In this role, you will be responsible for performing all coordination between InterCommunity HealthCare external visiting nurse agencies and other community patient support personnel. Essential Duties & Responsibilities: Ensure that all VNA orders are correct and that medications are reconciled. Coordinate home care services for patients, including, but not limited to, coordinating with healthcare providers.
