Summary:
The Emergency Room Registered Nurse (ER RN) delivers rapid-response, high-quality patient care in a fast-paced emergency department setting. This role involves assessing patient conditions, implementing urgent care plans, administering life-saving treatments, and collaborating with multidisciplinary healthcare teams to ensure high-quality, efficient, patient-centered care in critical situations. The ER RN demonstrates strong clinical skills, exceptional critical thinking abilities, excellent communication under pressure, and a commitment to maintaining the highest standards of patient safety and professional ethics.
Essential Work Functions:
Rapidly assess and triage patients upon arrival, prioritizing care based on the severity of their condition
Explain emergency procedures and treatments to patients and families
Administer prescribed medications and treatments in accordance with approved emergency nursing techniques and protocols
Prepare equipment and aid physicians during emergency treatments and examinations
Monitor patient comfort and safety throughout their time in the emergency department, responding to immediate needs
Observe and document patient conditions as required within scope of practice
Take and monitor vital signs using clinical judgment to address deviations and prevent complications
Respond to life-saving situations based upon emergency nursing standards, policies, procedures, and protocols
Document comprehensive nursing assessments, interventions, and outcomes in electronic medical records (EMR)
Initiate patient education plans according to individualized needs, considering the acute nature of emergency care
Collaborate effectively with interdisciplinary teams including physicians, specialists, paramedics, social workers and other support staff to ensure comprehensive care
Perform other duties as assigned within the scope of practice
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards

PURPOSE AND SCOPE:
Works with the Facility Manager, facility staff and physician to coordinate the facility operations and patient procedures to ensure provision of quality patient care on a daily basis in accordance with policies, procedures and training. Provides nursing support to patients and staff. Assists with staffing, staff training, equipment, physician and patient relations, cost containment, supply management, medical records, patient billing, OSHA and all company, state and federal compliance.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
QUALITY:
Responsible for supporting and driving quality standards through meeting regulatory requirements and the practice of Continuous Quality Improvement (CQI), including use of CQI tools.
Assists with implementing quality goals and develops facility specific action plans to achieve quality standards.
Accountable for outstanding quality of patient care, as defined by the quality goals, by working with management to ensure that policies and procedures are followed.
Assists with implementing appropriate training according to policy; ensure ongoing compliance with all risk management initiatives.
Responsible for aggressively addressing and acting on adverse events and action thresholds.
Ensures all Quality policies and procedures are communicated to and implemented by the facility staff.
Maintains integrity of medical records and other administrative and operational records.
Complies with all data collections and auditing activities.
Maintains facility environmental integrity, including safety.
PATIENT RELATED:
Coordinates all aspects of patient care with the appropriate staff members, from admission through discharge.
May confer with patients prior to the procedure to obtain necessary medical history, discuss procedure and answer questions, as well as, performs nursing assessment on the day of the procedure.
Ensures patients and patient's families are educated regarding post procedure care of vascular access.
Acts as a resource for the staff to address patient concerns and questions.
Ensures patient's procedures are scheduled in a timely manner and the facility capacity is utilized efficiently.
As needed, assist with patient workflow, including providing direct patient care and monitoring pre, intra and post procedure.
Identifies and communicates patient related issues to the Facility Manager.
Oversees and ensures accurate and complete documentation of patient treatments from admission to discharge.
Confirms written orders, consents and lab work are completed and documented in accordance with facility policies. This includes appropriate preparation of lab requisitions, delivery to appropriate labs, reporting and forwarding of lab results to appropriate physician.
Assumes primary responsibility in an emergency situation and must maintain competency with all emergency operational procedures, and initiate CPR and emergency measures as needed.
Administers medications and IV conscious sedation to patients as directed per physician's orders, and in compliance with, company, federal, state and local regulations.
PHYSICIANS:
Ensures regular and effective communication with all physicians on an as needed basis.
Attends and participates in Governing Body, assisting with meeting agenda and gathering of data/material for meetings.
Attends and participates in CQI meetings with physicians, reporting on assigned topics.
Works with the physicians and staff to ensure possession of all necessary instruments and equipment.
MAINTENANCE/TECHNICAL
Ensures appropriate operation of facility equipment and technology, including but not limited, patient monitors, defibrillators, medical recording devices and computers.
Ensures the inspection of all facility equipment on a regular basis. Report any malfunctions or maintenance issues immediately.
Ensures the inspection of all facility operating systems, including temperature checks in appropriate areas and inspect the physical structure of the facility.
Maintains a clean, safe and sanitary environment in the entire vascular access facility at all times.
Ensures all blood spills are immediately addressed according to company Bloodborne Pathogen Control Policies.
ADMINISTRATIVE:
Participates in cost control initiatives. Responsible for efficient utilization of medication, laboratory, inventory, supplies and equipment to achieve supply cost goals following all guidelines established in the formularies.
Assists in the development and revision of any applicable policies and procedures for the facility as directed to ensure continuous process improvement.
Initiates, documents and completes ongoing Continuous Quality Improvement (CQI) activities including monthly reports and attend meetings as required.
Manages medications, including ordering, receiving, and inventory control and checking for expired medications, including appropriate recording of controlled substances as required by law.
Oversees maintenance of usage logs for all medications with appropriate documentation, as well as, records of drug disposal. Communicates with the pharmacist to assure medication accuracy, including appropriate recording of controlled substances as required by law.
Ensures medical records are accurate, complete and in compliance with policies and procedures prior to discharge of the patient. Performs chart audits on a regular basis.
Along with Facility Manager, coordinates the annual medical testing and competency testing for all facility staff.
Participates in all facility regulatory and accreditation surveys and action plans to address any deficiencies identified.
Verifies records for all implanted devices are completed accurately, timely and maintained according to records management guidelines.
Coordinates the collection and forwarding of all vendor invoices to the accounting department.
Acts as the Facility Manager in the absence of the manager or at the direction of management.
Ensure nurses' progress notes are accurate and complete.
Acts as the Patient Safety Officer and conducts quarterly patient safety meetings and fire drills.
Complies with HIPPA policies and standards regarding patient information and medical records.
Other duties as assigned.

6 - 8 years' related experience.
3+ years' supervisory or project/program management experience preferred.
Minimum of three years' experience in an Ambulatory Surgical Center/Vascular Access Lab, or a combination of ASC, surgical, Interventional Radiology and ICU/CCU experience.
Minimum of three years of Interventional Radiology, scrub experience preferred.
Supervisory or management experience within a medical or vascular access environment preferred.
Proficiency with the Microsoft office suite is necessary. Experience with Medical database software.
Demonstrated management skills necessary to provide leadership and supervision of facility personnel and to ensure the delivery of maximum quality care to all patients.
Good communication skills - verbal and written.
Certified in CPR or successfully complete course in CPR certification

The Forensic Nurse Examiner (FNE) will provide timely, non-judgmental, compassionate care to the sexual assault victim. This will include a forensic exam, prophylaxis for pregnancy and sexually transmitted diseases, photographic documentation, referrals for appropriate medical and psychological follow-up, support and participation in legal proceedings. The FNE is expected to follow protocols established by the Medical Director and/or management of the Forensic Program for which the FNE is employed.
JOB RESPONSIBILITIES:
Ability to triage patient to determine if medical exam is needed before forensic exam.
Coordinate care by all agencies involved in assisting the sexual assault victim and coordinate referrals for appropriate medical and psychological follow-up as needed for each patient.
Perform and document sexual assault and forensic interview accurately.
Perform and document domestic violence/non-fatal strangulation and forensic interview accurately.
Perform a complete physical and forensic evidence collection examination including Head-to-Toe assessment for trauma, detailed genital examination, forensic specimen collection and documentation, including photography, and assess for mandatory reporting status, reporting when appropriate.
Document detailed injuries accurately with support of photographic evidence.
Maintain chain of custody for each sexual assault exam and adhere to evidence protocols
Maintain professional standards that do not create conflict of interests with employment or patient care, including patient and medical record confidentiality
Administer appropriate prophylactic medications per protocols
Provide testimony as fact witness to any/all cases performed as FNE and coordinate with attorneys during judicial process.

The RN serves as Subject Matter Expert for Root Cause Analysis, Safety Culture, National Patient Safety Goals, and other evidence-based programs as needed. This position is responsible for reviewing policy operational changes and making recommendations to the Director of Risk Management and Patient Safety. This position does not directly manage any other caregivers.
ESSENTIAL FUNCTIONS AND DUTIES:
Maintains oversight of the Safety Alert System (SAS) reporting and response process, including severity rating and evaluation of all safety events, the investigation and identification of risk exposures; risk analysis using selected tools, RCA and apparent cause analysis; risk evaluation and monitoring using algorithms and review criteria; and risk treatment including establishing a potential claim file, working with medical malpractice and general liability carriers follow-up and performance of trending analysis for assigned facilities/service areas. Stops the line to mitigate potential patient safety conditions and maximize learning.
Facilitates cross-functional/site process improvement endeavors to support elimination of errors and other factors that contribute to potential or actual adverse patient outcomes.
Uses trends obtained from safety reports, patient safety rounds and other sources of patient safety data to focus on areas for further analysis, risk management, patient safety, and loss prevention activities.
Identifies potential and actual sentinel events or serious safety events with prompt notification and debrief to key stakeholders including senior leadership, and site administration for conducting investigation of event and root cause analysis.
Leads investigation for a Sentinel or serious safety event investigation and analysis. Leads RCA process and failure effect mode analysis (FMEA) to identify opportunities for system and process improvements. Supports the preparation of documents for reporting serious harm events to State and accreditation agencies.
Develops and provides education/communication to leadership, physicians, colleagues and other stakeholders on patient safety needs, initiatives, activities, and for shared learning. Participates in assigned Medical Staff and Hospital Quality and Safety Committees.
Proactively performs surveillance and evaluates patient safety risks in a uniform and consistent manner for assigned facilities, service areas and affiliated joint ventures.
Collaborates with internal legal counsel to support their investigation and defense of claims against the Health System. Provides external defense attorneys requested information to prepare for pending litigation and/or claim review. Prepares and submits PCE (potentially compensable event) reports for potential claims.
Provides caregiver and provider guidance and support for patient care issues with patient safety/risk or legal implications, i.e. consent issues. Works in collaboration with the compliance department for potential EMTALA and HIPAA violations.
Provides guidance to leadership as to acceptable bill adjustments under CMS stipulations. Initiates bill adjustments to mitigate potential claims in collaboration with claims management and in congruence with policy and CMS limitations.
Reviews and supports the completeness of patient complaint and grievance response letters.
Provides guidance to caregivers and providers for disclosure of unanticipated events to patient and their families.
Proactively conducts failure mode and effect analysis for high risk or problematic care processes across System facilities and joint venture businesses, at minimum as required by The Joint Commission for continued accreditation.

EDUCATION:
Required: As required by license.
Preferred: BSN and/ or advanced training in oncology, chemotherapy or similar.
LICENSURE/CERTIFICATION/REGISTRATION:
Required: Current Oregon RN license. BLS/CPR.
Preferred: Oncology Nurse Society certified (OCN). Chemotherapy/Biotherapy certification required within one (1) year of hire.
EXPERIENCE:
Required: One (1) year of clinical experience in intravenous nursing or equivalent background in an oncology, medical-surgical, critical care, or home infusion setting. Must be profiecient in the care, maintenance, and accessing of central lines.
Preferred: Two (2) years of oncology focused intravenous nursing including the administration of chemotherapy, biotherapy, immunotherapy, and blood products.
PERSONAL PROTECTIVE EQUIPMENT
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
ADDITIONAL POSITION INFORMATION:

Proficient in insertion of peripheral IV's, accessing, care and maintain of port- a- Cath's and PICC lines.
PHYSICAL REQUIREMENTS:
Continually (75% or more): Standing and walking, keyboard operation, use of clear and audible speaking voice and the ability to hear normal speech level.
Frequently (50%): Sitting, lifting/carrying/pushing or pulling 1-25 pounds. The use and operation of a motor vehicle for Home Health and Wound Caregivers.
Occasionally (25%): Bending, stooping/kneeling/crouching, climbing ladder/step-stool (varies by area), reaching overhead, lifting/carrying/pushing or pulling 25-50 pounds, grasping/squeezing, ability to hear whispered speech level.
Rarely (10%): Climbing stairs.
