The Division of Public Health Nursing, within the Maine CDC, provides nursing services to individuals, families, and population groups statewide. Public health nursing is the practice of promoting and protecting the health of populations using knowledge from nursing, social, and public health sciences. Nursing services include maternal and child health support, adult health services, communicable disease control, lead poisoning management, asthma management, immunizations, and migrant and refugee health services.
As a Public Health Nurse I, you will provide care to new or expecting parents, older adults, individuals diagnosed with tuberculosis or latent tuberculosis, and community groups. Your responsibilities include care planning and management, conducting home and field visits, supporting clinics and conferences, and advising and assisting civic groups on nursing and public health needs. As a Public Health Nurse I, you will often collaborate and consult with internal and external DHHS partners to achieve desirable outcomes for families and communities. Work is performed under limited supervision.
This is generally a Monday through Friday, 8AM - 5PM position requiring you to travel within the assigned geographic area. Travel reimbursement is provided
Core Responsibilities:
* Apply the nursing process to individuals and families including health assessment, nursing diagnoses, care plan development, implementation, evaluation, resource coordination, and/or referral(s) to meet demonstrated health care needs.
* Observe the home environment, identify health problems or at-risk behavior, and contact appropriate resources and agencies to respond to family and client needs.
* Document assessments and interventions in medical records. Use established medical record software, forms, and documentation practices.
* Plan, develop, and coordinate client care with hospitals, medical providers, schools, and social services agencies to ensure necessary services are accessible.
* Support Division goals, objectives, and standards, including practicing evidence-based, best practice nursing care.
* Participate in regular staff meetings to share information and discuss client(s) status.
* Provide updates, reports, and case reviews, as needed, to the PHN District Supervisor.
* Participate in all mandatory Division trainings and meetings.
* Occasional statewide travel is required.
* Assume responsibility for your own professional growth and development by pursuing education, participating in professional committees and work groups.
* Comply with State of Maine human resource laws, policies, procedures, and practices.
Minimum Qualifications:
Graduation from an accredited school of nursing.
LICENSING/REGISTRATION/CERTIFICATION REQUIREMENTS: (These must be met by all employees prior to attaining permanent status in the class).
Licensed as a Registered Professional Nurse as issued by the State Board of Nursing.
Preferred candidates will also have:

The position of Nurse Care Manager is a member of the Behavioral Health Home (BHH) Adult Service Team and the BHH Children's Service Team. Qualifications:
Registered Nurse holding a valid license to practice nursing in the State of Maine, the license must be in good standing; with two (2) of experience required.
Experience with and knowledge of people experiencing mental health and substance use issues, poverty, homelessness, chronic disease, domestic violence, trauma and/or oppression very preferred.
Knowledge of, commitment to or proficiency in recovery-based, harm reduction, trauma-informed and strengths-based approaches preferred.
Familiarity with utilizing and reporting health population data.
Experience with integrated healthcare for adults is preferred but not required.
Experience in electronic data collection and reporting is very helpful. Comfort and experience with ECR is critical.
Appropriate licensing/credentialing to meet all MaineCare BHH/CBHH Regulations.
Computer proficiency in Microsoft Office is a requirement.
Must have a professional Nursing license that is current and in good standing.
Must be able to successfully pass a criminal background, child protective service check & sex offender check.
Must not be on the state or federal suspension and disbarment list.
Normal office conditions such as sitting, with occasional standing, walking, reaching, stooping, bending, kneeling, crouching, typing and lifting to 10 pounds.
Driving may be necessary to attend meetings and trainings and for the occasional need to meet with clients offsite and within the program service area. This may include transportation of clients or driving to the agency's location to complete work.

Job Overview:

The Division of Public Health Nursing, within the Maine CDC, provides nursing services to individuals, families, and population groups statewide. The Ocean Street Residential Program, with a capacity of eight residents, provides trauma-informed treatment and support services to adults living with mental illness complicated by co-occurring chronic health conditions as well as a potential substance use disorder. In a supportive, safe, and secure environment, the program meets the needs of individuals who have a major mental illness by providing psychosocial rehabilitation services and personal care services.
The Residential Nurse/ LPN contributes to the accomplishment of the Agency's mission through the evaluation of medication management practices, other pertinent health education programming, direct care of client medical needs as appropriate to the scope of a RN or LPN, care coordination of physical health needs with medical providers, as well as the shared oversight of the health and well-being of clients within Residential Treatment Services.
This position is a member of the Adult Residential Treatment Team. Residential Treatment serves adults living with serious and persistent mental illness, substance misuse, and may be experiencing homelessness. The Residential Nurse will partner closely with the Director to achieve positive program and client outcomes. The Residential Nurse will work within an interdisciplinary team approach to support integrated health outcomes for clients with mental illness complicated by core morbid chronic health conditions who may also have a substance use disorder. The Residential Nurse will utilize the recovery oriented, trauma-informed, and person-centered approach to care.
Primary responsibilities:
Providing direct nursing care, within the scope of RN practice, and as medically ordered, for clients in the residential program;
Support integrated physical health care, assist with admission and discharge, providing care transition planning and care coordination and monitor medication procedures
