Position Overview:
The agency's Mid-Hudson Permanency Resource Center (“PRC” or “Center”) is designed to support and deliver specialized services to post-adoption, post-guardianship, and kinship caregiving families. Working in a team environment, the PRC Case Manager participates in community outreach activities, completes monthly face-to-face contact with the families in their homes, and completes required case documentation, including various assessment forms and short, targeted service plans. The Case Manager provides 1:1 counseling sessions as needed, and participates in the design and delivery of educational events, family engagement activities, respite care, and other program-related events.
Position Specifications & Requirements:
Bachelor's Degree in social work, psychology, or human services, required.
Associate's with 2+ years of case work or human services field experience or other related professional experience, may be considered.
Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges, mental health diagnoses, and/or trauma history, a plus.
Experience working in community services as well with at-risk families, also preferred.
Knowledge of and ability to utilize community resources; Capable of building strong working relationships with regulatory bodies.
Experience with NYS OCFS CONNECTIONS or other electronic record system, preferred.
Familiarity with kinship caregiving and wraparound services in a family-centered system of care, a plus.
Clean & Valid New York State Driver's License (actively licensed at least one year) with a reliable vehicle for transporting clients as needed, required*.
Proficiency in various computer applications: Microsoft Office Suite, Google Business Workspace Suite; and use secure Internet practices.

Essential Duties & Responsibilities include:
Manages a caseload of families, as assigned, working to ensure the program meets all performance targets.
Completes face-to-face contacts with the families in their homes, completing all required case documentation, including various assessment forms and service plans.
Provides 1:1 counseling sessions, as needed; incorporates trauma-informed support and motivational interviewing into the program milieu.
In a team environment, supports the program's design and delivery of parent education sessions, educational events, pro-social respite activities, and family engagement.
As a part of community outreach, assists in the delivery of presentations to community groups and local organizations in various targeted communities.
In a team environment, supports the delivery of parenting education sessions, utilizing the Parenting a Second Time Around curriculum, or other training materials, as directed.
Safely transports community members and/or their children in the delivery of program services, to address basic transportation needs, or to meet the families' other social needs linked to service goal attainment.
Actively communicates program information, concerns, needs, and progress to program leadership.
Completes additional position-related duties and responsibilities, as assigned by the agency's foster care director or Assistant Program Coordinator.

Position Overview
The Residential Case Manager (RCM) acts as the primary case manager and care coordinator for children placed in residential care. This case manager coordinates the facets of services for the child while in care -- medical services with the agency nursing department, mental health care with a therapist and psychiatrist, and educational matters with the children's school. Additionally, the RCM monitors and supports the milieu treatment and actively engages with and communicates with the house staff linked to the child's care, treatment, and safety.
Position Requirements
Ability to work: Monday - Friday, 8am-4pm OR 9am - 5pm w/ flexibility as needed.
A Bachelor's Degree in a human services, with a demonstrated capacity to work with people from diverse backgrounds, required.
Experience working with clients who exhibit significant emotional, behavioral, and interpersonal challenges; mental health diagnoses and trauma history, preferred.
Ability to provide safe transportation to a client in the worker's personal vehicle, in the event that an Agency vehicle is not available for needed transportation.
Flexibility to work non-traditional hours to accommodate client schedules, which may include evening and/or weekend hours, as necessary.

Essential Duties & Responsibilities include
Act as liaison between child, family, school, the treatment team, and the referring agency; and the primary point of contact for the local department of social services caseworker and the child's family.
Arrange and supervise all child/family visitations; secure signed releases and consents from the family or agency of legal custody, including permission for the special trips and events which are beyond routine residence/school activities.
Transport the child to visitation, court hearings, and other required meetings not occurring at the St. Catherine's Center for Children program site, when necessary.
Maintain regular contact and coordinating treatment with the Day Treatment School social worker when a child is enrolled in the SCCC day treatment school. When a client is enrolled in a school other than SCCC Day treatment, the RCM will maintain sufficient contact with that school to ensure the child is optimally benefiting from the educational placement.
Participate in Committee on Special Education (CSE) meetings and maintain contact with the SCCC and CSE chairperson.
Liaise with child care staff and residence clinical staff through participation on the treatment team and during treatment reviews, service plan reviews, medication reviews, and staff meetings.
Document treatment and casework as required by SCCC and regulating agencies including:
Completion of thorough and up-to-date progress notes in the electronic record, New York State (NYS) Connections.
Development of the service plan in Connections, given the assigned role and duties.
Court reports, correspondence with other agencies, social summaries.
Maintenance of accurate, up-to-date data on the child and family.
Completion of quarterly treatment reviews.
Updating of permission release forms requiring parent signatures.
Collection and/or preparation of all materials necessary to bring records into compliance with internal agency and external regulations.
In conjunction with the House Management Team, develop and maintain a therapeutic environment at the site.
Implement the principles of motivational interviewing as an evidence-based practice as part of the therapeutic milieu at the program site and supporting activities for the children and families.
Attend all court hearings pertaining to the placement, permanency, and treatment of the child and family.
Directly observe all visits that require supervision as deemed by the county or court.
As the primary liaison for the child/family, maintain flexible work hours, at the supervisor's direction, to include evening hours outside of the traditional work day to accommodate family schedules if necessary. This may also include weekend hours on occasion.
Assist, as needed, in the agency's incorporation of elements of Family First Prevention Services legislation, including discharge planning, aftercare services, and ongoing changes requested by the NYS Office of Children and Family Services linked to the legislation; train in the Family Finding model.
Develop the discharge plan for each assigned child, identifying the resources to promote a smooth transition out of care; participate in discharge activities to ensure solid case closure.
Assume the role of Primary Contact Person, as assigned, to coordinate the discharge and aftercare activities that are part of agency protocols, completing assigned aftercare tasks.
Function as a member of the direct care planning team, meeting with RMT members, unit child care supervisors, consultants, nurses, and social workers to evaluate and plan for the therapeutic milieu.
Assume additional position-related duties and responsibilities as assigned by the Supervisor of Residential Case Management.

Position Overview:
The Certified Teacher Assistant collaborates with the Master Teacher in conducting the therapeutic educational program for each student assigned to the classroom.
Duties/Responsibilities include, but are not limited to:
Collaborating:
The Teacher Assistant collaborates with the Master Teacher to develop and implement the classroom educational programs directed by the IEP and Care Plan.
Informs the Master Teacher and the Care Plan Team of student and tutorial responses to the educational program s/he offers, and makes program recommendations as needed.

Direct Work with Students:

Following the Children's Health Homes Care Management program guidelines as set forth by the NYS Department of Health, the Health Homes Care Manager provides comprehensive care planning, ongoing care coordination, and community linkage to Medicaid eligible children with complex physical and behavioral health conditions and their families. The aim is to improve their overall health and well-being, and to foster their independence. The Children's Health Homes Care Management program is now under Community Outreach & Care Coordination.
This full-time position is Mon - Fri, 9am to 5pm, with some flexibility.
Position Requirements
Bachelor's degree in psychology, sociology, counseling or other human service related field, required.
Minimum of 2 years of experience in human services field (child welfare, mental health, substance abuse and physical health), required.
Proficiency in various computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.); and secure Internet practices.
Experience with AWARDS / NYS Connections, a plus!
Ability to pass the test to become a certified CANS assessor.

Essential Duties & Responsibilities
Uses a strength-based and client-centered approach to build rapport and establish a therapeutic relationship with the child and care-givers.
For all enrolled children, conducts a comprehensive assessment of the child's healthcare needs and the families' social determinants of health needs using the contracted Health Home's designated health assessment tool, and the NYS CANS assessment.
Coordinates and conducts interdisciplinary team meetings with associated health care providers, social support providers, and identified family supports to develop and the child's Care Plan.
Along with child and family input, develops a Safety and Crisis Plan.
Carries out responsibilities set forth in the child's Care Plan, which may include making referrals, monitoring access to needed health care providers, linkage to social and community support providers, coordination amongst the interdisciplinary care team, and advocacy.
Maintains the Care Plan and monitors the completion of Care Plan goals.
When a child is admitted and/or discharged from a health care facility, including ER and inpatient hospital stays, will provide transitional care to ensure the continuity of care.
Documents the delivery of billable health home core services and all other case-related contacts in the contracted health home's EHR.
For all youth, ensures the requisite number of billable health home core services and face to face contacts are made during the month.
Complies with all time-lines for the completion of the assessments, development of the child's Care Plan and the scheduling of the initial and ongoing interdisciplinary team meetings.
Conducts comprehensive discharge planning with all children exiting the program.
Participates in monthly Quality Assurance meetings.
Completes all mandated trainings as set forth by DOH, the contracted Health Homes and the MCO's.
Assists with the distribution and collection of bi-annual satisfaction surveys.

Participates in weekly meetings of Day Treatment Placement Team, considering new admissions, the children's transitions from one program to another, and continuum of care for least restrictive environment and/or return home to permanent family.
Ensures that assigned social work staff facilitate interdisciplinary treatment team meetings at initial (30 Day), quarterly, and other as needed intervals, for the children and families in the Day Treatment Department, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.
Ensures that assigned social work staff complete and document psycho-social assessments, and treatment plans developed by treatment teams at Treatment Planning Meetings, at initial (30 Day), quarterly, and other as needed intervals, for the children and families in the Day Treatment Department, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.
Ensures that treatment planning is focused on return to permanent home or to least restrictive setting as soon as possible, with provision of aftercare services when needed.
Actively engages parents in treatment planning and fosters partnership with parents in all Day Treatment operations.
Ensures that child, family members, referring agency, and other involved community members participate in the assessment and treatment planning process for each child/family served, collaborate in the development and implementation of the service plan, and that the needs and goals of all involved parties are addressed.
Collaborates with the R&E May School CSE Chair in the development and implementation of social work services as mandated in the IEP.
Ensures that social workers participate in the development and implementation of the FBA, BIP, ICSP, and Safety Plan, according to the requirements of regulatory bodies and the policies and philosophy of St. Catherine's Center for Children.

Provides timely review and response for all incident reports involving social work staff, according to required agency and regulatory protocols/procedures.
Completes and submits NIMRS reports, as required by NYS OMH.
Ensures that required Post Crisis Response steps are conducted for youth, parents, and staff following critical incidents that involve social work staff.
Ensures active participation by social work staff in weekly meetings of the Classroom Team meetings.
Participates in monthly Quality Assurance and Critical Incident Review meetings with the Principal for Continuous Quality Improvement.

Duties & Responsibilities include
As a member of the foster care treatment team, provides clinical updates to the team including, but not limited to, child's level of engagement in treatment, movement toward treatment goals, and recommendations for permanency.
Foster Care Clinician will maintain sufficient contact with the child's school to ensure the child is benefiting from optimum educational placement. The clinician will also participate in CSE meetings and maintain contact with CSE chairperson as requested.
Document treatment processes as required by SCCC and by its regulating agencies including:
Completion of thorough and up-to-date progress notes in both SCCC's EHR and NYS Connections.
Work with foster parents to develop an ICSP for the child.
Preparation of clinical assessments, court reports, correspondence with other agencies, and social summaries.
Completion of clinical assessments upon intake.
Attend all court hearings pertaining to the placement, permanency, and treatment of the child and family as requested by referral source.
Function as a member of the Foster Care Team, including the Family Specialist and program Supervisor.
Additional position-related duties and responsibilities assigned by the Supervisor of 29i Clinical Services and/or AED of Clinical Services.

Position Overview
Following the Children's Health Homes Care Management program guidelines as set forth by the NYS Department of Health, the Health Homes Care Manager provides comprehensive care planning, ongoing care coordination, and community linkage to Medicaid eligible children with complex physical and behavioral health conditions and their families. The aim is to improve their overall health and well-being, and to foster their independence. The Children's Health Homes Care Management program is now under Community Outreach & Care Coordination.
This full-time position is Mon - Fri, 9am to 5pm, with some flexibility.
Position Requirements
Bachelor's degree in psychology, sociology, counseling or other human service related field, required.
Minimum of 2 years of experience in human services field (child welfare, mental health, substance abuse and physical health), required.
Proficiency in various computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Google Docs, Meet, Sheets, etc.); and secure Internet practices.
Experience with AWARDS / NYS Connections, a plus!

Coordinates and conducts interdisciplinary team meetings with associated health care providers, social support providers, and identified family supports to develop and the child's Care Plan.
Along with child and family input, develops a Safety and Crisis Plan.
Carries out responsibilities set forth in the child's Care Plan, which may include making referrals, monitoring access to needed health care providers, linkage to social and community support providers, coordination amongst the interdisciplinary care team, and advocacy.
Maintains the Care Plan and monitors the completion of Care Plan goals.
When a child is admitted and/or discharged from a health care facility, including ER and inpatient hospital stays, will provide transitional care to ensure the continuity of care.
Documents the delivery of billable health home core services and all other case-related contacts in the contracted health home's EHR.
For all youth, ensures the requisite number of billable health home core services and face to face contacts are made during the month.
Complies with all time-lines for the completion of the assessments, development of the child's Care Plan and the scheduling of the initial and ongoing interdisciplinary team meetings.
Conducts comprehensive discharge planning with all children exiting the program.
Participates in monthly Quality Assurance meetings.
Completes all mandated trainings as set forth by DOH, the contracted Health Homes and the MCO's.
Assists with the distribution and collection of bi-annual satisfaction surveys.

Position Overview
The Client Outreach and Engagement staff members will act as the triage point for Law Enforcement Assisted Diversion (“LEAD”) in Albany, Cohoes, and Watervliet. As such, this position must ensure effective and efficient communication and collaboration between all partners involved in referring and receiving referrals and diversions for LEAD.
This position will provide person-centered case management in the community to individuals referred to LEAD. Case management services may include accompanying clients to necessary services such as homeless shelters, the Department of Social Services, court appointments, probation appointments, medical services, and meeting at a client's home or in places in the community that have been mutually agreed upon.
This position may also provide screening, outreach and engagement services on an as-needed basis. The Case Management staff members will conduct street outreach as needed to engage referred and diverted individuals who have yet to engage with their assigned case manager.
The Outreach and Engagement staff must develop and maintain positive, collaborative relationships with all LEAD partners and other service providers in order to best serve LEAD participants.
Position Requirements:
High School Diploma or equivalent (GED/ TASC) required; College highly preferred.
One year of experience in homeless services.
Flexibility to work hours outside of regularly scheduled work hours, within reason.
Demonstrated ability to work well with clients and cohesively as a team member.
Previous experience working in a human services agency is a plus!

Position Overview:
The agency's Mid-Hudson Permanency Resource Center (“PRC” or “Center”) is designed to support and deliver specialized services to post-adoption, post-guardianship, and kinship caregiving families. Position Specifications & Requirements:
Bachelor's Degree in social work, psychology, or human services, required.
Associate's with 2+ years of case work or human services field experience or other related professional experience, may be considered.

Position Requirements

Position Overview
The Marillac Case Manager is responsible for assisting families with all facets of the intake process. This includes, but is not limited to performing assessments, service plan development, and coordinating housing activities with families.
Requirements & Specifications
Ability to work: Monday-Friday, 9:00am - 5:00pm w/ flexibility as needed.
Associates degree in related field with two years of relevant experience; BSW or Bachelor's Degree in Human Services field, preferred.
Will consider previous professional and/or life experience in lieu of college degree (must have a high school diploma or GED equivalent).
Previous experience in case planning or case management, preferred.
A Clean and Valid New York State Driver's License (actively licensed at least one year) is required*.
Proficient computer skills: Microsoft Office Suite; Google Business Workspace Suite; and use secure Internet practices.
A professional record of integrity and strong advocacy for homeless families.
Ability to work flexible hours to accommodate client schedules, which may include evening and/or weekend hours.
Previous experience working in a human services agency is a plus!
Capable of relating to a culturally diverse and economically disadvantaged population; capacity to exhibit empathy for those in need; willing and able to work in close proximity with the clients we serve.
Familiarity with public assistance and other community resources.
Self-motivated and engaged, with a strong attention to detail; Excellent writing aptitude.
A compassionate, warm and caring mindset; Capable of exhibiting empathy for those in crisis.
Ability to work with clients with mental health issues, substance abuse and developmental disabilities.
A professional record of integrity, proven reliability and dependability; Qualities of positivity, flexibility and adaptability.
Capable of exercising ethical judgment and discretion while handling sensitive data with strict confidentiality.
Outstanding interpersonal and communication skills; Capable of building strong working relationships with regulatory bodies.
*Staff in driving positions must be able to meet the agency's minimum requirements of our insurance policy for authorized drivers; including, but not limited to the minimum age of 21 at time of hire, and the ability to maintain insurability throughout employment.
Essential Duties & Responsibilities
Conduct and complete documentation for intake/assessment process, initial assessment, and quarterly/biweekly treatment planning review meetings.
Coordinate service plan delivery to families through a large network of area service providers.
In accordance with the NYS OTDA, ensure forward progression of goals; structured review of individualized families' service plans meet deadlines as set by NYS OTDA requirements.
Develop service plan goals for families; Assist families with to assess their needs and strengths and achieving service plan goals.
Act as the primary advocate and primary liaison for families when communicating with public assistance and other resource providers.
Assist families in obtaining necessary services needed to retain permanent housing (e.g. mental health services, education and employment services, substance abuse services).
Assist in identifying and resolving barriers to housing retention by securing needed rehabilitative and/or supportive care, day care.
Provide intensive case management services to settle family in permanent housing and stabilize conditions.
Coordinate transportation when appropriate for families.
Complete and submit all assessments and reports on time; Perform and maintain record keeping statistics, as required.
Intervene in crisis situations and assist with conflict resolution; providing mediation and facilitating communication, especially between landlord and tenant.
Additional position-related duties and responsibilities as assigned by the Director.

Position Overview
The Transporter, Case Aide is primarily responsible for transporting foster care children to and from case-related appointments which may include, but are not limited to, medical appointments, therapy appointments, and family visitation. Additionally, the Transporter, Case Aide is also responsible for assisting the Family Specialists in other case-related functions which may include helping with family visits.
Position Requirements & Specifications
High School diploma / GED Equivalent, required

Capable of directing children in a structured environment
Possess an attitude of compassion for children in crisis
Physical and emotional health conducive to working with children in care
It is essential to have patience, flexibility and adaptability
Experience with children who have a range of mental health diagnosis
Effective communication skills (written and verbal)
Willing and able to work in close proximity with the clients we serve

Communicate with medical professionals, therapists, and caseworkers about the child's current needs and issues.
Appropriately clean out the vehicle after each transport.
Assist with case-related family visitations.
Set limits necessary to provide a safe, secure, and therapeutic environment when transporting children.
Following transport of other case-related contacts, update the child's Family Specialist or Program Supervisor on the quality of the contact, detailing any unusual or concerning behavior.
Assist with other case-related functions directed by program staff.
Additional position-related duties and responsibilities as assigned by the Supervisor.
