidays based on the department needs. Participate in interdepartmental collaboration within the institution, to provide the highest quality patient care.

This analyst in this position is responsible with interacting with end users across the organization to develop, implement, maintain, analyze and or update requested reports. Reports can be clinical or non-clinical in nature providing insight into clinical workflows, operations, quality, finance or other needs as required. Reports can be disseminated either as static (one-time, Ad hoc) or dynamic (recurring, Data Visualizations) via multiple pathways (Epic, Excel, Tableau, PowerBI, etc.). The report writer will be responsible for developing specifications for, and writing individual reports to satisfy facility and enterprise reporting needs. Ad hoc reporting tasks include one-time SQL queries, troubleshooting, restarting and creating report subscriptions, investigating the logic used in existing reports to determine what type of data populates the report, and researching reports within our EMR Vendor, Epic, for possible implementation and or updates.

Clinical Social Worker in the Collaborative Care is responsible to provide brief interventions using evidenced based techniques such as Behavioral Activation, Problem Solving treatment, Motivational Interviewing or other appropriate treatments in primary care settings. Work collaboratively with PCP and psychiatric consultant. Supports psychotropic medication management prescribed by PCP's focusing on treatment adherence, side effects and other complications and effectiveness of treatment.
