You will be responsible for: Providing interim clinical coverage at Skilled Nursing Facilities (SNFs) nationwide for providers on extended leave. Train and educate new NPs and PAs on the principles and procedures of physiatry, including joint and trigger point injections, spasticity treatments, and various pain management strategies. Facilitate successful integration by introducing new providers to facility administrators and primary care teams, helping them establish excellent working relationships. Provide guidance on developing efficient patient rounding schedules and streamlined charting processes as well as the skills needed to perform treatments, even to providers inexperienced in physiatry and new graduates. Travel extensively (48-50 weeks per year, Monday through Friday; longer only if the interim clinical coverage requires a longer stay - but rarely longer than 2 weeks at most) to various skilled nursing facilities nationwide. We currently operate in 10 states with ambitions to serve all 50 states within the next 5 years. You will be crucial in achieving this goal. Qualifications To be considered for this exciting role, you must have: A current Nurse Practitioner or Physician Assistant certification. A valid medical and DEA license.     Reporting to the Clinical Supervisor, Care Management, the Care Management Nurse provides clinical assessment, nursing interventions, and coordinated care planning for SFHP members with complex and chronic health needs in multiple programs. The nurse also delivers chronic-condition evaluation, reconciles medications, and supports members in navigating care across multiple settings. Additionally, it necessitates consistent communication with several team members. These team members include members, caregivers, CMCC colleagues, physicians, hospitals, and community partners. The nurse is an important contributor to interdisciplinary case reviews and collaborates with Utilization Management, Pharmacy, Behavioral Health, and SFHP's Medical Directors to ensure members receive appropriate, and coordinated services.
