ESSENTIAL JOB FUNCTIONS

Conducts the medical assessment of each individual referred to the program and collaborates with an Intake staff member to determine appropriate placement in the community
Participates in the development and implementation of service plans and supervises, in conjunction with the physicians, the medical components of the plan
Reviews documents and revises all nursing/nursing related care plans on a regular basis
Ensures medical records and reports are current and complete in accordance with regulatory and corporate standards, including medication administration records and physician's orders
Maintains contact with primary care physicians and specialists, and, when directed, attends doctor visits with mentor and/or individual receiving services.
Promotes health and wellness for individuals and ensures nursing actions and steps are put in place for prevention
Administers medications according to established guidelines
Provides documentation for all medications given and reviews medication administration records for compliance
Oversees the management of high-risk medical conditions and private duty nursing services
Develops training flow charts specific to each individual receiving services, provides and documents medical skills training to clients, mentors, respite providers, staff, and where appropriate, the biological family
Presents medical skills components at Pre-service Orientation
Participates as an Individual Health Plan (IHP) or Plan of Care (POC) team member,
Evaluates the medical needs of each individual for initial IHP/POC,
Ensures IHP/POC goals and objectives are medically indicated, and
Generates ongoing health care protocols for the specific medical conditions outlined in the IHP/POC
Provides consultation and technical assistance to staff and mentors regarding home care issues along with operation of durable medical equipment.
May be required to coordinate, organize and/or assist with household activities such as light housekeeping and meal preparation; transport individuals receiving services to planned and/or necessary activities and appointments
Participates in on call system as needed
May actively participate in IDT meetings, Human Rights Committee, Health and Safety Committee and Administration meetings
Performs other related duties and activities as required

Functions as part of the hemodialysis health care team in providing safe and effective dialysis therapy for assigned patients under the direct supervision of a registered nurse. **DUTIES / ACTIVITIES** :

+ Monitor and document dialysis treatment parameters on dialysis flow sheets. Document other information related to the care of the patient in the individual patient record.
+ Responsible for reporting any significant information, change in patient condition, or equipment problems to the supervisor.
+ Assist in developing and following the teaching plan to educate the patient and family regarding end stage renal disease, dialysis therapy, diet and medications.
+ Assist with all emergency operational procedures.
+ Administer intravenous medication in accordance with physician orders and state nurse practice laws.
+ Perform and document pre, interim, and post treatment review of patient condition.
+ Initiate basic CPR measures in the event of cardiac and/or pulmonary arrest, and respond to emergency situations related to dialysis treatment.
**Technical:**
+ Safely operate all dialysis related equipment according to the proper procedures.
+ Provide minor troubleshooting when necessary.
+ Ensure a clean, safe and sanitary environment in the dialysis facility treatment area.
+ Ensure all blood spills are immediately addressed according to FMCNA Bloodborne Pathogen Control Policies.
+ Operate all emergency equipment effectively in accordance to the standard operation procedures.
**Staff Related:**
+ Participate in staff meetings as scheduled.
+ Assist in short term and long term patient care plan meetings.
+ Acquire information and knowledge in current practice related to dialysis principals and technique by participating in scheduled in-service classes.
+ Act as a resource person for other staff members. Assist with the coordination of the care plan with Physician, Dietitian and Social Worker.
+ Other duties as assigned.
**PHYSICAL DEMANDS AND WORKING CONDITIONS** **:**

Essential Functions:
Manage and monitor the delivery of services to patients ensuring compliance in the performance of all aspects of youth care in accordance with Company policy and procedures, State and Federal guidelines.
Prepare youth for examinations and record the medical intake, take vital signs, height and weight. Initiate routine lab tests after triage of a youth such as pregnancy tests.
Assist the medical providers staff with examination procedures when is needed such as patient screening and triage.

Update youth charts in regards to allergies, medication use, and immunization history at each visit.
Administer immunizations under standing orders from provider.
Educate and counsel youth concerning their disease, treatment, and prevention of disease.
Answer youth's questions after consultation with the provider or as per established procedures in that regard (to include but not limited to: health questions, medication refills, and lab results.)
Continually assess for areas of improvement and develop plans and evaluation criteria for such.
Oversee and direct all youth care staff to ensure adequate monitoring. Ensure delivery of safe and efficient quality care. Assist in health education classes when necessary.
Obtain, compile, and maintain medical files on each intake. Ensure proper daily documentation and maintain hard copy of required documents and medical record.
Schedule and comply with required medical appointments and timelines. Respond and comply to individual emergency medical needs immediately.
Maintain and review for accurate and complete list of medical supplies, prescribed medications, and over-the-counter medication. Order medical supplies, and medication when necessary.
Collect, maintain, and submit weekly, monthly, quarterly, and annual reports and audits to the supervisors.
Facilitate and conduct self-administration of medication and side effects training to employees as required.
Provide a fail-safe plan with medication count, label accuracy, and self-administration of medication, including over-the-counter drugs.
Schedule and provide transportation when is needed to clients on medical appointments with assistance from other staff when necessary.
Submit, review and follow up all the Significant Incidents Report regarding Medical Issues.
Participate in Quality Improvement as required.
Other Functions:
Able to react to change productively and handle other essential tasks as assigned.
Develop and maintain effective communication and working relationships with staff, co-workers, physicians, and youth.

WORK ENVIRONMENT
Nurse Corps Officers may serve at any one of more than 250 Navy and medical facilities around the globe, from Hawaii to Japan, Germany to Guam and Washington, D.C., to Washington state.
