Certify that case files reflect current social histories, court reports, referral letters, and correspondence, and that staff is familiar with the contents of those documents and able to present relevant information to the court. Provide current information policy & procedures to staff responsible for detention screening, detention reviews, and related court hearings, and share related information with the youth and family. Follow-up with staff to monitor the status of that request as appropriate. Case Management/ Reviews Supervision Processes Conduct a case review to certify staff completed the appropriate Department approved screening/assessment instruments to determine the risk and needs for all youth screened/supervised by the department. Ensure that prior to the development of or revisions to a case plan that staff are negotiating, implementing, and modifying the plan based on input from the youth/family, and the risk and needs identified by the Department approved risk/needs instruments are in accordance with departmental rule/policy. Certify that staff conducted face-to-face contacts in the home, school and varied community-based settings with the youth, family and community during non-traditional hours weekends and evening hours in addition to business hours. Verify that staff initiates a face-to-face contact with the youth and family following court to conduct a preliminary review of the court order and provide the youth and family with the contact information on the assigned staff or program. Maintain communication with assigned staff to confirm adherence to departmental requirements for interventions during all stages of youth and family involvement with the department e.g., meeting in the detention centers, schools, homes, and out-of-home placements, place of employment, community sites, other agencies and treatment providers and/or offices. Ensure assigned staff provide crisis intervention by assisting the youth and family in accessing crisis intervention services/resources and making necessary referrals and follow-up on actions taken. Work with assigned staff to verify personal goals/plans have been developed with the youth and ongoing support has been provided that will help youth achieve their goals and maintain skills. Confirm youth, family and/or facilities have been contacted by the assigned staff and the planning and coordination of transportation of youth and family to and from detention centers, court, school, after school programs/activities, residential programs, homes and other community-based appointments has been arranged, as needed. Conduct case reviews to ensure assigned staff discussed with youth and family projected timeframes for completion of goals, sanctions/conditions of supervision and anticipated date for termination of supervision based on progress, and as ordered by the court. Assign and monitor specialized caseloads and verify that assigned staff provide services, supervision, participate in all case related staffing, document case work in JJIS, and prepare required reports for the other agencies involved with specialized cases, and/or the court as required. Respond to case specific concerns, needs, and risks as presented by staff during case staffing related to court recommendations, supervision planning, effective responses to violations (technical & new law violations), and transition planning that is in accordance with existing departmental policies, procedures, administrative rules, and FL statutes. Enlist concerned parties in a team conferencing approach to provide input on interventions and revisions to supervision plans. Discuss with assigned staff the recommendation for handling law violations and appropriate interventions based on the Effective Response Matrix, when a youth violates the conditions of supervision, and what follow-up actions must be taken in response to a violation. Conduct staffings with assigned staff when case benchmarks occur, milestones are achieved by the youth and family, factors need to be considered prior to case recommendations/ management i.e. when youth is still securely detained a certain number of days pre-adjudication or post disposition, or beyond a timeframe identified in statute, a violation of court order or supervision has occurred, transition and exit conferences are scheduled a set number of days prior to a youth's release from a residential commitment, all court ordered requirements and conditions of supervision have been completed, handling of complex cases, crossover cases with other agencies, and human trafficking, etc. Ensure case transfer or progress summaries are completed within required timeframes as defined by administrative rule. Case reviews must include an assessment of the assigned staff preparation of transition and exit planning for a youth pending release from a residential program to ensure the plan is finalized prior to release. Actions must include referrals for post-residential services for mental health, substance abuse, education and workforce services. Provide staff, youth and the family with needed support to ensure best outcomes are achieved prior to the Department's involvement being terminated. Provide technical assistance and conduct case reviews with staff to determine appropriate case management intervention strategies. Your Specific Responsibilities: The incumbent of this position functions as the Nutrition Educator in the Casselberry or Sanford location of the DOH-Seminole WIC Program and reports directly to the Senior Public Health Nutritionist Supervisor. Responsibilities include but are not limited to certifying eligible clients into the WIC Program, providing nutrition counseling and appropriate health related information to clients. Referrals to other programs which may include Breastfeeding, Healthy Start, Prenatal Clinic and any other social service agencies as needed. This individual may interact with Public Health Nutrition Program Director, Sr. Public Health Nutritionist, Staff Assistants (Clerical Supervisors), and other WIC staff nutritionists, clerical and breastfeeding staff and occasionally the Executive Director of Nursing. This position must always function under the direction of a licensed nutritionist. Evaluates the clinical and anthropometric data and dietary histories of women, infants and children who participate in the WIC Program. Reviews growth charts with client / caretaker, completes the medical and nutrition questions in FL-WiSE, assigns appropriate risk codes and food packages. Follows state guidelines regarding approval of prescriptions for non-contract and exempt formulas. Ensures that appropriate notes/alerts are placed and that high-risk follow-ups are scheduled and completed when required. Client services are provided efficiently and with regard to the timely flow of the clinic and will be measured according to established productivity standards. Provides individual nutritional counseling and teaches group nutrition classes to pregnant women, breastfeeding women, women up to six-month post-partum and caretakers of infants and children participating in the WIC Program. Documents nutrition counseling in a nutrition education note and/or care plan, as appropriate, and enters at least one individualized goal. Enters service codes into FL-WiSE by the end of the day. Promotes breastfeeding to WIC clients. Ensures that the benefits of breastfeeding are discussed with prenatal clients and that they are provided with the appropriate educational materials and that this is documented in the nutrition education note or care plan. Follows protocol for scheduling prenatal clients for infant feeding class. Helps to ensure that breastfeeding moms and babies receive the needed contacts with the lactation staff and that timely follow-ups are scheduled according to protocol and individualized requests of lactation staff. Accurately completes and submits certified employee activity reports (EARs) and People First timesheets by required deadlines. Ensures that time is recorded accurately, and that EARs and timesheets match prior to submitting to supervisor. Represents DOH-Seminole WIC Program through active participation in outreach activities / community events that will promote increased WIC participation; attends at least one event per year. Trains and educates clerical staff on the quarterly Second Nutrition Education kits. Assists with training of new WIC staff, as needed. Participates in nutrition and breastfeeding education activities and completes all periodic DOH and WIC required training. Provides quarterly 2nd Nutrition Education Kits and training to Health Support Specialist staff. May be required to perform the following additional duties in order to cover shortages, as needed: Lab duties in the absence of the Health Support Technician (height/length and weight measurements as well as finger stick for Hemoglobin); entering demographic data, issuing EBT card benefits and scheduling appointments. Performs other duties, as assigned, to support the mission of the Department.
