** Summary:**
This position is a part time, night shift position with weekend and holiday rotation. Must hold LPN license in state of Ohio. This position provides nursing care to patients and families along the health illness continuum in diverse health care settings while collaborating with the health care team. The LPN is accountable for the practice of nursing as defined by the Ohio Board of Nursing. The LPN delegates nursing tasks to unlicensed assistive personnel (UAPs) using the criteria written in the OhioNurse Practice Act. This position serves as a resource for other associates. May communicate instructions, do minor scheduling, and inspect work. Does not hire, fire, discipline, promote, demote, transfer, or appraise, but may be consulted by higher management in these areas. This job has no supervisory responsibilities.
**Responsibilities And Duties:**
25% Performs initial, ongoing and functional health status assessment as applicable to the population and/or individual. Collects and documents initial and ongoing subjective and objective data from patients, significant others, and communities. Reviews data obtained by other members of health care team and incorporates into assessment. Identifies and communicates assessment findings, both initial and ongoing, to appropriate people via referrals, rounds, huddles, reports and care conference, etc. Uses assessment data to identify pertinent nursing diagnoses and collaborative problems. Collaborates assessment findings with providers and members of healthcare team as appropriate. Uses scientific knowledge, Critical thinking skills, technical skills, and effective interpersonal and teaching skills to provide nursing care.
25% Based on nursing diagnoses and collaborative problems, documents planned nursing interventions to achieve outcomes appropriate to patient needs including: age, size, and development stage. Initiates and/or coordinates care plans and individualizes critical paths/medical management plans. Promotes cost effective care by minimizing fragmentation, maximizing coordination and facilitating patient/family through the health care system. Accountable for quality improvement and clinical outcomes and any other duties assigned. Carries out and documents interventions to assist patient/community to achieve expected outcomes. Uses scientific knowledge, critical thinking, technical skills, and effective interpersonal skills to provide direct patient care as needed.
20% Evaluates and documents response to nursing interventions and achievement of outcomes at appropriately determined intervals; as part of a multidisciplinary team, revises plan of care based on evaluative data.
20% Delegates tasks in accordance with specific criteria and state statutes. Reviews work performed by those to whom it is delegated. Coordinates continuity of care as indicated by patient/family/community needs and resources along health/wellness continuum in collaboration with other members of the health care team. Participates in development of self, peers and others, e.g. MAs, nursing students. Coaches other employees and provides feedback to others, as appropriate.
10% Actively participates in process improvement activities to achieve targeted measures of clinical quality, customer satisfaction, and financial performance. Acts as patient advocate. Participates and shares information from committees, research and self-development programs. Communicates and participates appropriately in staff meetings, and collaborates with providers. Manages resources including personnel, supplies and equipment to maintain a safe and efficient environment.

What You'll Do:
As a Registered Nurse, you'll play a meaningful role in providing nursing services to youth residing in an intensive residential setting. Essential Duties and Responsibilities:
Escort patients to the exam rooms and document their history, medications, and visit information prior to exam with provider into the Electronic Medical Record (EMR).
Scribing information for the dermatologist.
Assist provider to maintain a good workflow and meet expectations of patients in relation to being on time.
Providing post op wound care to patients.
Drawing blood and suture removal.
Assisting providers with clinical procedures and surgeries.
Responsible for patient education including handing out educational pamphlets, giving verbal instructions, explaining specialty pharmacy information, medications, products, procedures, post-operative wound care instructions, and answering patient questions.
Assist with procedures and administer injections/local anesthesia per provider instructions.
Answering phone calls, voicemails, telephone triage, and patient clinical questions.
Review medical history and counsel patients regarding their clinical status and need for further evaluation or treatment.
Prepares patients for examination and treatment by escorting them from the waiting area to the exam room. Takes patient's history (HPI), determines chief complaint, verifies their medical and surgical histories, current medications, etc., and takes vital signs if applicable.
Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition.
Modify patient treatment plans as indicated by patients' responses and conditions.
Prepare patients for and assist with examinations or treatments.
Prior to surgery, reviews and completes required surgical documents prior to appointment with recent test results and correspondence and determines needed services per practice protocol.
Prior to patient visit, confers with Medical Provider regarding any incomplete patient tests/consults or other incomplete orders.
Responds to patient communications. Contacts patients regarding test results and schedules further surgical treatment(s).
Gives injections, assists with examinations and office procedures as needed; and provides other patient care services as directed by the Medical Providers.
Performs suture removals as directed by the Medical Providers.
Effectively uses EMR system to document patient records using proper English grammar, spelling, and syntax.
Verifies all patient/specimen information corresponds with tissue specimen(s). Records tissue specimen(s) and enters them into log.
Assist with exams, biopsies, surgeries, and excisions. Assist with bringing up pictures on iPad and drawing blood.
Assisting with Mohs (previous experience not required)

Must have 2 years or more of recent clinical experience

Graduate of accredited nursing program
Active state RN license and in good standing

About the Unit:
The Cardiovascular Progressive Care Unit (CVPCU) at Mount Carmel St. Ann's has 30 beds, with space to accommodate between 6-9 progressive care patients. The Registered Nurse could expect to see patients related to NSTEMI and post STEMI/cardiac arrest, post-cardiac cath and acute coronary syndrome, AAA repair, atrial fibrillation, amputations, and femoral-popliteal bypass. The RN will also care for patients in diabetic ketoacidosis and acute respiratory failure on Bipap or AirVo. This unit offers a strong, welcoming teamwork environment. An RN would have many amazing opportunities to learn and grow their nursing knowledge with both intermediate and progressive care patient populations. About the job:
The Registered Nurse (RN), within the scope of the OhioNurse Practice Act, will provide care in an atmosphere sensitive to each person's physical, emotional, social and spiritual needs.
