RN Case Manager Position Overview The Registered Nurse Case Manager (RNCM) is experienced in nursing, and leads each patient's hospice care team, helping to create and manage their specific Plan of Care in collaboration with the Hospice Medical Director, patient, family and/or facility staff. The RN Case Manager provides day-to-day patient care, manages treatment, administers medications, maintains patient records and directs other members of the patient care team to meet the changing needs of the hospice patient, with a focus on providing comfort and enhanced quality of life. Essential Functions and Skills Patient Care * Provides care in accordance with the Nurse Practice Act and professional nursing standards. * Conducts comprehensive patient and family assessments to determine hospice needs, including physical assessments and health history. * Develops, implements, and revises the Plan of Care based on ongoing patient evaluations. * Administers medications and treatments as prescribed in the physician's plan of care. * Educates patients and families on self-care, medications, and diet as appropriate. * Maintains accurate clinical documentation, recording pain management, symptoms, and changes in condition. * Coordinates care with physicians, interdisciplinary teams, and community health partners. * Ensures a safe environment for patients and supports their activities of daily living. * Provides emotional support and counseling to patients and families. * Arrange for necessary medical equipment and supplies. * Ensures HIPAA compliance and patient confidentiality. Additional Duties * Participates in on-call visits as required including evenings and weekends. * Attend interdisciplinary group meetings. * Supervises ancillary personnel and delegates responsibilities as needed. * Engage in professional development through training and continuing education. * Contributes to quality assessment and performance improvement activities. * Performs other duties as assigned.

Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. * Provides and maintains a safe environment for the patient. * Assists the patient and family/caregiver and other team members in providing continuity of care. * Works in cooperation with the family/caregiver and hospice interdisciplinary group to meet the emotional needs of the patient and family/caregiver. * Attends interdisciplinary group meetings.
