What you will do:
We are looking for a Primary Care Physician interested in providing primary care to underserved communities and join us as Residency Faculty as we enter into the 2nd year of our ACGME accredited Family Medicine Residency Training Program. DVCH is the first and only teaching FQHC in Philadelphia!
As a member of the primary health care team, this position is responsible for rendering professional medical services to the patient population of Delaware Valley Community Health, Inc.. This position provides care to patients with special awareness of vulnerable populations treated in the health centers that may face barriers to care due to residence, health, age, personal characteristics, ability to communicate, chronic illnesses and comorbidities, high ER utilization, developmental status or financial status. The position is also accountable for the complete documentation of patient visits in the patient chart as well as administrative reporting activities as assigned by management.

Graduation from an accredited medical school is required.
Possession of a current license to practice medicine in the state of Pennsylvania is required.
Board certification in specialty is preferred.
Bilingual in Spanish and English preferred.
Experience with treating substance use disorder preferred
Interest in precepting medical students and residents
A minimum of one year experience as a practicing physician in primary health care setting is preferred.

Under the supervision of the medical director, the Primary Care physician (PCP) provides primary medical care to PACE Organization (PO) participants, including assessment, development of a plan of care in collaboration with the interdisciplinary team, provision of direct participant medical care, and evaluation of effectiveness of the plan of care. Functions in a collegial relationship with the other health care professionals, making independent decisions regarding medical needs and health regimens. Carries out medical procedures as indicated.
*Position Details:*
Mercy LIFE Assisi House is the retirement convent for the Sisters of St. Francis of Philadelphia. It is located across the street from Our Lady of Angels Convent, the congregation's motherhouse. A “person-centered care” approach fosters a sense of family throughout the 125 bed, multi-level care facility, which includes an Alzheimer's and Dementia unit.
Mercy Life Valley View provides both housing and the Mercy LIFE PACE program for senior adults. This program provides services to a community specifically comprised of deaf seniors. Candidates must be willing to learn American Sign Language.
This position will serve two 8 hour days at Assisi House and one 4 hous day at Valley View. Days worked during the week are flexible.
No night, weekend, on call or holidays coverage is required with this position.
*What you will do:*
* Participate as a member of the interdisciplinary team (IDT) and in initial, semi-annual, unscheduled, and annual assessments. Report changes in participants' baseline status to appropriate staff on a daily basis.
* Provide initial and ongoing participant assessments, care plan development, and implementation of care plan. Integrate the primary care treatment plan into the overall plan of care developed by the interdisciplinary team.
* Refer participants to medical specialist as indicated. Manages all care of participants in the nursing home: Provides regular visits to manage the care of participants in the nursing home in collaboration with the Interdisciplinary Team; maintains communication with nursing home staff for continuous monitoring of PACE participants on a case-by-case basis; admits nursing home participants to hospital when necessary.
* Oversees and manages participant's use of medical specialist and inpatient care. Communicates and follows up with specialist regarding outcome of visit.
* Review medical outcomes to establish best practices and improve participant health. Complete documentation and uses appropriate diagnostic coding.
* Advise the Medical Director in ways and means to establish better accountability of PO services to participants and referral sources, keeps Medical Director aware of needed material and human resources as program expands.
* Assist with the development of policies and procedure, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.

Position Summary:
The Physician will provide comprehensive medical care to patients with myeloid malignancy, including but not limited to acute myeloid leukemia (AML), chronic myeloid leukemia (CML), myelodysplastic syndromes (MDS), and myeloproliferative neoplasms (MPN). This includes overseeing all aspects of patient care, from initial diagnosis and treatment planning to follow-up and supportive care. A significant component of the role involves participation in the allo HSCT program, encompassing pre-transplant evaluation, conditioning regimen selection, transplant procedures, post-transplant management, and the management of complications.
Duties:
Provide comprehensive medical care to patients with myeloid malignancy, including diagnosis, staging, treatment planning, and ongoing management
Perform bone marrow aspiration and biopsy
Develop and implement individualized treatment plans, incorporating current best practices and clinical trial participation
Participate actively in the allo HSCT program, including patient selection, pre-transplant conditioning, transplant procedure, post-transplant management, and complication management

Essential Functions & Responsibilities
The physician serves as medical director for skilled nursing facilities (SNF) and participates in administrative and quality initiatives for the facilities he or she is responsible for;
Medical directorship functions include attending and participating in monthly quality assurance and process improvement meetings; participating in quality improvement initiatives; providing guidance to facility staff; reviewing and revising (if necessary) facility's clinical guidelines; insuring compliance with state and federal regulations, training facility staff; and supervising facility clinical staff.
In addition to the medical director role, the physician also serves as the attending physician for the residents residing at each facility;
The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;
Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;
Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes
Physician must be able to supervise collaborating nurse practitioners and physicians assistants
Compensation
Compensation ranges approximately from $1,100.00 - $1,200.00 (Part-Time)
Qualifications
Medical License
Strong understanding of medical terminology and physiology
Experience with managed care and EHR systems
Experience in patient care and hospital environments
