This position, on a regular basis, provides direct health care services to persons in the care or custody of the state and has unsupervised access to controlled substances. This position provides consultative services for a specialized program of psychiatric services at WRPH for the patients, including training other health care professionals and support staff in the medical review of cases. The Emergency Department Chief is a medical officer at the FVHCS and is responsible for the quality of care and the utilization of health care resources at the facility and within the community. The Fargo VA Healthcare System Emergency Department provides care to approximately 9500 patients per year. Patients seen principally have a wide range of medical problems with occasional minor trauma. Two years of emergency department management experience is required. A primary objective of the Chief of the Emergency Department is to mentor and lead competent, satisfied staff dedicated to providing high quality patient-centered care at the Fargo VA Health Care System (FVAHCS.) The incumbent is responsible for the direct management and oversight of clinical and administrative operations within the Fargo VA Healthcare System Emergency Department. These duties include supervision (direct or indirect) of physicians, NPs and PAs in the Emergency Department, coordination of work schedules and responsibility for clinical performance measures. The incumbent works closely with other leaders within the ICC and facility to ensure optimization of resources in the care delivery model. Dingell VA Medical Center in Detroit, MI. The General Radiologist will be involved in all aspects of pulmonary and cardiac imaging, including lung cancer screening and diagnosis and evaluation of airway diseases, benign and malignant conditions of the lungs and pleura, coronary artery diseases and congenital or acquired diseases of the heart. ABR and fellowship training in cardiothoracic imaging are preferred. This is not a virtual position. The General Radiologist will be working with a great team of radiologists and technologists, support staff, and will enjoy the following new, top-of-the-line, medical imaging systems being installed currently at the Detroit VA: a 3T PET-MRI, a 3T wide-bore MRI, a dual source multidetector CT, XR systems, 2 Fluoroscopy systems, and an Angiography suite. Oncologic Radiology and Cardiovascular Radiology are key areas of interest, and interest in research and development is strongly encouraged, but not mandatory. Interventional Radiology skills and involvement is encouraged but not mandatory. The Detroit VA is affiliated with the Wayne State University School of Medicine, and is a key member of the Detroit Medical Center complex. Adhering to the standards of the American College of Radiology, including but not limited to the following. Oversight of quality and safety: * Review exam requests for appropriateness. * Prescribe exam protocols and modify as needed for specific clinical indications. * Provide oversight of technical image quality with feedback to technologists. * Provide clinical oversight of medication administration, including contrast, preps, and other medications relevant to the Imaging Service. Image Interpretation and Consultation: * Interpret imaging exams including relevant exam comparison and correlation with clinical information in CPRS. * Support patient care services with reports timely to the care being delivered with the method of communication appropriate to the level of clinical importance or acuity. * Interprets X-ray examinations, Ultrasound, CT, Fluoroscopy, and MRI exams. * Interpret Nuclear Medicine exams and may or may not performs some interventional procedures (imaging-guided biopsies and drainages, thoracentesis etc,...) The gastroenterologist is responsible for seeing patients in consultation in both inpatient and outpatient settings. Patients seen in GI Consult Clinic are evaluated and treated by the gastroenterologist for disorders such as GERD/dyspepsia, dysphagia/non-cardiac chest pain, irritable bowel syndrome, pancreatitis/pancreatic insufficiency, chronic liver disease (not viral hepatitis), chronic diarrhea, constipation, and inflammatory bowel disease. The gastroenterologist is responsible to provide endoscopies and procedures to include colonoscopy, sigmoidoscopy, and esophagogastroduodenoscopy (EGD). Performance of endoscopic retrograde cholangiopancreatography (ERCP) is desirable but not required. Other duties include responding to patient telephone calls, informing patients of test/lab results, renewing medication prescriptions, giving telephone or e-mail advice to other providers, and administrative duties, if any. The incumbent is responsible for entering accurate, complete, timely progress notes for each episode of care in the patient's electronic record. This is accomplished by utilizing the Electronic Medical Record System (EMR). The incumbent is responsible for the proper use, maintenance, and calibration of specialized equipment used in the assessment and care of patients. The incumbent adheres to safety, health, and infection control standards, regulations and work practices, as well as ergonomic standards. He/she reports any unsafe conditions to supervisor and/or appropriate officials. The incumbent assists in orientation of new staff, and will provide patient and family education, including informed consent. The incumbent maintains supplies, equipment and clinical areas to optimize safety and efficiency. He/she demonstrates knowledge and maintains standards in the use and reprocessing of Reusable Medical Equipment (RME). Hospice and Palliative physicians will be responsible for delivery of care to all patients enrolled in VA Medical Center Fargo healthcare system. The provider will manage scheduled and unscheduled patients in the outpatient clinic setting and provide services to patients in hospital and CLC. The providers in the outpatient setting are typically assigned a panel of patients to manage. Panel size currently is determined by utilizing VHA PCMM software. Panel size is determined at the National level, through a complex formula which extracts local information regarding the acuity of assigned patients, available support staff, and the number of exam rooms. Each provider's panel size is specific to their practice and setting. Alternative or collateral duties may include management of a subsection of hospice and palliative care patients, the subsection may be defined either by a disease status (e.g. spinal cord injury patients), or in the CLC or inpatient setting). Typically, these duties are designated at the time of joining but occasionally during employment, participation in special projects pertinent to assigned section may be necessary as the projects become available through strategic or other initiatives. Applying for and keeping inpatient privileges active is optional for primary care providers. When on inpatient duty, the provider is expected to participate in discharge planning and other interdisciplinary patient care plan activities. They must participate in sign-in and out rounds daily. During this time information regarding patient status should be discussed with the oncoming physician/MOD and ensure proper hand off at the end of the shift. Physicians may be assigned duty as a supervising or collaborating physician for a mid-level provider. Physicians are expected to provide clinical oversight and review. Supervising or collaborating responsibilities and performance will be noted in the physician's performance evaluation. Most providers will need to act as surrogates for the patients of their colleagues, when one or more of their colleagues are on leave. Typical work hours are Monday to Friday 8-4.30 PM. Alternative work hours, if desired, should be agreed upon at the time of joining. Each full-time provider is required to work a minimum of 80 hrs per pay period (every two weeks) unless on leave status. All physicians need to follow departmental and VHA policies for leave and attendance and will work within Title 38 appointment guidelines. Currently VHA guidelines do not permit comp time or overtime pay for physicians. However, physicians who are asked to work outside their normal assigned tour of duty may request administrative leave for rest and relaxation. Leaves should be requested in as much advance as possible to minimize patient cancellations. All unscheduled absences must be properly communicated, and patients should be rescheduled within a time frame as specified in the department's policies. Timely documentation is necessary in all areas in accordance with departmental policy. All administrative work should be completed in a timely fashion. Clinical reminders should be addressed as medically appropriate. Quality of care should be able to always meet professional standards. All physicians will always work within their privileges, unless necessitated by an emergency.
