Position Overview
The Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a Health Facilities Surveillance Nurse in the Division of Assisted Living. This individual will conduct surveys of facilities, including state licensure and complaint surveys. The HFSN will be responsible for preparing written survey documentation. Under general supervision, surveys facilities to determine compliance with state licensure requirements and Department directives, program policies and procedures. Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided at facilities. Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance.

Essential Functions
* Under general direction, surveys Assisted Living and Shared Housing Establishments to determine compliance with state licensure requirements, and Department directives, program policies and procedures.
* Prepares written survey findings.
* Performs monitoring functions at noncompliant facilities.
* Completes necessary surveyor training.
* Provides technical assistance to facilities in emergencies.
* Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed.
* Performs other duties as required or assigned within the scope of the responsibilities enumerated above.
Minimum Qualifications
* Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience or, (b) a bachelor's degree in nursing and two (2) years of professional nursing experience or, (c) a master's degree in nursing.
* Requires licensure as a Registered Nurse in the State of Illinois.
Preferred Qualifications
* One (1) year experience in assisted living care setting.
* Three (3) years professional nursing experience.
* Two (2) years' experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints, and incident investigation surveys.
* One (1) year experience preparing complex written & oral reports.
* One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel PowerPoint, Smartsheet).
* One (1) year experience exercising judgement, discretion and maintain confidentiality during the conduct of the survey process.

Key Responsibilities
Perform telephone triage using established protocols and sound nursing judgment.

Qualifications
Required:

Position Overview
The Division of Developmental Disabilities is seeking to hire an independent, highly organized, and detail-oriented licensed Registered Nurse for the Kiley Developmental Center located Waukegan, Illinois. This position will be responsible for performing direct nursing care for people with Developmental and Intellectual Disabilities. The successful candidate shall demonstrate good oral and written communication skills, be a self-starter, and be able to carry out nursing assignments pursuant to the direction of medical personnel.
Essential Functions
Provides professional nursing care to individuals served on an assigned shift.
Provides work-related guidance, instructions and explanations to lower-level nursing staff and other direct support staff in the care of individuals, injury reduction, health and safety, hygiene, treatment, procedures, and the use of equipment.
Attends treatment team meetings to update the Interdisciplinary Team on changes in medical status, additional medical needs and completes paperwork to document medical information in accordance with policies and guidelines.
Performs quality assurance activities including conducting audits and preparing Infection Control Reports.
Attends training to keep abreast of latest nursing techniques and concepts.
Serves as Center-wide medical/nursing resource on the assigned shift during the absence of other professional nursing personnel.
Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above.

Essential Functions:

Bachelor's prepared registered nurses will be expected to attain the RN-IEP designation or Certified School Nurse (CSN) certification within 2 years. The nursing assignments at SEDOL may require travel. Fluency in English and Spanish is preferred.
Coordination:
1.Administer a coordinated school health program in cooperation with the school nurse.
2.Collect and analyze epidemiological and other school health information.
3.Assess the health and safety needs of the school environment; implement bloodbornepathogens control and reinforce universal precautions.
4.Participate in maintaining school health records and promote compliance with health-related regulations such as immunizations and school physicals as outlined in the IllinoisSchool Code and by the Illinois Department of Public Health.
5.Participate in the development of health-related policies and procedures in compliancewith state mandates/current health practices in cooperation with the school nurse.
Clinical Practice:
1.Provide direct nursing services, first aid, illness, and emergency care to students includingnursing assessment, identifying health problems, making referrals for diagnosis and treatment,recommending educational modifications, providing follow-up and evaluation, and maintainingappropriate documentation.

6.Make appropriate assessments and referrals for suspected abuse/neglect as a mandatedreporter.
7.Accompany students on mobility trips as indicated by their medical status or IEP.
8.Provide nursing assistance to others in the department: 1:1 nurse, SEDOL program nursesand agency nurses.
9.A registered nurse may be assigned as a 1:1 nurse, if the IEP team determines that astudent is so medically fragile that continual monitoring of health needs and/or equipment isrequired. This 1:1 assignment may also include the bus trip to and from school.
Health Office Management and Communication:
1.Maintain an organized and efficient health services facility.

Position Overview
The Illinois Department of Public Health is seeking a highly motivated, detailed orientated individual to serve as a Health Facilities Surveillance Nurse in the Division of Field Operations. This individual will conduct surveys of facilities, including state licensure, federal certification and complaint surveys. The HFSN will be responsible for preparing written survey documentation. Under general supervision, surveys facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures. Travels to conduct routine health and safety surveys to determine compliance with state and federal requirements. Interviews facility residents, staff, administrators, health care providers and other individuals. Observes patient care provided at facilities. Reviews medical records, policies, standard operating procedures and other facility documents related to licensure and/or regulatory compliance.

Essential Functions
* Surveys long term care facilities to determine compliance with state licensure requirements, federal certification mandates, and Department directives, program policies and procedures.
* Prepares written survey findings.
* Performs monitoring functions at noncompliant facilities.
* Completes necessary surveyor training.
* Provides technical assistance to facilities in emergencies.
* Participates in staff meetings and assist with orientation of newly employed health facilities surveillance nurses and other Department staff as directed.

Minimum Qualifications
* Requires (a) graduation from an approved nursing education program resulting in an associate or a diploma degree in nursing and three (3) years of professional nursing experience or, (b) a bachelor's degree in nursing and two (2) years of professional nursing experience or, (c) a master's degree in nursing.
* Requires licensure as a Registered Nurse in the State of Illinois.
Preferred Qualifications
* One (1) year experience in long-term care setting.
* Three (3)years professional nursing experience.
* Two (2) years experience participating in the survey process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints and incident investigation surveys.
* One (1) year experience preparing complex written & oral reports.
* One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel PowerPoint, Smartsheet).
* One (1) year experience exercising judgement, discretion and maintain confidentiality during the conduct of the survey process.
* Two (2) years of experience establishing, maintaining, and improving cooperative working relationships with various stakeholders.
