DESCRIPTION: This is professional nursing work involving the care and treatment of hospital patients who are emotionally, mentally or physically ill. Supervision may be exercised over a staff of professional and paraprofessional personnel. Work is performed under limited supervision. REPRESENTATIVE TASKS: Plans, assigns, oversees, and evaluates the work of hospital professional and paraprofessional employees in giving medical and direct care to residents of hospitals in order to ensure compliance with established standards. Assesses residents' physical and mental state and recommends possible treatment in order to apprise the physician of client's physical and mental condition. Assigns, leads, and monitors direct care personnel in performing nursing functions in order to comply with prescribed physician orders. Plans, develops, and implements individual treatment plans (ITP) for patients upon identification of problems in order to establish individualized treatment goals. Instructs direct care staff in nursing procedures and techniques in order to ensure knowledgeable and effective personnel. Examines medication records in order to determine whether medications have been properly and accurately dispensed. Leads treatment team meetings in order to discuss and share information regarding patient progress and needs and facility management problems. KNOWLEDGES, SKILLS, AND ABILITIES REQUIRED: Knowledge of professional nursing theory and practice. Knowledge of materia medica, hospital dietetics, sanitation, and personal hygiene. Knowledge of human anatomy and physiology. Knowledge of pharmacology. Knowledge of mental and physical health theory and treatment practices. Knowledge of group dynamics. Knowledge of patient behavioral problems and issues. Ability to instruct and supervise hospital staff nurses, assistants, and attendant personnel. You will apply proper nursing principles, practices, and techniques in the care and custody of hospital patients who are emotionally, mentally, and/or physically ill.
These positions require a highly-motivated individual who can adapt quickly in a fast paced, demanding work situation. You must demonstrate a high level of critical-thinking skills that allow you to process a large amount of information, discern the overall scope of a situation, and make sound decisions based on the information, experience, and knowledge of systems and your scope of practice.
You must demonstrate good communication skills, exceptional attention to detail, and be able to articulate and employ the Mission, Vision, and Values of DDPC with our staff as well as with community providers.
To be successful you must be able to:
* Plan and oversee work on the unit including psychiatric nursing care in accordance with nursing practices.
* Administer medications and treatment to ensure patients receive proper medication and treatment in a timely manner.
* Write patient progress reports to maintain patient medical records and document unusual behavior.
* Assess emergency situations and take appropriate action to overcome and resolve patient issues.
* Intervenes therapeutically when patient exhibits signs of behavioral escalation to overcome situation.
* Assesses patient's health status, plans care, or makes appropriate referrals to ensure patient needs are met.

The Opportunity Alliance seeking a Full Time Residential Nurse (RN) or Licensed Practical Nurse (LPN) to support our Ocean Street adult residential program.
The Ocean Street Residential Program, with a capacity of eight residents, provides trauma-informed treatment and support services to adults living with mental illness complicated by co-occurring chronic health conditions as well as a potential substance use disorder. In a supportive, safe, and secure environment, the program meets the needs of individuals who have a major mental illness by providing psychosocial rehabilitation services and personal care services.
The Residential Nurse/ LPN contributes to the accomplishment of the Agency's mission through the evaluation of medication management practices, other pertinent health education programming, direct care of client medical needs as appropriate to the scope of a RN or LPN, care coordination of physical health needs with medical providers, as well as the shared oversight of the health and well-being of clients within Residential Treatment Services.
This position is a member of the Adult Residential Treatment Team. Residential Treatment serves adults living with serious and persistent mental illness, substance misuse, and may be experiencing homelessness. The Residential Nurse will partner closely with the Director to achieve positive program and client outcomes. The Residential Nurse will work within an interdisciplinary team approach to support integrated health outcomes for clients with mental illness complicated by core morbid chronic health conditions who may also have a substance use disorder. The Residential Nurse will utilize the recovery oriented, trauma-informed, and person-centered approach to care.
Primary responsibilities:
Providing direct nursing care, within the scope of RN practice, and as medically ordered, for clients in the residential program;
Support integrated physical health care, assist with admission and discharge, providing care transition planning and care coordination and monitor medication procedures
Contributing to the achievement of positive client outcomes and results by working closely with Residential Director and developing wellness programming
Providing training, teaching, coaching, instruction, consultation and health education for residential staff that contributes to the achievement of positive client outcomes
Schedule: This is a full-time, 40-hours/week position

Qualifications:
Licensed practical nurse holding a valid license to practice nursing in the State of Maine, and license must be in good standing are required; with two (2) to four (4) years of experience required, with recommendation that one of those years being in clinical practice in metal health nursing, or Registered Nurse holding a valid license to practice nursing in the State of Maine, and license must be in good standing are required; with two (2) to four (4) years of experience required, with one of those years being in clinical practice in metal health nursing.
Previous training experience in care coordination and/or integrated care is preferred.
Experience within the Mental Health and Substance Use delivery system and/or direct service is preferred.
Experience in the education and clinical care of clients with diabetes is a plus.
Advanced computer skills in programs including Microsoft Word, Excel, and PowerPoint required.
Strong written and oral communication and interpersonal skills required
Strong attention to detail required.
Experience in team or multidisciplinary approach to service planning is preferred.
Must be able to able to successfully pass a criminal background, child protective service check & sex offender check and must not be on the state or federal suspension and disbarment list.
Must be able to exert moderate physical effort with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, typing and frequently lift and carry material weighing up to 50 pounds.
Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to agency location to complete work. A good driving record is required.
