Within DCF, the Substance Abuse and Mental Health (SAMH) Program office is recognized as the single state authority for substance abuse and mental health services. The Office of SAMH is statutorily responsible for the planning, evaluation, and implementation of a comprehensive statewide system of care for the prevention, treatment, and recovery of children and adults with serious mental illnesses or substance use disorders.
Northeast Florida State Hospital (NEFSH) is a 613-bed, licensed mental health treatment facility that provides person-centered services to adults suffering from severe and persistent mental illness. The hospital, located in Macclenny, was established in 1959 and is the largest state operated civil treatment facility.

This is a highly responsible and professional position serving as the RegisteredNurse Specialist OPS within the Nursing Division. The ideal candidate will be a collaborative team player who works seamlessly across all sections within the program office. This role requires the use of independent judgment, a proactive approach to responsibilities, and exceptional attention to detail. Specific Duties and Responsibilities include:
* Participates in a comprehensive, multi-discipline, outcome oriented, treatment team approach to resident care.
* Evaluates and plans for individual resident treatment programs.
* This includes completing nursing care plan within twenty-four hours following admission assessment and to be reviewed and updated as per policy.
* Participates in multi-discipline treatment team in the evaluation and planning of the Service Implementation Plan (SIP).
* Provides input into the formulation of the SIP.
* Encourages resident interactions. Increases the number of residents served by encouraging residents to participate in therapies and activities.
* Provides for residents' physical wellbeing, health and safety.
* Assesses resident and determines extent of assistance needed in hygiene, feeding, etc.
* Establishes and monitors standards of hygiene.
* Determines monitoring necessary for residents' wellbeing, i.e. weight, vital signs, medication reaction, sleeping, and eating habits.
* Arranges for x-rays and lab work as needed.
* Encourages proper nutrition by residents.
* Within one hour after reporting for duty, makes rounds on all assigned wards.
* Makes regular rounds on all assigned wards.
* Identifies physical symptoms and changes and initiates appropriate nursing intervention.
* Visually checks all residents in restraints and/or seclusion.
* Checks orders, progress notes, and flow sheets.
* Administers or supervises administration of treatments.
* Demonstrates understanding of medical asepsis, practices principles of infection control and provides inservice to other health care personnel.
* Recognizes hazards to resident safety and takes appropriate action to remedy the situation.
* Remains on assigned wards.
* May be assigned to other units for coverage by the Senior RegisteredNurse Supervisor in order to reduce controllable expenses.
* Is an active member of the Emergency Response Team and carries the code beeper as assigned.
* Adequately documents resident care.
* Completes nursing progress notes as assigned.
* Records in progress notes all incidents, medication and treatment, and their effects.
* Completes administrative forms as required.
* Responsible for administration of medication.
* Reviews residents' response to medication and reports to the physician on a regular schedule.
* Assesses residents need for PRN medication.
* Administers prescribed medication and treatment.
* Administers all injectable medication as ordered.
* Properly documents medication given.
* Responsible for ordering, administering, and count of controlled substances.
* Provides inservice and reviews all aspects of medication with staff on an ongoing basis.
* Maintains cleanliness of cart/cassettes.
* Does not keep extra or outdated medication on the ward.
* Checks charts for new orders on a regular schedule.
* Transcribes physician orders.
* Orders and receives prepackaged unit dose medications from the Pharmacy.
* Reduces length of stay by providing for rehabilitation of residents through health education and training.
* Determines education necessary for residents' independence and assigns level of staff to teach the resident.
* Teaches resident self-assistance skills as needed.
* Determines methods to be used to enhance independence.
* Evaluates effectiveness of methods and revises plans when necessary.
* Conducts and participates in individual/group sessions including medication education groups.
* Observes and reports resident behavior and illnesses.
* Counsels with residents on problems relating to health or wellbeing.
* Ensures optimum customer satisfaction by communicating with others to provide for adequate care of residents.
* Keeps Nurse Supervisor informed of but not limited to admissions; transfers; physically ill residents; residents placed in and out of restraints; residents placed in and out of seclusion; unauthorized departures; residents receiving injuries; rapes; discharges; suicidal and homicidal attempts; and deaths.
* Records this information in the Nurses Logbook in the Nursing Office.
* Participates in unit meetings, weekly nurse's meetings, designated hospital and service-related meetings.
* Communicates with staff regarding unit problems affecting resident care.
* Nurses working on weekends, evenings, nights, and holidays will notify the Nurse Supervisor that they are on duty before the previous shift's nurses go off duty.
* Maintains professional accountability by knowing policies, procedures, and standards pertaining to resident care.
* Responsible for own growth, development, and refining of knowledge in both the nursing and mental health fields by participation in inservice programs, continuing education, and attendance at nurse's meetings.
* Maintains current registerednurse license.
* Arranges with the Nurse Supervisor for mandatory training.
* Performs other duties as assigned.

Where You Will Work:
The Bureau of Field Operations is responsible for the survey and certification process of health care facilities across the State of Florida.
These surveys ensure that all state licensed, and federally certified health care facilities meet the established state and/or federal requirements and patients and residents receive safe, quality health care.
Field Operations staff also ensure the safety of residents in health care facilities when emergencies such as natural disasters occur.
The Work You Will Do:
The primary function of the RegisteredNurse Specialist is to conduct inspections utilizing an established survey process.
The survey process includes evaluation of health care facilities for state licensure and federal certification requirements for participation under Medicare/Medicaid.
The incumbent will document findings and complete written reports of survey findings.
The incumbent may be required to perform surveys as an individual, team member or team leader.
As assigned by management, this position is required to successfully complete the Surveyor Minimum Qualifications Test within the first 12 months of employment as required by Sections 1819(g)(2)(c)(ii) of the Social Security Act, as amended, and Article IV(B) of the Agreement pursuant to Section 1864 of the Social Security Act.
This position has been identified as mission essential.
The incumbent in this position may be required to work during the weekend or on holidays.
In addition, mission essential personnel will be required to work during disasters, to include but not limited to, work before, during and/or beyond normal work hours or days in the event of an emergency.
Emergency work may involve the incumbent to work in another county or staffing location to assist other State Agencies with emergency work.
Emergency duties may include, but not limited to, responses to or threats involving any disaster or threat of disaster, man-made or natural.
In accordance with 400.275, F.S., each member of a nursing home survey team who is a health professional licensed under Chapter 464 of the Florida Statutes shall earn not less than 50 percent of required continuing education credits in geriatric care.
The incumbent must be proficient with computer equipment and software programs, such as Microsoft Word and Outlook, as well as smart phone technology.
The incumbent must also be available to work before and after normal work hours including weekends and holidays.
This position may require extensive travel, including significant overnight travel.
Successful completion of a criminal background investigation is a condition of employment.
A good attendance record is essential for any individual in this position as the work involved occurs daily and is time-critical.
The individual in this position is expected to report to work daily and on time.
Leave must be requested in advance except in emergency circumstances not under the control of the employee.

How you will make an impact
The incumbent of this position provides age/special needs appropriate nursing care to school age student patients ranging from preschool to 22 years of age. Including assessment of student patient needs specific to their age. Nursing care is planned, interventions are performed to assure those needs are met and evaluated for effectiveness of nursing care. Health care services are provided according to physician orders, nursing clinical guidelines, FSDB and HCC policies and procedure and in accordance with FS 464 and FAC 64B9.Including assessment of student patient needs specific to their age.

Your Specific Responsibilities:
This position is a professional nursing position providing support services and subject matter expertise for public health nursing operations including family planning, infectious disease management, public health response, and clinic operations. This position reports directly to the Executive Community Health Nursing Director. The position must be able to work independently requiring little supervision and be self-motivated. This position will work at the Florida Department of Health - Holmes and Washington County Health Departments, as scheduled by the Supervisor. The below duties and responsibilities will be carried out for each county stated above.
Works in general clinic to include, but not limited to, family planning, STD screenings and treatment, immunization administration, medication administration, assists provider, as indicated. Documents and maintains appropriate health records in the Health Management System (HMS) and Florida Shots, as indicated. Performs diagnostic and/or screening test as ordered by the provider. Obtains/prepares laboratory specimens as indicated.
Back-up for disease surveillance, investigation, reporting and follow-up when primary staff is unavailable. Enters information as indicated into the Merlin System. May at times be required to monitor systems after-hours when primary staff are out of office.
Reviews non-compliant Healthcare Effectiveness Data and Information Set (HEDIS) list, supplied by Billing Clerks at Holmes and Washington. Sends reminder letters each month as indicated (by the 25th of each month) to the client and documents in HMS. Coordinates with Insurance representative to review assigned list of clients to ensure Gare Gaps are met. Submits records to insurance to ensure Care Gaps are met.
Back-up for the Tuberculosis (TB) Program when primary staff is unavailable. Conducts interview of patient to obtain history of possible exposure, symptoms, skin test and or IGRA history, chest x-rays and contacts. Follows and manages clients found to have active TB and Latent Tuberculosis Infection (LTBI) based on policy and procedure. Communicates with regional TB liaison regarding client tracking and treatment. Updates, along with the Executive Community Health Nursing Director, the Policy and Procedure annually. This will be done by May 30th of each year.
Responsible back-up for the AIDS Drug Assistance Program (ADAP). Completes eligibility using Provide and CareWare database as indicated. Participates in ADAP conference calls.
Participates in community outreach activities and disaster relief efforts as assigned.
Serves as the back-up Information Custodian for Epidemiology (EPI), HIV/AIDS, ADAP, and Tuberculosis. Responsibilities include maintaining information in accordance with state and federal statutes, rules regulations and Department of Health Policies, Protocols, and Procedures.
Assists clinic RN with sending immunization reminders to parents/guardians to ensure two-year-old immunization goal is 95% or greater. (State target 95%).
Participates in mandatory programmatic conference calls for improvement and updates. Participates in educational training and staff meetings. Obtains training for professional development and administrative knowledge of program requirements. Attends workshops, meetings, and in-services to maintain license and advance knowledge level. Packaging and Shipping Certification required.
Serves as back-up for Vaccine for Children (VFC) Coordinator for Washington and Holmes CHD. This includes assisting in maintenance of VFC temperature logs, log tags and Florida Shots records. Will respond to emergency notifications received by the Sensaphone/Verilert notification system as indicated. Will work with VFC coordinator to prevent vaccine expiration and wastage, may assist with vaccine transportation and/or relocation.
Attends workshops, meetings, and in-services for professional growth. Participates in community outreach activities and disaster relief efforts as assigned.
Complete Employee Activity Report (EARS) within required timeframe.
