PRIMARY JOB RESPONSIBILITIES
* The RN2CM works as a Care Manager with patients by offering exceptional communication and an ability to expertly assess patient needs.
* Works closely with Providers and applies professional skills and knowledge of established concepts, principles, and practice to provide care coordination to patients.
* Assesses psychosocial needs and implements strategies to ensure patients receive the appropriate levels of care.
* Completes a nursing assessment to include patient's medical history, disease process, medications, laboratory data and manages therapy per provider's treatment plan.
* Communicates plan of care in a timely manner to provider, patient, and family to establish mutual goals.
* Assesses patients/families' knowledge, learning needs, readiness, and comprehension abilities.
* Educates and coaches patients/families on disease states, behavior modification, and ED and resource utilization.
* Applies nursing best practices to identify and prioritize healthcare triage interventions that are appropriate, effective and that reduce the risk of adverse outcomes.
* Answers calls in a timely manner, follows policy to document call inquiries, maintains confidentiality of all caller and personnel issues, and transcribes patient information electronically and through telemedicine digital technology.
REQUIREMENTS
* Current license to practice as a registerednurse in the State of Washington

POSITION SUMMARY:
Responsible for the supervision of all nursing services within Health Services.
Duties and Responsibilities
KEY RESPONSIBILITIES:
Develop, document, and maintain nursing policies and procedures which ensure compliance with University, state, and federal regulations, as well as meet the standards of the American College Health Association and the Joint Commission.
Monitor performance for compliance with established policies and procedures to ensure effective operation.
Evaluate opportunities for improvement in operations and present proposals for innovation or revision.
Perform nursing duties consistent with RI state licensure.
Establish, document, and monitor an infection control program consistent with regulations (OSHA) and accreditation standards (JC).
Coordinate operation of clinical areas to include oversight of stocking of exam rooms, ordering of medical supplies, and maintenance/calibration/safety testing of all medical devices.
Develop, document, and coordinate an employee orientation program consistent with regulations and accreditation standards.
Participate in a broad variety of meetings that include leadership team, safety and infection control and clinical staff. Chair regular department and special function meetings.
Schedule nurses and develop and implement appropriate staffing patterns.
Develop and implement continuing nursing education programs which are recognized by the Rhode Island State Nurses Association.
Coordinate nursing services with other departments within Health Services.
Initiate job descriptions and assignment descriptions for all nursing personnel.
Interview and evaluate applications for nursing vacancies.
Relate to various union contracts and function as liaison with the various units. Administer the Collective Bargaining Agreements as they relate to nursing services.
Make recommendations to the Director regarding the Nursing Department's budget.

Saint Elizabeth Home, a 168 bed skilled nursing and rehab center is currently hiring an RN MDS Coordinator.
The RN MDS Coordinator schedules, coordinates, directs, oversees, and participates in the completion of the Resident Assessment Instrument (MDS, RAPS, CARE PLANNING) in accordance with federal and state regulations as well as standards for long term care facilities established by the Joint Commission on the Accreditation of Healthcare Organizations. Ensures MDS and RAPS are completed accurately, and care plans evolve from comprehensive, multidisciplinary assessments.
Qualifications for an RN: MDS Coordinator:
Must be a RegisteredNurse licensed by the state of Rhode Island.
Must have, as a minimum, one (1) year experience as a supervisor in a hospital, long-term care facility, or other related health care facility.
Must have 2-3 years geriatric nursing experience.
