This position will receive a short-staffing temporary pay differential of (4%) from July 1st 2025 - June 30th 2026.

CO State Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of qualify and dignity for every resident. At the Colorado State Veterans Community Living Center at Rifle, our staff is known for showing love and compassion to our residents. Giving residents a hug, holding their hand or sitting next to them on them on the arm of a chair are just a few of the ways we give care that comes from the heart.
Provides basic hands-on patient/resident care in a safe, clean, infection controlled environment by bathing, dressing, grooming, hygiene, skin care, range of motion, positioning, ambulating, transferring, toileting programs, feeding programs, transporting, performing basic assessments (vital signs, blood pressures, weights, etc.), and charting/documenting all of the above.
Primary Job Duties:
Daily Nursing Care
* Performs basic nursing care: bathing, feeding, dressing, transferring, ambulating, positioning, lifting, range of motion, toileting, skin care, and interacting socially to meet residents' basic needs, and FMP Programs.
* Assistnursing staff in resident safety, importance of Park and Walk programs, resident transfer and mobility, assistive devices, mobility aids, equipment and gait belts. Perform CPR as needed.
Safety
* Safety of residents and others by use of correct lifting techniques, monitoring the environment and resident whereabouts/activities, use of safety devices such as lifts, etc.
* Performs basic resident assessments such as weights, bowel and bladder incontinence, intake and outputs, vital signs, and records the same.
* Records hall sheet data each shift.
* Implements proper infection control practices via the use of standard precautions.
* Equipment checks, equipment maintenance, and placement of equipment, i.e. mattresses on beds, foot cradles, etc.
Communication
* Communicates with others to ensure proper resident care and work completion.
* Inform RN's and LPN's of findings and changes in resident condition.
* Maintains therapeutic milieu through appropriate interpersonal skill use, monitoring of environments and assisting residents to maintain rooms and possessions.
* Maintains resident care and equipment such as cleaning oxygen concentrators, filling portable oxygen tank, maintain/disinfect tubs and showers, changing lift batteries, etc.
* Participate in restorative meeting/GAPA meetings.
Transports
* Transports residents for medical appointments, etc. in state vehicles when the need arises.
* Attends educational training and staff meetings to maintain current on techniques and policies.
* Provide adequate inter-shift communication.
Minimum Qualifications
* A high school diploma or GED ~and~ At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA.
Education and Work Experience Substitutions:
* A valid nurse aide certification (CNA) from the Colorado Department of Regulatory Agencies for the high school diploma or GED ~or~ A valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact for the nurse aide certification (CNA) can be substituted for the education and work experience

* This position requires travel to conferences and industry specific trainings/meetings.

This PART-TIME position is located in BUENA VISTA and provides medical assistance to inmates housed in the Buena Vista Correctional Complex.
This position is responsible for, but not limited to, the following:
* In a clinical setting, assist patients with activities of daily living to include bathing, mouth care, dressing, toileting and clothing changes when incontinence occurs, ambulation and feeding;
* Obtains and records vital signs for review by staff nurse;
* Provide and assist with patient transfers and turning in bed;

* Talk with patients to keep oriented to person, place, and time;

* Successfully pass all modules of the 6 week Colorado Department of Corrections Basic Training, to include English reading comprehension, relevant competency assessments and written/practical exams in Defensive Tactics;
* Possess the physical ability to successfully complete Defensive Tactics;
* Lift arms above head and kick as high as own waist;
* Stabilize another person to accomplish a controlled take down;
* Use arms, palms of hands, shins, and feet to deliver blows;
* Withstand impact (slow speed or with a safety bag) on own body from strikes/blows;
* Swing a baton in a striking technique;
* Rotate body 90 degrees with feet planted for striking with foot or using a defensive tactic;
* Get down on one or both knees and up again with multiple repetitions;
* Will be exposed to Oleoresin Capsicum during Basic Training and will be exposed while on assignment;
* Must be able to see and hear in order to observe, address and respond to potential breaches in safety and security;

* Demonstrate body agility, strength of all four limbs and the ability to walk and climb stairs consistently;

* Work with people of the opposite sex, diverse racial ethnic groups, mentally ill and emotionally disturbed offenders;

* Work in an environment with fluctuating noise and temperature (hot and cold) which may include working outdoors in inclement weather and/or concrete floor/wall work settings;
* Willing and able to travel;
* Willing and able to work various shifts, days, afternoons, graveyards (nights) which includes a rotating shift schedule;
* TB (tuberculosis) screening is required of all new employees upon hire.

GS 4 (NA) are at the intermediate or developmental level and are closely supervised. The significant factor is that they provide care under close supervision and refer issues/questions to supervisors, higher graded NAs, or other healthcare professionals.
GS-5 (NA) function as a member of the nursing care team and assist licensed nursing staff in the care of patients receiving acute inpatient mental health care. The NA at this level also assumes a peer education/mentorship.

This role is responsible for assisting physicians with basic patient care and providing unparalleled customer service to patients while adhering to established protocols, processes and volume without ever compromising quality.
MEDICAL ASSISTANT ESSENTIAL FUNCTIONS:
Prepares patient for examination and procedures
Ensure all initial consultations, pertinent notes, lab slips, radiology and pathology reports are entered in the patient's file and available to the physician prior to patient arrival
Obtain and document patient history and chief complaint, and prepare them for the office visit by taking and documenting the patient's vitals
Provide basic wound care
Assist casting technicians as they cast, splint, or brace patients
Assist physician as needed during procedure; monitor patient in recovery area
Prepare all medication prescriptions, physical therapy orders, diagnostic testing orders and others in accordance with the physician's direction
Track incoming imaging and reports for upcoming consultations / office visits
Provide patient education with regards to office policies, medications, management of pain/injury/disease, home treatments and special diets using appropriate medical terminology as directed by the physician
Ensure all consent forms are completed, signed and filed
Respond to patient / physician / pharmacy calls calls/messages and assist with their needs
Coordinate patient care with team members
Communicate physician referral requests to referral staff for processing
Assist in obtaining any necessary insurance authorizations for the patient's treatment
Maintain supplies, equipment, prepare and clean exam/procedure rooms, stocking and sterilizing instruments ensuring patient rooms are stocked, cleaned, and presentable
Schedule surgery appointments and send reminders
Assist physician with office visit coding
Complete, send and answer questions regarding surgery paperwork
Other duties as assigned

Able to frequently stoop, lift carry and push/pull 15 pounds and occasionally up to 30 pounds
Hours of business are Monday - Friday from 8:00 a.m. to 5:00 p.m.
