Full-time Description We are seeking a highly skilled and compassionate Clinic Registered Nurse to join our team: Full-time Benefit Eligible (0.8 - 1.0 FTE) Primarily scheduled at our St. Croix Falls location but may need to travel to community clinics to provide support. Start and end times vary with hours ranging from 7 AM-6 PM Monday through Friday and 8-4 on rotating weekends and holidays. As a Clinic Registered Nurse, you will be responsible for providing exceptional patient care and support in a clinical setting. You will work closely with physicians and other healthcare professionals to ensure that patients receive the best possible care. The Clinic RN rooms patients, acts as telephone triage nurse, refills medications per SCRMC protocol, coordinates chronic disease care, triages walk-in patients and assists providers with exams as needed. Essential Duties and Responsibilities: Plans for delivery of optimum patient care, patient care coordination, assesses and delivers patient care based on department and facility standards. Perform various nursing procedures within the scope of licensure, including but not limited to IV starts, urinary catheterization, patient education Room patients Assist providers with procedures, chronic disease care management and care coordination Administer immunizations and complete appropriate documentation in EMR Assist LPN and CMA staff with advanced procedures as needed Ensure patient phone calls to the provider are accurately documented in the EMR, including supporting documentation detailing resolution Reviews and completes medication refills for primary clinic provider's in a timely matter Manage triage duties and responsibilities. Answer triage line; assist concerned patients via phone Follow triage protocols to determine appropriate next steps Act as walk-in patient triage nurse; perform RN duties and responsibilities as needed based on situation Complies with department and facility standards. Understand Clinic Policies and Procedures Perform work according to organization policies and procedures Other duties as assigned. In this role, you will ensure that intake and referral processes are completed efficiently and effectively to meet the needs of patients, families, referral sources, and the organization. You will play a vital part in building strong professional relationships, coordinating admissions, and supporting community awareness of hospice services. Essential Responsibilities Receive and respond to referrals from hospitals, home health, physicians, case managers, skilled nursing facilities, and other healthcare professionals Assess patient eligibility for hospice services by reviewing medical records, conducting clinical evaluations, and communicating with physicians and families Educate patients and families on what hospice care includes, how it works, and why it matters-providing clarity, reassurance, and compassion Complete necessary documentation and coordinate the intake and admission process, ensuring all legal, clinical, and payer requirements are met Serve as a liaison between referral sources and our hospice care team-ensuring seamless handoffs, clear communication, and rapid response to needs Make daily visits to hospitals and facilities, maintaining visibility and accessibility with discharge planners, case managers, and physicians Build and maintain strong relationships with referral partners and identify new outreach opportunities to increase awareness of hospice services About You Education Licensed Nurse with three to five years of hospice experience required. BSN with five or more years of hospice experience preferred. Experience Minimum of three years of hospice experience required. Prior experience as a discharge planner, case manager, or educator preferred. Licenses and Certifications Current RN license to practice nursing in the state of employment. CPR certification. Valid driver's license and automobile insurance. Skills and Knowledge Strong knowledge of hospice care and the unique needs of patients and families. Familiarity with community resources and referral networks. Excellent patient assessment and documentation skills. Ability to supervise staff, coordinate services, and manage programs effectively. Strong communication and interpersonal skills with sensitivity to diverse backgrounds. Personal Qualities Compassion and sensitivity to the needs of terminally ill patients and their families. Flexibility to manage a variety of tasks and shifting priorities. Professional maturity, collaboration, and tact in working with interdisciplinary teams. Qualifications On an An Average Day you Will: (Includes, but not limited to) Will perform after hours and weekend visits and admissions as needed.
