About the Position
We have an opportunity for an Outpatient Therapist to provide clinical services at our Downtown Clinic in Medford, Oregon. The Outpatient Therapist reports to the Outpatient Clinical Manager for day-to-day supervision and work assignments. In this position, you are responsible for performing a Provisional Assessment for individuals experiencing crisis. The Outpatient Therapist will also provide clinical oversight and consultation to facility staff and liaison with other community providers. In addition, you will receive crisis de-escalation training, which requires performing various physical, hands-on maneuvers and techniques. If desired, the Outpatient Therapist will be eligible to participate in our supervision toward licensure program after their successful completion of the 90-day introductory period.
Work Schedule: Monday through Friday, 8:00am to 5:00pm (Full Time, Day)

About the Program
Our Outpatient Program provides assessment and evaluation, treatment plan development, and provides clinically indicated outpatient therapy, case management, skills training, and psychiatric (medication) services. The Psychiatric Services Team operates as a branch within the Outpatient Program, specializing in psychiatric, nursing, and medication services. Psychiatric services are provided both on-site and remotely through telemedicine.

About the Position
We have an exciting career opportunity for an Intensive Supportive Housing Therapist who possesses a combination of skills and QMHP eligibility to provide clinical services for our Southern Oregon supportive housing programs in Medford, Oregon. The Intensive Supportive Housing Therapist position requires the ability to conduct mental status examinations and DSM-5 diagnoses. They will conduct comprehensive psychosocial mental health assessments and complete assessment reviews and updates every six months or as clinically indicated. Swing Lane Supportive Housing is an exciting, newly constructed housing program in Medford, Oregon that provides permanent, intensive Supportive Housing, as well as transitional Room & Board units for adults who have a serious mental illness.
Rogue Ridge Apartments is a brand-new, 28-unit, permanent Supportive Housing program in Ashland, Oregon. Rogue Ridge serves adults who are houseless and living with a mental illness. The program is in a beautiful three-story apartment complex with elevators, ADA units, and a community/activity room for residents to enjoy. Supportive services include on-site staff who provide peer support, skills training, tenancy assistance, community navigation, and more; 24/7 access to housing support; and tenancy services and support around move in and out, rent collection, maintenance, and repairs.

About the Position
We are looking for a Residential Treatment Therapist to join our team for our Southern Oregon region. The Residential Treatment Therapist will serve as a role model through the provision of recovery-oriented interventions to promote and nurture a healthy treatment culture. Qualifications
Education: a Master's Degree from an accredited college or university in Social Work, Counseling, Psychology, Marriage and Family Therapy, or equivalent is required.
Experience: Preferred: 1+ years in behavioralhealth. Experience in group therapy, crisis intervention, and treatment planning.
Licensure: Clinical licensure (i.e. LPC, LCSW, LMFT) is not required, but a Master's Degree (as described above) is

* Welcome assigned patients and inquire as to their well-being since their last treatment. Report any complaints or observations to the nurse supervisor.
* Evaluate vascular access for patency and report any unusual findings to nurse supervisor.
* Obtain necessary pre and post treatment vital signs and weight. Report unusual findings to nurse supervisor.

* Perform vascular access cannulation and report any difficulties in cannulation or unusual findings to nurse supervisor.

* Evaluate the patient for prior to discharge and report any unusual findings to nurse supervisor.
* Perform and record Pre and Post dialysis evaluation weight and vital signs with initial identification.
* Responsible for calculating and entry of individual patients' dialysis machine programming for Ultrafiltration (UF) goal; treatment time; Sodium (Na) modeling; and UF modeling as prescribed.
* Responsible for ensuring appropriate safety alarms are enabled including Narrow Venous Limits (NVL) 160.
* Record accurate and timely information regarding vital signs and treatment parameters onto the Hemodialysis Treatment Sheet with initial identification.
* Responsible for accurate documentation of information related to patient treatment including completion of Hemodialysis Treatment Sheet and if applicable progress notes.
* Report any significant information and/or change in patient condition directly to the nurse supervisor.
* Observe patient and conduct machine safety checks according to facility policy. Report any change or unusual findings to the nurse supervisor.
* Perform and document any intervention for unusual patient status and document patients' response to intervention.
* Evaluate patients' vascular access during treatment including arterial and venous monitoring pressures provide appropriate intervention as needed document and report any unusual findings to the nurse supervisor.
* Obtain all prescribed laboratory testing and prepare specimens for collection.
MAINTENANCE/TECHNICAL:
* Maintain environmental integrity and aesthetics - ensure all areas are safe and clean.
* Clean and disinfect dialysis machine surface chair equipment and surrounding area between treatments according to facility policy.
* Conduct all tasks necessary for preparation for dialysis treatment and document where appropriate.
* Perform all required pretreatment dialysis machine alarm testing including Pressure Holding (PHT).
* Initiate Solution Delivery System (SDS) system.
* Ensure that all blood spills are immediately cleaned with appropriate disinfectant according to facility policy.
* Prepare organize and efficiently utilize supplies and equipment to prevent wastage.
CLERICAL & ADMINISTRATIVE:
* Complete and document ongoing participation in Continuous Quality Improvement (CQI) activities.
* Enters all treatment data into the designated clinical application accurately and in a timely manner.
* Review treatment sheets for completeness ensure nursing signatures are documented and ensure omitted entries are completed or corrected where appropriate.

Qualifications
Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field required.
Must possess a TX State licensure: LPC, LPC-I, LPC-A, LMSW, LMFT or LCSW required.

Hackensack Meridian Health Carrier Clinic's East Mountain Youth Lodge provides a continuum of out of home residential treatment services for adolescents between the ages of 13-18. We provide 24 hour clinical, therapeutic treatment for emotional, behavioral, psychiatric and substance abuse difficulties based on the resident strengths and needs.
Responsibilties:
1. Directly supervises and knows the whereabouts of every resident they are assigned. 2. Supervises, observes, and participates with residents in the carrying out of the daily routine (i.e. chores). Consistently implements all program procedures (i.e. point systems, level systems).
3. Runs activity, support and recreation groups with guidance from clinical staff. Organizes, supervises and conducts planned activities for groups and individuals. Provides assistance to residents in the planning of appropriate recreational activities.
4. Serves as a primary counselor for an assigned number of residents.
5. Assists residents in learning home management skills (cooking, cleaning, etc). Teaches residents about the importance of cleanliness, good nutrition, meal planning, prudent buying, etc., while guiding them in learning skill mastery.
6. Corrects client behavior by administering reasonable and fair disciplinary measures when necessary. Ensures each resident is informed of the rules and regulations of the program.
7. Provides crisis intervention to residents as needed. Assists in escorts and personal restraints of agitated residents. Attentive to building security and performs such checks as designed by the program.
8. Conducts resident room checks and room searches as designed by the program.
9. Carries a beeper for one week in rotation. Is on-call and ready to respond to staff shortages by calling and locating a replacement and/or filling the shift him/herself.
10. Inspects and handles belongings and valuables of residents to ensure that prohibited material is not brought into the Lodge at admission, upon return from pass, or as clinically indicated, under the direction of a Shift Leader.
11. Assists residents with daily living activities (i.e.; waking up, hygiene, cleaning room, participating in structure, behaving appropriately on activity).
12. Provides input to administration members regarding unit-based PI measures based on concerns, high risk, high volume issues.
13. Ensures that paperwork is completed accurately and within procedural time limits. 14. Other duties and/or projects as assigned.

Notifies licensed staff of abnormal findings related to Vital Signs, Capillary Blood Glucose Measurement and changes in patient status.
Understands that attendance affects team morale and confidence. Demonstrates reliability and punctuality in regard to attendance. Consistently is present and ready to begin work activities at start of shift, stays within established break periods, arrives on time for meetings and leaves work area promptly at the conclusion of the shift to reduce potential distractions. Schedules personal appointments or requests adjustment to schedule with least amount of disruption to service delivery. Provides prompt notice for unexpected absences.
