We are modeled after Concierge Medicine and go above and beyond to provide the highest level of customer service and care to our patients. The member's experience at HealthWorks is extremely convenient and accessible, with our locations being onsite or near site to the employer, and well dispersed throughout the county. HealthWorks providers and clinical support staff provide relationship-based care and are able to spend quality time with members. Our network of primary care practices is growing and we currently have 10 locations; opening 2-4 new offices a year. This position is for a new practice opening at Manheim Township School District.
POSITION SUMMARY: To provide nursing services to patients at a provider's office. Patient care includes care to patients in following age group: pediatric (infant to 12 years), adolescent (13 to 17 years), adult (18 to 64 years) and geriatric (over 64 years).
ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:
Provides clinical advice by phone/electronic correspondence based on clinical judgments and/or triage protocols in accordance with regulatory requirements and standards of practice. Consults with provider as necessary.
Provides patient specific education including but not limited to preventive care, chronic disease management, acute diagnosis, medications and other areas pertinent to patient outcomes.
Prepares patient for visit by using appropriate rooming standards tool.
Administers injections and medications as prescribed by the provider and in accordance with department protocol, regulatory requirements and standards of practice.
Prepares and assists providers with various procedures, minor surgeries and other exams as designated by the provider.
Performs Point of Care Testing, including but not limited to hearing, flu, urine and strep tests.
Collects blood or other laboratory specimens and prepares them for further testing.
Authorizes prescription refills as instructed by the provider.
Assists with test tracking and monitoring overdue results.
Removes sutures and changes dressings on wounds as necessary.
Assures exam rooms are adequately supplied and cleaned after each patient visit. Maintains and cleans medical instruments and equipment after each procedure.
Assists provider and patient in reviewing and revising care plans and assesses patient progress. Follows up with patient between visits as necessary.
Assists with proactive outreach for patients requiring preventive and chronic disease services.
Performs care coordination activities among specialists and transitions of care to obtain all necessary patient information.
Assists in coordination of referrals, education and community resources.
Actively participates in performance improvement initiatives to improve overall patient experience and quality of care.
Participates in daily huddles with the provider.
Serves as a patient experience advocate by being a positive influence at all times when interacting with patients, visitors and colleagues. Communicates with all patients, visitors and colleagues in a professional manner while keeping customer service a top priority.
Handles all patient information in a confidential manner according to Lancaster General Health privacy and confidentiality policies and procedures.
SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:
Performs various front office responsibilities as necessary.
May require some regional travel - valid driver's license is required if traveling.
Participates in the on-call responsibilities for this position as required.

JOB REQUIREMENTS
MINIMUM REQUIRED QUALIFICATIONS:
Completion of a LPN or a RN educational program
Current licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN), issued by the PA State Board of Nursing
AHA CPR/BLS certification required within 30 days of hire.
This role requires occasional regional travel. Ability to travel to multiple sites is required.
PREFERRED QUALIFICATIONS:
One (1) to Two (2) years of nursing experience in a primary or specialty care physician's office.
Prior triage, emergency room, cardiac care, intensive care or other related specialty nursing experience.

CCRM is currently comprised of eleven locations spanning the U.S. and Canada with growth planned for continued network development on a domestic and international basis.
ESSENTIAL FUNCTIONS:
• Perform direct patient care as scheduled in all areas, including but not limited to, preparation of supplies/equipment necessary for the surgical patient, and perform cleaning and sterilization of instruments in the surgical suite.
• Assist in overseeing the daily operations in the Surgery Center, be a resource for staff, physicians, and other disciplines.
• Assist in the daily oversight and responsibility for all surgery center nurses, assistants, and surgery scheduler.
• Assist with interviewing, hiring, counseling, disciplining and terminating employees as necessary.
• Assist with flex/budget staff in accordance with patient volume; assuring sufficient staff are available for patient care during the surgery center’s hours of operation.
• Assist with the purchasing of all supplies, medication, and surgical equipment.
• Assist with ensuring that all surgical equipment is in working order and available for patient care at all times. Oversee the demonstration of new equipment for possible purchase for patient care use.
• Assist with development, maintenance, updates, and implementation of all surgery center policies and procedures to assure high quality patient care is provided to all surgery center patients.
• Assures compliance with all AAAASF or AAAHC as well as the State Department of Health standards related to the surgery center and administration of anesthesia in the surgical setting. Complete the peer review process with the credentialing agency.
• Complete credentialing process with all CCRM surgeons and anesthesiologists.
• Assist with the privileging, credentialing and ongoing re-credentialing of all medical staff members, physicians, and other non-employee patient care providers in the surgery center. Alerts medical director to all situations requiring follow-up of these personnel by the medical director. Alerts medical director to a possible impaired provider immediately upon assessment of an unsafe provider.
• Assist with coordinating ongoing peer review of medical staff members, physicians, and other non-employee patient care providers in the surgery center. May serve as the peer review committee chairperson to the governing board, as directed.
• Assist with ensuring compliance with the Department of Health’s standards for surgery center licensing.
• Assist with coordinating the infection control program for the surgery center setting, and assists the governing board to ensure compliance for the center’s overall infection control. May serve as the infection control nurse for the surgery center, as directed.
• Assist with coordination of educational offerings/training of employees including orientation of new staff.
• Assist with coordination ongoing Basic Life Support and Advanced Cardiac Life Support training for all clinic staff required to maintain these certifications.
• Create a focus of quality management and performance improvement. Participate in QI program, committees, and research as appropriate. Coordinates and oversees ongoing QI studies for all surgery center staff.
• Coordinates chart review prior to procedure to assure all surgery center and IVF laboratory consent forms are complete and signed.
• Inquire with patient if he/she has advanced directives and input the forms into the patient’s EMR if forms have been completed.
• Handle emergent situations calmly and efficiently, responding quickly and demonstrating knowledge of emergency procedures and equipment.
• Assist with ensuring team has annual OSHA/HIPAA training.
• Responsible to report and participate in quality program to practice manager and CCRM management team.
• Assists with ensuring current contract are in place with anesthesia team is current; credentials new anesthesia team as necessary.
• Report incidents or adverse incidents to practice manager and CCRM management team. Report any concerns about impaired physician, nurse or employee to practice manager and CCRM management.
• Promote a positive work environment, be accountable for personal conduct and actively participate in staff meetings and unit problem solving.
• Demonstrate organizational skills that provide efficient patient care and the smooth operation of the unit on a daily basis.

REQUIRED QUALIFICATIONS:
o Associate Degree, Diploma, and/or Bachelor of Science in Nursing from an accredited school of nursing.
o Current State Registered Nurse license.
o Current BCLS and ACLS certifications.
o 2+ years of previous experience related to operating room nursing and/or PACU, and/or critical care experience.
o 3+ years nursing experience in other RN roles.
o 2+ years leadership experience.
