Supervisor Title: Director of Nursing
Location: Afton, WY
Employment Status: Full-time, Part-time, & PRN openings are available after completion of the class.
Compensation: $14.40 throughout the course of the training. Once licensed as a CNA, compensation will increase to a minimum of $16.36 per hour.

Summary
While upholding the Star Valley Health mission, vision and values this key position is responsible to provide quality, compassionate resident care through performing delegated tasks within the realm of the non-certified nursing assistant training. Tasks that are delegated and resident needs will be addressed in an efficient competent manner. This position will comply with all federal, state and local regulations. This is an at will position.
Job Duties
Complete all delegated tasks within the scope of your training and practice, according to state and federal regulations and facility policies and procedures.
Observe residents needs and behavior and respond accordingly to the resident's rights and application.
Report on needs of all patients under assigned care to charge nurse and CNA.
Promotes a cooperative and congenial working environment with residents, colleagues and guests of the care center.
Attends, or is responsible for information presented, in-service meetings and training.
Maintains equipment and environment to safety standards.
Utilizes computer skills in the documentation of resident cares.
Performs other duties as assigned.

Follows plan of care as noted on daily mini-care-plan sheet and gives input to nurses on any changes that should be made to further individualize plan of care. Prepares residents for meals, offering choices, distributing food and assisting residents as needed. Provides a meaningful dining experience for residents by engaging them in conversation and creating a positive dining environment. Knows resident diet and offers snacks and appropriate hydration to residents. Consistently documents meal and fluid intake.
Resident Safety
Makes rounds to anticipate resident needs. Promptly responds to call lights and all other alarms. Assures for resident safety including: fall alarm in place and working; call light within reach; staying with resident as required; using gait belt.
Completes resident positioning, lifting, turning, and ambulating according to standards of care and resident's plan of care using proper equipment and tools to facilitate safe care for both the resident and employee. Collaborates in the implementation of rehab goals such as ROJM, ambulation, positioning, and turning.
Consistently follows appropriate infection control practices including hand washing, linen handling, glove use, catheter care, and isolation procedures.
Assists in maintaining a safe and clean environment for residents by reporting malfunctioning equipment, environmental hazards, cleans equipment, keeps hallways clear, and equipment in good working order. Implements fire and disaster plan correctly.
Keeps residents' rooms neat, safe, and clean. Handles resident's personal belongings properly including prosthetic devices, hearing aides, dentures, glasses, and clothes.
Checks oxygen for correct flow and makes certain that oxygen is available to meet resident needs.
Collaborative Communication and Teamwork
Communicates to nursing staff any changes in resident condition including but not limited to mental status, mobility, positioning needs, skin condition, dietary intake, participation in care.
Consistently communicates effectively with team members to effectively address resident needs and implement the plan of care. This includes communication with members of the interdisciplinary team on all shifts.
Consistently and accurately completes required documentation for dietary intake, I and O, and aide duties.
Other Duties

Prepares residents for participation in activities and participates with the resident in the LC activity program.
Performs fundamentals of customer service such as answering phones, paging, and relaying messages.

Summary: While upholding the Star Valley Health mission, vision, and values, this key position is responsible for assisting therapists administer care to residents at the care center. This key position helps residents, maintain or regain mobility and/or manage pain through the use of exercise, stretches, massages, and other physical therapy techniques under the oversight of the restorative nurse and under the direction of the physical therapist in accordance with local, state and federal regulations, standards, state physical therapy practice act, and established facility policies and procedures to provide appropriate care and services to residents. Job Duties:
Performs restorative nursing services in accordance with care plans and facility policies and procedures. These services may include activities related to range of motion, splint/brace assistance, bed mobility, transfers/walking, dressing/grooming, eating/swallowing, amputation/ prosthesis care, and communication.
Provides therapy services as directed and in accordance with orders, and delivers restorative nursing services per the individualized care plan to support residents with physical, neurological, or functional limitations.
Is familiar with assistive devices that might be used during treatment and ambulatory assistive devices, like canes and walkers that some residents may require for walking.
Monitors the resident's environment to identify any needs or barriers that might negatively impact the resident's therapy and recovery. Observes and makes recommendations for the necessary accommodations to the restorative nurse and physical therapist and enacts those approved recommendations.
Treats residents with a variety of physical therapy techniques including but not limited to massaging sore muscles and pushing injured ones into deeper stretches.
Provides resident information needed for the resident assessment instrument according to facility policy such as but not limited to functional abilities, functional status, range of motion, balance during transitions and walking, physical therapy minutes and distinct calendar days, etc. during the defined observation period(s) as assigned by the physical therapist, restorative nurse, or MDS coordinator.
Adapt to the changing needs of the patients, ensuring high-quality care and efficient operations.
Assists with weighing residents according to facility policy, and records weight in designated locations. Documents and reports weights appropriately.
Attends care-plan meetings of assigned residents as able restorative nurse or physical therapist guidance.
Participates in QAPI or facility assessment activities as needed, such as carrying out duties assigned as part of a performance improvement committee.
Uses proper lifting and body mechanics while delivering care to residents.
Treats all residents with dignity and respect. Promotes and protects all residents' rights.
