Chaplain Position Overview The Hospice Chaplain provides emotional, practical and/or spiritual guidance for patients and their families, meeting them where they are on their hospice journey. As part of the patient care team, chaplains support the unique needs of each patient, including visiting with patients and their families, coordinating with the clinical team and connecting them with community resources and services. Full-time Description St. Croix Health is looking for a full-time (1.0 FTE) Environmental Services Aide to join our team. This position will work 80 hours per pay period, 2:30 pm - 11:00 pm, every other weekend and be a part of the holiday rotation. The Environmental Services Aide is responsible for executing the daily duties required to establish and maintain a safe, hygienic and aesthetically pleasing care environment. This position will have contact with patients, visitors and clinical/non-clinical staff. The Environmental Services Aide is responsible for following basic principles of environmental hygiene and infection prevention. 1. Cleans all assigned areas throughout the facility. Responsible for cleaning and disinfection of assigned patient care areas in a timely manner Adheres to organization safety, risk management and infection control guidelines and standards while promoting a culture of safety Delivers clean linen to appropriate areas as directed; Monitors and replenishes necessary inventory levels as needed Removes waste and transports to appropriate disposal area; Identifies proper waste handling method Maintains department standards on an ongoing basis and assists in the responsibilities for survey readiness 2. Compliance Auditor (Clinical) Position Overview The Compliance Auditor is responsible for assisting with the development and implementation of the compliance audit program to ensure regulatory adherence, risk mitigation and operational integrity. This role ensures that St. Croix Hospice complies with federal and state laws and regulations and internal policies by identifying potential risks and working cross-functionally to implement corrective actions. The Compliance Auditor will lead risk assessments, internal audits and compliance investigations while collaborating with leadership to mitigate risks and strengthen internal controls. Part-time Description St. Croix Health is currently seeking a casual Polysomnographic Technologist to join our team! This position will work one shift every two weeks and fill in as needed. The Polysomnographic Technologist is responsible for performing comprehensive polysomnographic testing, analysis of the testing, and working under the general supervision of the Medical Director. The Technologist performs tests and procedures that test the brain waves and functions on sleep patients to perform studies and determine why they are having trouble sleeping through the night. They will review patient medical history and document their sleep issues, and keep records of patient progress. They will also explain to patients the nature of procedures that will be performed on them and prepare them for the procedures. During studies, equipment may be attached to their bodies and make sure the patient is comfortable before sleeping. This position is responsible for scoring all sleep studies once completed. Requirements Essential Duties and Responsibilities: 1. Conduct sleep study procedures by following proper procedural protocols and accurately score the completed sleep study. Document routine observations including sleep stages and clinical events, changes in procedure and significant events to facilitate scoring and interpretation of polysomnographic results Perform data acquisition while monitoring study-tracing quality to ensure signals are artifact-free. Identify, correct and document artifacts Implement appropriate interventions (including actions necessary for patient safety and therapeutic intervention such as positive airway pressure, oxygen administration, etc.) Score sleep/wake stages by applying professionally accepted guidelines Score clinical events (such as respiratory events, cardiac events, limb movements, arousals, etc.) utilizing American Academy of Sleep Medicine (AASM) standards Generate accurate reports by tabulating sleep/wake and clinical event data Demonstrate the knowledge and skills necessary to recognize and provide age specific care in the treatment, assessment, and education of neonatal, pediatric, adolescent, adult, and geriatric patients Demonstrate the knowledge and skills necessary to perform portable monitoring equipment preparation and data download Perform patient education and instruction appropriate for home sleep apnea testing Demonstrate adherence to cleaning and disinfection procedures Monitor patient safety and adhere to patient safety protocols Demonstrate adherence to AASM scoring parameters; identify artifact, inadequate signals and equipment failures and generate an accurate report 2. Follow procedure and protocols appropriately to ensure proper safety and patient care, including “lights out” and “lights on” procedures. Follow “lights out” procedures to establish and document baseline values (such as body position, oxyhemoglobin saturation, respiratory and heart rates, etc.) Follow “lights on” procedures to verify integrity of collected data and complete the data collection process (e.g. repeat the physiological and instrument calibrations and instruct the patient on completing questionnaires, checking for completeness, etc.) 3. Social Worker Position Overview The Social Worker is an experienced professional and member of the patient care team who provides emotional and practical support for patients and their families in accordance with their unique Plan of Care. As part of the patient care team the Social Worker meets with patients and their families, coordinates with the clinical team and connects them with community resources and services. Essential Functions and Skills Patient Care * Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the Registered Nurse and other members of the interdisciplinary care team. Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required. * Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, suicide risk, neglect, or abuse, and plans intervention based on evaluation findings. * Counsels' patients and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Provides crisis intervention when necessary. * Provides ongoing Bereavement support to individuals identified as moderate or high-risk. * Assesses any special needs related to the culture of the patient and family while educating interdisciplinary care team. The Sterilization and Reprocessing Technician assists the Operating Room staff in the organization and daily function of the operating room in accordance with established accreditation standards and facility policies and procedures. Inspect all instruments for pitting, rust, fractures, cracks, and compromised insulation before placing an appropriate container, wrap or package. Ensure instruments and equipment are in good working order and recommends the repair or replacement appropriately. Respond to requests from materials management, outlying clinics and specialty areas. Follow all department and hospital policies. Maintain a clean and orderly work area. Maintain basic knowledge in infection control and prevention. Able to recognize potential and/or errors and take appropriate action. Implement best practice and utilize continuing education to promote process improvement. Monitor & re-order instrumentation, stock and reprocessing supplies.
